Detection of diabetes from whole-body MRI using deep learning.

ABSTRACT: Obesity is one of the main drivers of type 2 diabetes, but it is not uniformly associated with the disease. The location of fat accumulation is critical for metabolic health. Specific patterns of body fat distribution, such as visceral fat, are closely related to insulin resistance. There might be further, hitherto unknown, features of body fat distribution that could additionally contribute to the disease. We used machine learning with dense convolutional neural networks to detect diabetes-related variables from 2371 T1-weighted whole-body MRI data sets. MRI was performed in participants undergoing metabolic screening with oral glucose tolerance tests. Models were trained for sex, age, BMI, insulin sensitivity, HbA1c, and prediabetes or incident diabetes. The results were compared with those of conventional models. The area under the receiver operating characteristic curve was 87% for the type 2 diabetes discrimination and 68% for prediabetes, both superior to conventional models. Mean absolute regression errors were comparable to those of conventional models. Heatmaps showed that lower visceral abdominal regions were critical in diabetes classification. Subphenotyping revealed a group with high future diabetes and microalbuminuria risk.Our results show that diabetes is detectable from whole-body MRI without additional data. Our technique of heatmap visualization identifies plausible anatomical regions and highlights the leading role of fat accumulation in the lower abdomen in diabetes pathogenesis.

Project description:BackgroundDeep convolutional neural networks have demonstrated robust and reliable PET attenuation correction (AC) as an alternative to conventional AC methods in integrated PET/MRI systems. However, its whole-body implementation is still challenging due to anatomical variations and the limited MRI field of view. The aim of this study is to investigate a deep learning (DL) method to generate voxel-based synthetic CT (sCT) from Dixon MRI and use it as a whole-body solution for PET AC in a PET/MRI system.Materials and methodsFifteen patients underwent PET/CT followed by PET/MRI with whole-body coverage from skull to feet. We performed MRI truncation correction and employed co-registered MRI and CT images for training and leave-one-out cross-validation. The network was pretrained with region-specific images. The accuracy of the AC maps and reconstructed PET images were assessed by performing a voxel-wise analysis and calculating the quantification error in SUV obtained using DL-based sCT (PETsCT) and a vendor-provided atlas-based method (PETAtlas), with the CT-based reconstruction (PETCT) serving as the reference. In addition, region-specific analysis was performed to compare the performances of the methods in brain, lung, liver, spine, pelvic bone, and aorta.ResultsOur DL-based method resulted in better estimates of AC maps with a mean absolute error of 62 HU, compared to 109 HU for the atlas-based method. We found an excellent voxel-by-voxel correlation between PETCT and PETsCT (R2 = 0.98). The absolute percentage difference in PET quantification for the entire image was 6.1% for PETsCT and 11.2% for PETAtlas. The regional analysis showed that the average errors and the variability for PETsCT were lower than PETAtlas in all regions. The largest errors were observed in the lung, while the smallest biases were observed in the brain and liver.ConclusionsExperimental results demonstrated that a DL approach for whole-body PET AC in PET/MRI is feasible and allows for more accurate results compared with conventional methods. Further evaluation using a larger training cohort is required for more accurate and robust performance.

Project description:ObjectivesTo investigate the diagnostic performance of deep transfer learning (DTL) to detect liver cirrhosis from clinical MRI.MethodsThe dataset for this retrospective analysis consisted of 713 (343 female) patients who underwent liver MRI between 2017 and 2019. In total, 553 of these subjects had a confirmed diagnosis of liver cirrhosis, while the remainder had no history of liver disease. T2-weighted MRI slices at the level of the caudate lobe were manually exported for DTL analysis. Data were randomly split into training, validation, and test sets (70%/15%/15%). A ResNet50 convolutional neural network (CNN) pre-trained on the ImageNet archive was used for cirrhosis detection with and without upstream liver segmentation. Classification performance for detection of liver cirrhosis was compared to two radiologists with different levels of experience (4th-year resident, board-certified radiologist). Segmentation was performed using a U-Net architecture built on a pre-trained ResNet34 encoder. Differences in classification accuracy were assessed by the χ2-test.ResultsDice coefficients for automatic segmentation were above 0.98 for both validation and test data. The classification accuracy of liver cirrhosis on validation (vACC) and test (tACC) data for the DTL pipeline with upstream liver segmentation (vACC = 0.99, tACC = 0.96) was significantly higher compared to the resident (vACC = 0.88, p < 0.01; tACC = 0.91, p = 0.01) and to the board-certified radiologist (vACC = 0.96, p < 0.01; tACC = 0.90, p < 0.01).ConclusionThis proof-of-principle study demonstrates the potential of DTL for detecting cirrhosis based on standard T2-weighted MRI. The presented method for image-based diagnosis of liver cirrhosis demonstrated expert-level classification accuracy.Key points• A pipeline consisting of two convolutional neural networks (CNNs) pre-trained on an extensive natural image database (ImageNet archive) enables detection of liver cirrhosis on standard T2-weighted MRI. • High classification accuracy can be achieved even without altering the pre-trained parameters of the convolutional neural networks. • Other abdominal structures apart from the liver were relevant for detection when the network was trained on unsegmented images.

Project description:PurposeTo develop a super-resolution technique using convolutional neural networks for generating thin-slice knee MR images from thicker input slices, and compare this method with alternative through-plane interpolation methods.MethodsWe implemented a 3D convolutional neural network entitled DeepResolve to learn residual-based transformations between high-resolution thin-slice images and lower-resolution thick-slice images at the same center locations. DeepResolve was trained using 124 double echo in steady-state (DESS) data sets with 0.7-mm slice thickness and tested on 17 patients. Ground-truth images were compared with DeepResolve, clinically used tricubic interpolation, and Fourier interpolation methods, along with state-of-the-art single-image sparse-coding super-resolution. Comparisons were performed using structural similarity, peak SNR, and RMS error image quality metrics for a multitude of thin-slice downsampling factors. Two musculoskeletal radiologists ranked the 3 data sets and reviewed the diagnostic quality of the DeepResolve, tricubic interpolation, and ground-truth images for sharpness, contrast, artifacts, SNR, and overall diagnostic quality. Mann-Whitney U tests evaluated differences among the quantitative image metrics, reader scores, and rankings. Cohen's Kappa (κ) evaluated interreader reliability.ResultsDeepResolve had significantly better structural similarity, peak SNR, and RMS error than tricubic interpolation, Fourier interpolation, and sparse-coding super-resolution for all downsampling factors (p < .05, except 4 × and 8 × sparse-coding super-resolution downsampling factors). In the reader study, DeepResolve significantly outperformed (p < .01) tricubic interpolation in all image quality categories and overall image ranking. Both readers had substantial scoring agreement (κ = 0.73).ConclusionDeepResolve was capable of resolving high-resolution thin-slice knee MRI from lower-resolution thicker slices, achieving superior quantitative and qualitative diagnostic performance to both conventionally used and state-of-the-art methods.
