Dynamical intervention planning against COVID-19-like epidemics.

ABSTRACT: COVID-19 has got us to face a new situation where, for the lack of ready-to-use vaccines, it is necessary to support vaccination with complex non-pharmaceutical strategies. In this paper, we provide a novel Mixed Integer Nonlinear Programming formulation for fine-grained optimal intervention planning (i.e., at the level of the single day) against newborn epidemics like COVID-19, where a modified SIR model accounting for heterogeneous population classes, social distancing and several types of vaccines (each with its efficacy and delayed effects), allows us to plan an optimal mixed strategy (both pharmaceutical and non-pharmaceutical) that takes into account both the vaccine availability in limited batches at selected time instants and the need for second doses while keeping hospitalizations and intensive care occupancy below a threshold and requiring that new infections die out at the end of the planning horizon. In order to show the effectiveness of the proposed formulation, we analyze a case study for Italy with realistic parameters.

Project description:Against the current COVID-19 pandemic, governments worldwide have devised a variety of non-pharmaceutical interventions to mitigate it. However, it is generally difficult to estimate the joint impact of different control strategies. In this paper, we tackle this question with an extended epidemic SEIR model, informed by a socio-political classification of different interventions. First, we inquire the conceptual effect of mitigation parameters on the infection curve. Then, we illustrate the potential of our model to reproduce and explain empirical data from a number of countries, to perform cross-country comparisons. This gives information on the best synergies of interventions to control epidemic outbreaks while minimising impact on socio-economic needs. For instance, our results suggest that, while rapid and strong lockdown is an effective pandemic mitigation measure, a combination of social distancing and early contact tracing can achieve similar mitigation synergistically, while keeping lower isolation rates. This quantitative understanding can support the establishment of mid- and long-term interventions, to prepare containment strategies against further outbreaks. This paper also provides an online tool that allows researchers and decision makers to interactively simulate diverse scenarios with our model.

COMPARE Analysis, a Bioinformatic Approach to Accelerate Drug Repurposing against Covid-19 and Other Emerging Epidemics.

Project description:A novel bioinformatic approach for drug repurposing against emerging viral epidemics like Covid-19 is described. It exploits the COMPARE algorithm, a public program from the National Cancer Institute (NCI) to sort drugs according to their patterns of growth inhibitory profiles from a diverse panel of human cancer cell lines. The data repository of the NCI includes the growth inhibitory patterns of more than 55,000 molecules. When candidate drug molecules with ostensible anti-SARS-CoV-2 activities were used as seeds (e.g., hydroxychloroquine, ritonavir, and dexamethasone) in COMPARE, the analysis uncovered several molecules with fingerprints similar to the seeded drugs. Interestingly, despite the fact that the uncovered drugs were from various pharmacological classes (antiarrhythmic, nucleosides, antipsychotic, alkaloids, antibiotics, and vitamins), they were all reportedly known from published literature to exert antiviral activities via different modes, confirming that COMPARE analysis is efficient for predicting antiviral activities of drugs from various pharmacological classes. Noticeably, several of the uncovered drugs can be readily tested, like didanosine, methotrexate, vitamin A, nicotinamide, valproic acid, uridine, and flucloxacillin. Unlike pure in silico methods, this approach is biologically more relevant and able to pharmacologically correlate compounds regardless of their chemical structures. This is an untapped resource, reliable and readily exploitable for drug repurposing against current and future viral outbreaks.

Recent biotechnological advances as potential intervention strategies against COVID-19.

Project description:The emerging SARS-CoV-2 viral disease (COVID-19) has caused a global health alert due to its high rate of infection and mortality in individuals with chronic cardiovascular comorbidities, in addition to generating complex clinical conditions. This has forced the scientific community to explore different strategies that allow combating this viral infection as well as treating life-threatening systemic effect of the infection on the individual. In this work, we have reviewed the most recent scientific evidence to provide a comprehensive panorama regarding the biotechnological strategies that have been proposed to combat this new viral infection. We have focused our analysis on vaccine production, nanotechnology applications, repurposing of know drugs for unrelated pathologies, and the search for bioactive molecules obtained from natural products. The goals include safely use as potential prophylactic or therapeutic treatments, based on in silico and in vivo studies, including clinical trials around the world for the correct and timely diagnosis of the infection. This review aims to highlight the development of new ideas that can decrease the time lines for research output and improve research quality while at the same time, keeping in mind the efficacy and safety aspects of these potential biotechnological strategies.

