Project description:Fast and accurate diagnosis is critical for the triage and management of pneumonia, particularly in the current scenario of a COVID-19 pandemic, where this pathology is a major symptom of the infection. With the objective of providing tools for that purpose, this study assesses the potential of three textural image characterisation methods: radiomics, fractal dimension and the recently developed superpixel-based histon, as biomarkers to be used for training Artificial Intelligence (AI) models in order to detect pneumonia in chest X-ray images. Models generated from three different AI algorithms have been studied: K-Nearest Neighbors, Support Vector Machine and Random Forest. Two open-access image datasets were used in this study. In the first one, a dataset composed of paediatric chest X-ray, the best performing generated models achieved an 83.3% accuracy with 89% sensitivity for radiomics, 89.9% accuracy with 93.6% sensitivity for fractal dimension and 91.3% accuracy with 90.5% sensitivity for superpixels based histon. Second, a dataset derived from an image repository developed primarily as a tool for studying COVID-19 was used. For this dataset, the best performing generated models resulted in a 95.3% accuracy with 99.2% sensitivity for radiomics, 99% accuracy with 100% sensitivity for fractal dimension and 99% accuracy with 98.6% sensitivity for superpixel-based histons. The results confirm the validity of the tested methods as reliable and easy-to-implement automatic diagnostic tools for pneumonia.

Project description:BackgroundStress echocardiography is an emerging tool used to detect exercise-induced pulmonary hypertension (EIPH). However, facilities that can perform stress echocardiography are limited by issues such as cost and equipment.ObjectiveWe evaluated the usefulness of a deep learning (DL) approach based on a chest X-ray (CXR) to predict EIPH in 6-min walk stress echocardiography.MethodsThe study enrolled 142 patients with scleroderma or mixed connective tissue disease with scleroderma features who performed a 6-min walk stress echocardiographic test. EIPH was defined by abnormal cardiac output (CO) responses that involved an increase in mean pulmonary artery pressure (mPAP). We used the previously developed AI model to predict PH and calculated PH probability in this cohort.ResultsEIPH defined as ΔmPAP/ΔCO >3.3 and exercise mPAP >25 mmHg was observed in 52 patients, while non-EIPH was observed in 90 patients. The patients with EIPH had a higher mPAP at rest than those without EIPH. The probability of PH based on the DL model was significantly higher in patients with EIPH than in those without EIPH. Multivariate analysis showed that gender, mean PAP at rest, and the probability of PH based on the DL model were independent predictors of EIPH. A model based on baseline parameters (age, gender, and mPAP at rest) was improved by adding the probability of PH predicted by the DL model (AUC: from 0.65 to 0.74; p = 0.046).ConclusionApplying the DL model based on a CXR may have a potential for detection of EIPH in the clinical setting.

Project description:ObjectiveTo develop prognostic models for survival (alive or deceased status) prediction of COVID-19 patients using clinical data (demographics and history, laboratory tests, visual scoring by radiologists) and lung/lesion radiomic features extracted from chest CT images.MethodsOverall, 152 patients were enrolled in this study protocol. These were divided into 106 training/validation and 46 test datasets (untouched during training), respectively. Radiomic features were extracted from the segmented lungs and infectious lesions separately from chest CT images. Clinical data, including patients' history and demographics, laboratory tests and radiological scores were also collected. Univariate analysis was first performed (q-value reported after false discovery rate (FDR) correction) to determine the most predictive features among all imaging and clinical data. Prognostic modeling of survival was performed using radiomic features and clinical data, separately or in combination. Maximum relevance minimum redundancy (MRMR) and XGBoost were used for feature selection and classification. The receiver operating characteristic (ROC) curve and the area under the ROC curve (AUC), sensitivity, specificity, and accuracy were used to assess the prognostic performance of the models on the test datasets.ResultsFor clinical data, cancer comorbidity (q-value < 0.01), consciousness level (q-value < 0.05) and radiological score involved zone (q-value < 0.02) were found to have high correlated features with outcome. Oxygen saturation (AUC = 0.73, q-value < 0.01) and Blood Urea Nitrogen (AUC = 0.72, q-value = 0.72) were identified as high clinical features. For lung radiomic features, SAHGLE (AUC = 0.70) and HGLZE (AUC = 0.67) from GLSZM were identified as most prognostic features. Amongst lesion radiomic features, RLNU from GLRLM (AUC = 0.73), HGLZE from GLSZM (AUC = 0.73) had the highest performance. In multivariate analysis, combining lung, lesion and clinical features was determined to provide the most accurate prognostic model (AUC = 0.95 ± 0.029 (95%CI: 0.95-0.96), accuracy = 0.88 ± 0.046 (95% CI: 0.88-0.89), sensitivity = 0.88 ± 0.066 (95% CI = 0.87-0.9) and specificity = 0.89 ± 0.07 (95% CI = 0.87-0.9)).ConclusionCombination of radiomic features and clinical data can effectively predict outcome in COVID-19 patients. The developed model has significant potential for improved management of COVID-19 patients.

