Exploration of Estimated Emigration Trends of Polish Health Professionals.

ABSTRACT:

Background

Due to the significant staff shortages, emigration of health professionals is one of the key challenges for many healthcare systems.

Objective

The aim of this article is to explore the estimated trends and directions of emigration among Polish health professionals.

Methods

The emigration phenomenon of Polish health professionals is still under-researched and the number of studies in this field is limited. Thus, the authors have triangulated data using two methods: a data analysis of five national registers maintained by chambers of professionals (doctors, nurses, midwives, physiotherapists, pharmacists, and laboratory diagnosticians), and data analysis from the Regulated Profession Database in The EU Single Market.

Results

According to the data from national registers, between 7-9% of practicing doctors and nurses have applied for certificates, which confirm their right to practice their profession in other European countries (most often the United Kingdom, Germany, Sweden, Spain, and Ireland). The relatively high number of such certificates applied for by physiotherapists is also worrying. Emigration among pharmacists and laboratory diagnosticians is rather marginal.

Conclusions

Urgent implementation of an effective mechanism for monitoring emigration trends is necessary. Furthermore, it is not possible to retain qualified professionals without systemic improvement of working conditions within the Polish healthcare system.

Emigration from the perspective of Polish health professionals – insights from a qualitative study

Project description:The Polish health workforce is facing many problems, including shortages, heavy workload, burnout, and dissatisfaction, which may lead to job emigration, mainly among younger generations. The objective of the article is to explore the reasons, consequences, and circumstances of the emigration of Polish health professionals with a qualitative approach. We have conducted semi-structured interviews with 15 practicing health professionals, exploring the perspectives of different professions and different generations. The interviews were conducted using the on-line MS Teams platform from February until March 2022. We transcribed all interviews verbatim and performed directed qualitative content analysis. Currently, the scale of emigration is rather stable and mainly affects the younger generations. The main reasons for emigration, as indicated by representatives of different health professions, are similar, including excessive workload and long working hours. However, doctors and dentists also emphasized problems with professional development and specialization training, while nurses, midwives, and physiotherapists underlined problems with low prestige and work autonomy. Taking into account the substantial shortage of health workforce and the high average age of practicing health professionals, emigration requires special attention from health decision makers. Implementation of an effective mechanism for monitoring the emigration of medical staff is recommended.

Nursing emigration in the United Kingdom: A qualitative exploration of the Spanish nursing community.

Project description:AimTo understand and describe the experiences and perceptions of migrant Spanish nurses working in the UK.DesignOnline survey with open-ended questions.MethodsA total of 371 participants were recruited from online groups related to Spanish nurses working in the UK.ResultsFour themes emerged from their content analysis: "Motivations for emigration: running away from job insecurity and unemployment," "Professional recognition," "Seeking better life stability" and "Future neediness: between a rock and a hard place." In these results, professional recognition was highlighted as a reason to stay in the host country, although the uncertainty represented by Brexit is reversing the migration trend that began years ago. Policymakers should be aware of these experiences to promote strategies and programmes, which encourage inclusion in the host country and will help these nurses in their transition process.

Experience of Polish Patients with Obesity in Contacts with Medical Professionals

Project description:Purpose Discrimination and stigmatization of patients with obesity are a commonly occurring social problem. The purpose of our research was to analyze the scale of the experience including medical staff’s improper behaviours towards patients with obesity in Poland. Patients and Methods In a completed national study, we studied the statements of 621 adult patients who suffer from obesity. An original closed question survey was used as a tool to collect the data. Patients were informed about the possibility to participate in the study through social media, medical institutions and patient foundations. Results As many as 82.6% have experienced improper behaviours. Usually, it came from doctors (90%), nurses and midwives (51%), people who operated medical equipment (24%), nutritionists (14%) and paramedics (9%). Exactly 81% of the respondents pointed to unpleasant and judgmental comments as the most frequent form of improper behaviour which they have encountered mainly during diagnostic tests, palpation or procedures. Conclusion There is an urgent need for developing national strategies connected with care for individuals with higher body weight. Introducing dedicated solutions in this field may contribute to increasing the quality of health care and reducing stigmatizing behaviours.

An exploration of genetic health professionals' experience with direct-to-consumer genetic testing in their clinical practice.

Project description:Direct-to-consumer genetic testing (DTC-GT) allows individuals to obtain genetic tests directly from companies without necessarily involving health professionals. This study explores genetic health professionals' opinions of health-related DTC-GT and the reported frequency of individuals presenting to clinical genetics services after undertaking testing. Genetic counsellors and clinical geneticists, members of the Human Genetics Society of Australasia, completed an online survey in mid 2011. The 130 genetic counsellors (estimated response fraction=43%) and 38 clinical geneticists (estimated response fraction=46%) had mixed opinions regarding DTC-GT, with only 7% confident in accurately interpreting and explaining DTC-GT results. Nineteen respondents (11%) reported one or more client(s) referred to them after undertaking DTC-GT. Descriptions of 25 clients were extracted from responses, and respondents reported that all clients were concerned for the health of either themselves or family members. Most clients presented to genetic clinics specifically as a result of their DTC-GT (96%) and were self or GP referred (92%). Respondents perceived that their clients typically undertook DTC-GT because they wanted to identify monogenic conditions, including carrier testing and/or know their susceptibility or predisposition for complex conditions (88%). The majority of clients needed help interpreting DTC-GT results (80%), however in general were not questioning the validity of their DTC-GT results (92%) nor seeking further genetic testing (84%). Currently, DTC-GT is not a major reason for referral to clinical genetics services in Australia and New Zealand and the majority of genetic health professionals lack confidence in being able to accurately interpret and explain DTC-GT results.

