New heat-treated vs electropolished nickel-titanium instruments used in root canal treatment: Influence of autoclave sterilization on surface roughness.

ABSTRACT: Nickel-titanium (NiTi) instruments used to treat root canal infections are affected by autoclave sterilization in various ways. The aim of this study was to compare the effect of autoclave sterilization on two NiTi rotary instruments that undergo different manufacturing treatments: The electro-polished Race and the heat-treated Race Evo, using scanning electron microscope analysis. In this in-vitro study, Race and Race-Evo instruments were subjected to a number of autoclaving cycles (0, 1, 3, 5, and 10). Scanning electron microscopy images were obtained at 3 mm from the tip of the file at 450x and 1000x magnifications. Surface roughness parameters were measured using ImageJ software. The results showed that autoclave sterilization caused a significant decrease in conventional NiTi Race surface roughness. While in Race Evo, surface roughness increased following the first autoclaving cycle. After 10 autoclaving cycles, surface roughness significantly decreased for both Race and Race Evo files.

Project description:BackgroundTo investigate the complications associated with the use of nickel titanium rotary instruments (NiTi-RIs) for root canal treatments (RCTs), in Saudi Arabia dental practice, and to explore the influencing factors.MethodsAfter obtaining an ethical approval, two pilot studies were conducted to formulate the final questionnaire. The sample size was measured taking into consideration 60% expected response rates and confidence level of 99.9%. The questionnaire was emailed to 600 general dentists (GDs) randomly selected from the dental register and all of the endodontists (175). The email's introduction clarified objectives of the study and guaranteed that all of the collected information would remain confidential. A reminder was sent after 10 weeks. The data were collected and analyzed using the chi-squared test at a 0.05 significance level.ResultsWith a 51% overall response rate, 71.9% off the respondents used NiTi-RIs. The majority (83.1%) experienced complications while using NiTi-RIs; with the instruments' fracture being significantly the most common complication (52.7%) (p < 0.001). The majority (87.7%) experienced NiTi-RIs' fracture at least once; with more endodontists (94.3%) than GDs (83.3%) (p < 0.001). The greater the number of weekly performed RCTs and participants' experiences, the more NiTi-RIs fractures and the greater the number of fracture incidents (p < 0.001). While 60% of those who performed 1-3 RCTs per week experienced NiTi-RIs fractures, 100% of those who performed more than 12 RCTs per week did so. The highest percentage of those who experienced more than 10 fractured NiTi-RIs (60%) was within the group who performed more than 12 RCTs per week. Although fracture incidents decreased with a smaller number of reuses, there was no significant correlation between the number of fractured instruments and NiTi-RIs discard strategy (p ≥ 0.05).ConclusionFracture incidence was the most common complication while using NiTi-RIs, regardless of the clinicians' experiences and skills. While the single use may reduce NiTi-RIs fractures, to some extent, the greater number of RCTs performed per week was the most influential factor.

Project description:BackgroundFemale sterilization is the second most commonly used method of contraception in the United States. Female sterilization can now be performed through laparoscopic, abdominal, or hysteroscopic approaches. The hysteroscopic sterilization may be a safer option than sterilization through laparoscopy or laparotomy because it avoids invading the abdominal cavity and undergoing general anaesthesia. Hysteroscopic sterilization mainly includes chemical agents and mechanical devices. Common issues related to the toxicity of the chemical agents used have raised concerns regarding this kind of contraception. The difficulty of the transcervical insertion of such mechanical devices into the fallopian tubes has increased the high incidence of device displacement or dislodgment. At present, Essure® is the only commercially available hysteroscopic sterilization device being used clinically. The system is irreversible and is not effective immediately.Presentation of the hypothesisOur new hysteroscopic sterility system consists of nickel-titanium (NiTi) shape memory alloy and a waterproof membrane. The NiTi alloy is covered with two coatings to avoid toxic Ni release and to prevent stimulation of epithelial tissue growth around the oviducts. Because of the shape memory effect of the NiTi alloy, the device works like an umbrella: it stays collapsed at low temperature before placement and opens by the force of shape memory activated by the body temperature after it is inserted hysteroscopically into the interstitial tubal lumen. The rim of the open device will incise into interstitial myometrium during the process of unfolding. Once the device is fixed, it blocks the tube completely. When the patient no longer wishes for sterilization, the device can be closed by perfusing liquid with low temperature into the uterine cavity, followed by prospective hysteroscopic removal. After the device removal, the fallopian tube will revert to its physiological functions.Testing the hypothesisCurrently, experimental and clinical studies are needed to attest the safety, efficiency and reversibility of the novel sterilization device.Implications of the hypothesisIf our hypothesis is confirmed, appropriate and reversible contraceptive can be achieved with the device we have designed, which may have significant repercussions for numerous women worldwide.

Project description:Root canal obturation involves filling of the chemomechanically prepared root canal space. Despite reduced microbial load, residual bacteria can still lead to reinfection and treatment failure. Currently, obturation techniques use a combination of gutta-percha and sealer, which requires the preparation of the root canal to specific sizes and tapers to enable the fitting of customized cones. This study aims to characterize the physical, chemical, and antimicrobial properties of a new light-curable injectable material (OdneFill, Switzerland) used to obturate the root canal. Odnefill and 2 root canal sealers (AH Plus and BioRoot RCS) were characterized by scanning electron microscopy (SEM) and energy-dispersive spectroscopy following exposure to chlorhexidine, sodium hypochlorite, and water. The flow, film thickness, radiopacity, solubility, and contact angle were evaluated. The susceptibility to microbial degradation was assessed by weight changes after contact with bacterial enzymes (lipase and cholesterol esterase). A multispecies biofilm composed of Streptococcus mutans, Enterococcus faecalis, Fusobacterium nucleatum, and Veillonella dispar was used to assess changes to the material microstructure (SEM). Further, bacterial viability in contact with the materials was evaluated using live/dead staining and confocal microscopy. A direct contact assay was carried out, and the utilization of the materials as a carbon source for the bacterial biofilm was also assessed. Statistical analysis was performed using 1-way analysis of variance and Tukey post hoc tests (P = 0.05). OdneFill was composed of an organic matrix with zirconium oxide filler. It exhibited comparable physical properties to AH Plus and BioRoot RCS and was stable in contact with irrigating solutions and with the bacterial enzymes (cholesterol esterase and lipase). Its antimicrobial characteristics were better than those of AH Plus when placed in contact with a multispecies biofilm. Based on the findings, OdneFill presents itself as suitable root canal-filling material and warrants further clinical investigation.