Project description:Pneumonia is a fatal disease responsible for almost one in five child deaths worldwide. Many developing countries have high mortality rates due to pneumonia because of the unavailability of proper and timely diagnostic measures. Using machine learning-based diagnosis methods can help to detect the disease early and in less time and cost. In this study, we proposed a novel method to determine the presence of pneumonia and identify its type (bacterial or viral) through analyzing chest radiographs. We performed a three-class classification based on features containing diverse information of the samples. After using an augmentation technique to balance the dataset's sample sizes, we extracted the chest X-ray images' statistical features, as well as global features by employing a deep learning architecture. We then combined both sets of features and performed the final classification using the RandomForest classifier. A feature selection method was also incorporated to identify the features with the highest relevance. We tested the proposed method on a widely used (but relabeled) chest radiograph dataset to evaluate its performance. The proposed model can classify the dataset's samples with an 86.30% classification accuracy and 86.03% F-score, which assert the model's efficacy and reliability. However, results show that the classifier struggles while distinguishing between viral and bacterial pneumonia samples. Implementing this method will provide a fast and automatic way to detect pneumonia in a patient and identify its type.

Project description:BackgroundAccording to the World Health Organization (WHO), pneumonia is the leading infectious cause of death in children below 5 years old. Hence, the early detection of pediatric pneumonia is crucial to reduce its morbidity and mortality rates. Even though chest radiography is the most commonly employed modality for pneumonia detection, recent studies highlight the existence of poor interobserver agreement in the chest X-ray interpretation of healthcare practitioners when it comes to diagnosing pediatric pneumonia. Thus, there is a significant need for automating the detection process to minimize the potential human error. Since Artificial Intelligence tools such as Deep Learning (DL) and Machine Learning (ML) have the potential to automate disease detection, many researchers explored how such tools can be implemented to detect pneumonia in chest X-rays. Notably, the majority of efforts tackled this problem from a DL point of view. However, ML has shown a higher potential for medical interpretability while being less computationally demanding than DL.ObjectiveThe aim of this paper is to automate the early detection process of pediatric pneumonia using ML as it is less computationally demanding than DL.MethodsThe proposed approach entails performing data augmentation to balance the classes of the utilized dataset, optimizing the feature extraction scheme, and evaluating the performance of several ML models. Moreover, the performance of this approach is compared to a TL benchmark to evaluate its candidacy.ResultsUsing the proposed approach, the Quadratic SVM model yielded an accuracy of 97.58%, surpassing the accuracies reported in the current ML literature. In addition, this model classification time was significantly smaller than that of the TL benchmark.ConclusionThe results strongly support the candidacy of the proposed approach in reliably detecting pediatric pneumonia.

Project description:Glioblastoma is a WHO grade IV brain tumor, which leads to poor overall survival (OS) of patients. For precise surgical and treatment planning, OS prediction of glioblastoma (GBM) patients is highly desired by clinicians and oncologists. Radiomic research attempts at predicting disease prognosis, thus providing beneficial information for personalized treatment from a variety of imaging features extracted from multiple MR images. In this study, first-order, intensity-based volume and shape-based and textural radiomic features are extracted from fluid-attenuated inversion recovery (FLAIR) and T1ce MRI data. The region of interest is further decomposed with stationary wavelet transform with low-pass and high-pass filtering. Further, radiomic features are extracted on these decomposed images, which helped in acquiring the directional information. The efficiency of the proposed algorithm is evaluated on Brain Tumor Segmentation (BraTS) challenge training, validation, and test datasets. The proposed approach achieved 0.695, 0.571, and 0.558 on BraTS training, validation, and test datasets. The proposed approach secured the third position in BraTS 2018 challenge for the OS prediction task.

Project description:An entity's existence in an image can be depicted by the activity instantiation vector from a group of neurons (called capsule). Recently, multi-layered capsules, called CapsNet, have proven to be state-of-the-art for image classification tasks. This research utilizes the prowess of this algorithm to detect pneumonia from chest X-ray (CXR) images. Here, an entity in the CXR image can help determine if the patient (whose CXR is used) is suffering from pneumonia or not. A simple model of capsules (also known as Simple CapsNet) has provided results comparable to best Deep Learning models that had been used earlier. Subsequently, a combination of convolutions and capsules is used to obtain two models that outperform all models previously proposed. These models-Integration of convolutions with capsules (ICC) and Ensemble of convolutions with capsules (ECC)-detect pneumonia with a test accuracy of 95.33% and 95.90%, respectively. The latter model is studied in detail to obtain a variant called EnCC, where n = 3, 4, 8, 16. Here, the E4CC model works optimally and gives test accuracy of 96.36%. All these models had been trained, validated, and tested on 5857 images from Mendeley.

