An explainable artificial intelligence approach for predicting cardiovascular outcomes using electronic health records.

ABSTRACT: Understanding the conditionally-dependent clinical variables that drive cardiovascular health outcomes is a major challenge for precision medicine. Here, we deploy a recently developed massively scalable comorbidity discovery method called Poisson Binomial based Comorbidity discovery (PBC), to analyze Electronic Health Records (EHRs) from the University of Utah and Primary Children's Hospital (over 1.6 million patients and 77 million visits) for comorbid diagnoses, procedures, and medications. Using explainable Artificial Intelligence (AI) methodologies, we then tease apart the intertwined, conditionally-dependent impacts of comorbid conditions and demography upon cardiovascular health, focusing on the key areas of heart transplant, sinoatrial node dysfunction and various forms of congenital heart disease. The resulting multimorbidity networks make possible wide-ranging explorations of the comorbid and demographic landscapes surrounding these cardiovascular outcomes, and can be distributed as web-based tools for further community-based outcomes research. The ability to transform enormous collections of EHRs into compact, portable tools devoid of Protected Health Information solves many of the legal, technological, and data-scientific challenges associated with large-scale EHR analyses.

Project description:Healthcare data are inherently multimodal, including electronic health records (EHR), medical images, and multi-omics data. Combining these multimodal data sources contributes to a better understanding of human health and provides optimal personalized healthcare. The most important question when using multimodal data is how to fuse them-a field of growing interest among researchers. Advances in artificial intelligence (AI) technologies, particularly machine learning (ML), enable the fusion of these different data modalities to provide multimodal insights. To this end, in this scoping review, we focus on synthesizing and analyzing the literature that uses AI techniques to fuse multimodal medical data for different clinical applications. More specifically, we focus on studies that only fused EHR with medical imaging data to develop various AI methods for clinical applications. We present a comprehensive analysis of the various fusion strategies, the diseases and clinical outcomes for which multimodal fusion was used, the ML algorithms used to perform multimodal fusion for each clinical application, and the available multimodal medical datasets. We followed the PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews) guidelines. We searched Embase, PubMed, Scopus, and Google Scholar to retrieve relevant studies. After pre-processing and screening, we extracted data from 34 studies that fulfilled the inclusion criteria. We found that studies fusing imaging data with EHR are increasing and doubling from 2020 to 2021. In our analysis, a typical workflow was observed: feeding raw data, fusing different data modalities by applying conventional machine learning (ML) or deep learning (DL) algorithms, and finally, evaluating the multimodal fusion through clinical outcome predictions. Specifically, early fusion was the most used technique in most applications for multimodal learning (22 out of 34 studies). We found that multimodality fusion models outperformed traditional single-modality models for the same task. Disease diagnosis and prediction were the most common clinical outcomes (reported in 20 and 10 studies, respectively) from a clinical outcome perspective. Neurological disorders were the dominant category (16 studies). From an AI perspective, conventional ML models were the most used (19 studies), followed by DL models (16 studies). Multimodal data used in the included studies were mostly from private repositories (21 studies). Through this scoping review, we offer new insights for researchers interested in knowing the current state of knowledge within this research field.

Project description:BackgroundComorbidities are common in chronic inflammatory conditions, requiring multidisciplinary treatment approach. Understanding the link between a single disease and its comorbidities is important for appropriate treatment and management. We evaluate the ability of an NLP-based process for knowledge discovery to detect information about pathologies, patients' phenotype, doctors' prescriptions and commonalities in electronic medical records, by extracting information from free narrative text written by clinicians during medical visits, resulting in the extraction of valuable information and enriching real world evidence data from a multidisciplinary setting.MethodsWe collected clinical notes from the Allergy Department of Humanitas Research Hospital written in the last 3 years and used it to look for diseases that cluster together as comorbidities associated to the main pathology of our patients, and for the extent of prescription of systemic corticosteroids, thus evaluating the ability of NLP-based tools for knowledge discovery to extract structured information from free text.ResultsWe found that the 3 most frequent comorbidities to appear in our clusters were asthma, rhinitis, and urticaria, and that 991 (of 2057) patients suffered from at least one of these comorbidities. The clusters which co-occur particularly often are oral allergy syndrome and urticaria (131 patients), angioedema and urticaria (105 patients), rhinitis and asthma (227 patients). With regards to systemic corticosteroid prescription volume by our clinicians, we found it was lower when compared to the therapy the patients followed before coming to our attention, with the exception of two diseases: Chronic obstructive pulmonary disease and Angioedema.ConclusionsThis analysis seems to be valid and is confirmed by the data from the literature. This means that NLP tools could have significant role in many other research fields of medicine, as it may help identify other important, and possibly previously neglected clusters of patients with comorbidities and commonalities. Another potential benefit of this approach lies in its potential ability to foster a multidisciplinary approach, using the same drugs to treat pathologies normally treated by physicians in different branches of medicine, thus saving resources and improving the pharmacological management of patients.

Project description:BackgroundWith the accumulation of electronic health records and the development of artificial intelligence, patients with cancer urgently need new evidence of more personalized clinical and demographic characteristics and more sophisticated treatment and prevention strategies. However, no research has systematically analyzed the application and significance of artificial intelligence based on electronic health records in cancer care.ObjectiveThe aim of this study was to conduct a review to introduce the current state and limitations of artificial intelligence based on electronic health records of patients with cancer and to summarize the performance of artificial intelligence in mining electronic health records and its impact on cancer care.MethodsThree databases were systematically searched to retrieve potentially relevant papers published from January 2009 to October 2020. Four principal reviewers assessed the quality of the papers and reviewed them for eligibility based on the inclusion criteria in the extracted data. The summary measures used in this analysis were the number and frequency of occurrence of the themes.ResultsOf the 1034 papers considered, 148 papers met the inclusion criteria. Cancer care, especially cancers of female organs and digestive organs, could benefit from artificial intelligence based on electronic health records through cancer emergencies and prognostic estimates, cancer diagnosis and prediction, tumor stage detection, cancer case detection, and treatment pattern recognition. The models can always achieve an area under the curve of 0.7. Ensemble methods and deep learning are on the rise. In addition, electronic medical records in the existing studies are mainly in English and from private institutional databases.ConclusionsArtificial intelligence based on electronic health records performed well and could be useful for cancer care. Improving the performance of artificial intelligence can help patients receive more scientific-based and accurate treatments. There is a need for the development of new methods and electronic health record data sharing and for increased passion and support from cancer specialists.
