The Role of Ranged Horses in Eco-Epidemiology of Rickettsia raoultii Infection in China

ABSTRACT: Rickettsia raoultii is a tick-borne pathogen that infects humans; however, the vertebrate hosts of this pathogen have not been clearly defined. Our molecular examination of Rickettsia spp. infecting mammals and ticks in China, identified the gltA, ompA, and 17KD gene sequences of R. raoultii in horses and their ticks. This indicates a role of horses in R. raoultii epidemiology.

Rickettsia raoultii in Haemaphysalis erinacei from marbled polecats, China-Kazakhstan border.

Project description:We found Rickettsia raoultii DNA in 2 out of 32 (6.25 %) Haemaphysalis erinacei ticks. Result showed that the sequences of five genes (17-kDa, gltA, ompA, rrs, and ompB) were 100 % identity with that of R. Raoultii in GenBank. This study is the first report on the presence of R. raoultii in H. erinacei from wild marbled polecat, Vormela peregusna. Our findings suggest that H. erinacei parasitizing wild marbled polecat may serve as reservoir and carriers for R. raoultii in areas around the China-Kazakhstan border. The transmission of tick-borne diseases originated from wildlife should not be underestimated in border region.

First report on the occurrence of Rickettsia slovaca and Rickettsia raoultii in Dermacentor silvarum in China.

Project description:BackgroundRickettsioses are among both the longest known and most recently recognized infectious diseases. Although new spotted fever group rickettsiae have been isolated in many parts of the world including China, Little is known about the epidemiology of Rickettsia pathogens in ticks from Xinjiang Autonomous Region of China.MethodsIn an attempt to assess the potential risk of rickettsial infection after exposure to ticks in Xinjiang Uygur Autonomous Region of China, a total of 200 Dermacentor silvarum ticks collected in Xinyuan district were screened by polymerase chain reaction based on the outer membrane protein A gene.Results22 of the 200 specimens (11%) were found to be positive by PCR. Phylogenetic analysis of OmpA sequences identified two rickettsial species, Rickettsia raoultii (4.5%) and Rickettsia slovaca (6.5%).ConclusionsThis study has reported the occurrence of Rickettsia raoultii and Rickettsia slovaca in Xinjiang Autonomous Region of China and suggests that Dermacentor silvarum could be involved in the transmission of rickettsial agents in China. Further studies on the characterization and culture of rickettsial species found in Dermacentor silvarum should be performed to further clarify this. Additionally, the screening of human specimens for rickettsial disease in this region will define the incidence of infection.

A case with neurological abnormalities caused by Rickettsia raoultii in northwestern China.

Project description:BACKGROUND:The number of new rickettsial species are rapidly increasing, and increasing numbers of Rickettsia raoultii (R. raoultii) infection cases have been detected in humans. However, neurological abnormalities caused by R. raoultii are rarely reported, especially in northwestern China. CASE PRESENTATION:A 36-year-old Kazakh shepherd with an attached tick on part temporalis, presented with right eyelid droop, lethargy, fever, headache, fever (38.0-41.0?°C) and erythematous rash. The examination of cerebrospinal fluid (CSF) showed cerebrospinal pressure of 200?mm H2O, leukocyte count of 300.0?×?106/L, adenosine deaminase of 2.15?U/L, and total protein concentration of 0.93?g/L. The diagnosis of R. raoultii infection was confirmed by six genetic markers, and semi-quantified by enzyme-linked immunosorbent assay for rickettsial antigen. The patient gradually recovered after treatment with doxycycline and ceftriaxone. R. raoultii DNA was found both in a tick detached from this patient and in 0.18% (2/1107) of blood samples collected from local shepherds. CONCLUSIONS:This is the first reported case with neurological abnormalities caused by R. raoultii in northwestern China. It is vital to detect rickettsial agents both in blood and CSF for tick bite patients with neurological abnormalities. Public health workers and physicians should pay attention to neurological abnormalities caused by Rickettsia.

Project description:Tick-borne lymphadenopathy (TIBOLA), also called Dermacentor-borne necrosis erythema and lymphadenopathy (DEBONEL), is defined as the association of a tick bite, an inoculation eschar on the scalp, and cervical adenopathies. We identified the etiologic agent for 65% of 86 patients with TIBOLA/DEBONEL as either Rickettsia slovaca (49/86, 57%) or R. raoultii (7/86, 8%).

Molecular evidence of Rickettsia raoultii, "Candidatus Rickettsia barbariae" and a novel Babesia genotype in marbled polecats (Vormela peregusna) at the China-Kazakhstan border.

Project description:In this study, two road-killed marbled polecats (Vormela peregusna) were molecularly analysed for tick-borne pathogens. Rickettsia raoultii, "Candidatus Rickettsia barbariae" and a novel Babesia genotype have been identified, for the first time in marbled polecat. DNA of this Babesia sp. genotype was also present in four out of 15 Haemaphysalis erinacei ticks collected from the Babesia PCR-positive marbled polecat. Results of this study suggest that marbled polecats may serve as reservoirs for these bacteria and protozoans.

Common epidemiology of Rickettsia felis infection and malaria, Africa.

Project description:This study aimed to compare the epidemiology of Rickettsia felis infection and malaria in France, North Africa, and sub-Saharan Africa and to identify a common vector. Blood specimens from 3,122 febrile patients and from 500 nonfebrile persons were analyzed for R. felis and Plasmodium spp. We observed a significant linear trend (p<0.0001) of increasing risk for R. felis infection. The risks were lowest in France, Tunisia, and Algeria (1%), and highest in rural Senegal (15%). Co-infections with R. felis and Plasmodium spp. and occurrences of R. felis relapses or reinfections were identified. This study demonstrates a correlation between malaria and R. felis infection regarding geographic distribution, seasonality, asymptomatic infections, and a potential vector. R. felis infection should be suspected in these geographical areas where malaria is endemic. Doxycycline chemoprophylaxis against malaria in travelers to sub-Saharan Africa also protects against rickettsioses; thus, empirical treatment strategies for febrile illness for travelers and residents in sub-Saharan Africa may require reevaluation.