Approaching discussions about genetics with palliative patients and their families: a qualitative exploration with genetic health professionals.

Project description:Genetic information can provide clinical benefits to families of palliative patients. However, integration of genetics into mainstream medicine has not focused on palliative populations. We explored the views and experiences of genetic health professionals in addressing genetics with palliative patients, and their families. We conducted an interpretive descriptive qualitative study with genetic counsellors and clinical geneticists using interviews and focus groups. Findings were generated using reflexive thematic analysis. Three themes were identified: (1) Focusing on the benefit to the family, (2) The discomfort of addressing genetics near end-of-life and (3) "It's always on the back-burner": Challenges to getting genetics on the palliative care agenda. Participants discussed the familial benefit of genetics in palliative care alongside the challenges when patients are near end-of-life. They perceived genetics as low priority for palliative care due to misunderstandings related to the value of genetic information. Acknowledging the challenges in the palliative care context, genetic health professionals want improved service leadership and awareness of the familial benefits of palliative genetic testing. Strong leadership to support genetic health professionals in addressing these barriers is needed for the benefits of genetic information to be realised.

Longitudinal and secular trends in dietary supplement use: Nurses' Health Study and Health Professionals Follow-Up Study, 1986-2006.

Project description:Most studies on the prevalence of supplement use in the United States were cross-sectional or evaluated trends in limited variety of supplements. The objective of this study was to describe the longitudinal and secular trend of dietary supplement use over the past 20 years in health professionals using data from two large prospective cohorts. We analyzed cohort data from 1986 to 2006 in the Nurses' Health Study (NHS) and the Health Professionals Follow-Up Study (HPFS). In 1986, 74,194 women aged 40 to 65 years in the NHS and 50,497 men aged 40 to 75 years in the HPFS were included. Use of dietary supplements including multivitamins, vitamins, and minerals was repeatedly asked every 4 years. Generalized estimating equation models were used for repeated analysis. Prevalence of use of any supplement increased among both women (71.3% to 88.3%) and men (56.4% to 80.7%) from 1986 to 2006. Notably, longitudinal increases in the prevalence of use of vitamin D (2.2% to 32.2% for women and 1.1% to 6.7% for men), folic acid (0.8% to 10.7% for women and 1.1% to 13.8% for men), and fish oil (1.6% to 18.1% for women and 3.3% to 22.2% for men) supplements were observed from 1990 to 2006. However, the use of vitamin A, beta carotene, vitamin C, and vitamin E supplements peaked in 1994 or 1998, then declined steadily. A secular increase in use of multivitamins, vitamin D, folic acid, and fish oil across same age group was noted. In conclusion, the use of many types of dietary supplements has increased over time, but the use of antioxidant supplements has declined. The secular increase in the prevalence of use of supplements across the same age group suggests that aging of the population is not the primary reason for the increase. These findings in health professionals need to be replicated in the general populations.

Qualitative exploration of health professionals' experiences of communicating positive newborn bloodspot screening results for nine conditions in England.

Project description:ObjectiveTo explore health professionals' experiences of communicating positive newborn bloodspot screening (NBS) results, highlight differences, share good practice and make recommendations for future research.DesignQualitative exploratory design was employed using semi-structured interviews SETTING: Three National Health Service provider organisations in England PARTICIPANTS: Seventeen health professionals involved in communicating positive newborn bloodspot screening results to parents for all nine conditions currently included in the newborn bloodspot screening programme in England.ResultsFindings indicated variation in approaches to communicating positive newborn bloodspot screening results to parents, largely influenced by resources available and the lack of clear guidance. Health professionals emphasised the importance of communicating results to families in a way that is sensitive to their needs. However, many challenges hindered communication including logistical considerations; difficulty contacting the family and other health professionals; language barriers; parental reactions; resource considerations; lack of training; and insufficient time.ConclusionHealth professionals invest a lot of time and energy trying to ensure communication of positive newborn bloodspot screening results to families is done well. However, there continues to be great variation in the way these results are communicated to parents and this is largely influenced by resources available but also the lack of concrete guidance. How best to support health professionals undertaking this challenging and emotive task requires further exploration. We recommend evaluation of a more cohesive approach that meets the needs of parents and staff while being sensitive to the subtleties of each condition.Trial registration numberISRCTN15330120.

Growth change in Polish women: Reduction of the secular trends?

Project description:The aim of the study was to analyse changes in the average height of adult Polish women born in 1931-2001 in the aspect of dynamically changing economic and socio-economic conditions of the living environment. An ethnically homogeneous group of 6,028 adult women from large Polish cities, born in 1931-2001, living between 1931 and 2020, were examined using the same research methods and research equipment. All women were divided into eight birth cohorts. The Kruskal-Wallis test and multiple regression analyses were used. Root Mean Square Standardized Effect (RMSSE), critical value of the test, and test power were calculated. The average height of women born during 70 years of the study increased by 9.63 cm, from 158.22 cm (SD = 5.57 cm) to 167.85 cm (SD = 6.91 cm) (H = 1084.84, p<0.001). The intensity of the intergenerational trend in subsequent cohorts of years of birth varied strongly between decades, averaging 1.34 cm/decade. The body height in women increased significantly up to the height of those born between 1970 and 1979 and then the trend weakened noticeably, although it remained positive. The observed secular trend confirms positive changes in the standard of living of Polish women between 1931 and 2020. Improving living conditions allow people to fully achieve their genetically determined growth potential.