Project description:BackgroundEarly detection and management of sarcopenia is of clinical importance. We aimed to develop a chest X-ray-based deep learning model to predict presence of sarcopenia.MethodsData of participants who visited osteoporosis clinic at Severance Hospital, Seoul, South Korea, between January 2020 and June 2021 were used as derivation cohort as split to train, validation and test set (65:15:20). A community-based older adults cohort (KURE) was used as external test set. Sarcopenia was defined based on Asian Working Group 2019 guideline. A deep learning model was trained to predict appendicular lean mass (ALM), handgrip strength (HGS) and chair rise test performance from chest X-ray images; then the machine learning model (SARC-CXR score) was built using the age, sex, body mass index and chest X-ray predicted muscle parameters along with estimation uncertainty values.ResultsMean age of the derivation cohort (n = 926; women n = 700, 76%; sarcopenia n = 141, 15%) and the external test (n = 149; women n = 95, 64%; sarcopenia n = 18, 12%) cohort was 61.4 and 71.6 years, respectively. In the internal test set (a hold-out set, n = 189, from the derivation cohort) and the external test set (n = 149), the concordance correlation coefficient for ALM prediction was 0.80 and 0.76, with an average difference of 0.18 ± 2.71 and 0.21 ± 2.28, respectively. Gradient-weight class activation mapping for deep neural network models to predict ALM and HGS commonly showed highly weight pixel values at bilateral lung fields and part of the cardiac contour. SARC-CXR score showed good discriminatory performance for sarcopenia in both internal test set [area under the receiver-operating characteristics curve (AUROC) 0.813, area under the precision-recall curve (AUPRC) 0.380, sensitivity 0.844, specificity 0.739, F1-score 0.540] and external test set (AUROC 0.780, AUPRC 0.440, sensitivity 0.611, specificity 0.855, F1-score 0.458). Among SARC-CXR model features, predicted low ALM from chest X-ray was the most important predictor of sarcopenia based on SHapley Additive exPlanations values. Higher estimation uncertainty of HGS contributed to elevate the predicted risk of sarcopenia. In internal test set, SARC-CXR score showed better discriminatory performance than SARC-F score (AUROC 0.813 vs. 0.691, P = 0.029).ConclusionsChest X-ray-based deep leaning model improved detection of sarcopenia, which merits further investigation.

Project description:Recent studies show the potential of artificial intelligence (AI) as a screening tool to detect COVID-19 pneumonia based on chest x-ray (CXR) images. However, issues on the datasets and study designs from medical and technical perspectives, as well as questions on the vulnerability and robustness of AI algorithms have emerged. In this study, we address these issues with a more realistic development of AI-driven COVID-19 pneumonia detection models by generating our own data through a retrospective clinical study to augment the dataset aggregated from external sources. We optimized five deep learning architectures, implemented development strategies by manipulating data distribution to quantitatively compare study designs, and introduced several detection scenarios to evaluate the robustness and diagnostic performance of the models. At the current level of data availability, the performance of the detection model depends on the hyperparameter tuning and has less dependency on the quantity of data. InceptionV3 attained the highest performance in distinguishing pneumonia from normal CXR in two-class detection scenario with sensitivity (Sn), specificity (Sp), and positive predictive value (PPV) of 96%. The models attained higher general performance of 91-96% Sn, 94-98% Sp, and 90-96% PPV in three-class compared to four-class detection scenario. InceptionV3 has the highest general performance with accuracy, F1-score, and g-mean of 96% in the three-class detection scenario. For COVID-19 pneumonia detection, InceptionV3 attained the highest performance with 86% Sn, 99% Sp, and 91% PPV with an AUC of 0.99 in distinguishing pneumonia from normal CXR. Its capability of differentiating COVID-19 pneumonia from normal and non-COVID-19 pneumonia attained 0.98 AUC and a micro-average of 0.99 for other classes.

Project description:The Coronavirus (COVID-19) pandemic has created havoc on humanity by causing millions of deaths and adverse physical and mental health effects. To prepare humankind for the fast and efficient detection of the virus and its variants shortly, COVID-19 detection using Artificial Intelligence and Computer-Aided Diagnosis has been the subject of several studies. To detect COVID-19, there are numerous publicly accessible datasets of Chest X-rays that the researchers have combined to solve the problem of inadequate data. The cause for concern here is that in combining two or more datasets, some of the images might be duplicates, so a curated dataset has been used in this study, taken from an author's paper. This dataset consists of 1281 COVID-19, 3270 Normal X-rays, and 1656 viral-pneumonia infected Chest X-ray images. Dataset has been pre-processed and divided carefully to ensure that there are no duplicate images. A comparative study on many traditional pre-trained models was performed, analyzing top-performing models. Fine-tuned InceptionV3, Modified EfficientNet B0&B1 produced an accuracy of 99.78% on binary classification, i.e., covid-19 infected and normal Chest X-ray image. ResNetV2 had a classification accuracy of 97.90% for 3-class classification i.e., covid-19 infected, normal, and pneumonia. Furthermore, a trailblazing custom CNN-based model, CoviXNet, has been proposed consisting of 15 layers that take efficiency into account. The proposed model CoviXNet exhibited a 10-fold accuracy of 99.47% on binary classification and 96.61% on 3-class. CoviXNet has shown phenomenal performance with exceptional accuracy and minimum computational cost. We anticipate that this comparative study, along with the proposed model CoviXNet, can assist medical centers with the efficient real-life detection of Coronavirus.