Data-driven optimized control of the COVID-19 epidemics.

Project description:Optimizing the impact on the economy of control strategies aiming at containing the spread of COVID-19 is a critical challenge. We use daily new case counts of COVID-19 patients reported by local health administrations from different Metropolitan Statistical Areas (MSAs) within the US to parametrize a model that well describes the propagation of the disease in each area. We then introduce a time-varying control input that represents the level of social distancing imposed on the population of a given area and solve an optimal control problem with the goal of minimizing the impact of social distancing on the economy in the presence of relevant constraints, such as a desired level of suppression for the epidemics at a terminal time. We find that with the exception of the initial time and of the final time, the optimal control input is well approximated by a constant, specific to each area, which contrasts with the implemented system of reopening 'in phases'. For all the areas considered, this optimal level corresponds to stricter social distancing than the level estimated from data. Proper selection of the time period for application of the control action optimally is important: depending on the particular MSA this period should be either short or long or intermediate. We also consider the case that the transmissibility increases in time (due e.g. to increasingly colder weather), for which we find that the optimal control solution yields progressively stricter measures of social distancing. We finally compute the optimal control solution for a model modified to incorporate the effects of vaccinations on the population and we see that depending on a number of factors, social distancing measures could be optimally reduced during the period over which vaccines are administered to the population.

Bayesian time-varying autoregressive models of COVID-19 epidemics.

Project description:The COVID-19 pandemic has highlighted the importance of reliable statistical models which, based on the available data, can provide accurate forecasts and impact analysis of alternative policy measures. Here we propose Bayesian time-dependent Poisson autoregressive models that include time-varying coefficients to estimate the effect of policy covariates on disease counts. The model is applied to the observed series of new positive cases in Italy and in the United States. The results suggest that our proposed models are capable of capturing nonlinear growth of disease counts. We also find that policy measures and, in particular, closure policies and the distribution of vaccines, lead to a significant reduction in disease counts in both countries.

Forecasting efforts from prior epidemics and COVID-19 predictions.

Project description:Since the onset of the COVID-19 pandemic, countless disease prediction models have emerged, shaping the focus of news media, policymakers, and broader society. We reviewed the accuracy of forecasts made during prior twenty-first century epidemics, namely SARS, H1N1, and Ebola. We found that while disease prediction models were relatively nascent as a research focus during SARS and H1N1, for Ebola, numerous such forecasts were published. We found that forecasts of deaths for Ebola were often far from the eventual reality, with a strong tendency to over predict. Given the societal prominence of these models, it is crucial that their uncertainty be communicated. Otherwise, we will be unaware if we are being falsely lulled into complacency or unjustifiably shocked into action.

Prediction of Epidemics Trend of COVID-19 in Bangladesh.

Project description:Background: Amid a critical and emergent situation like the coronavirus disease (COVID-19) pandemic related to extreme health and economic repercussions, we used and presented the mathematical modeling like susceptible-infectious-recovered (SIR) to have a numerical demonstration that can shed light to decide the fate of the scourge in Bangladesh. To describe the idea about the factors influencing the outbreak data, we presented the current situation of the COVID-19 outbreak with graphical trends. Methods: Primary data were collected and analyzed by using a pre-created Google Survey form having a pre-set questionnaire on the social distancing status of different districts. Secondary data on the total and the daily number of laboratory tests, confirmed positive cases, and death cases were extracted from the publicly available sources to make predictions. We estimated the basic reproduction number (R ◦) based on the SIR mathematical model and predicted the probable fate of this pandemic in Bangladesh. Results: Quarantine situations in different regions of Bangladesh were evaluated and presented. We also provided tentative forecasts until 31 May 2020 and found that the predicted curve followed the actual curve approximately. Estimated R ◦-values (6.924) indicated that infection rate would be greater than the recovery rate. Furthermore, by calibrating the parameters of the SIR model to fit the reported data, we assume the ultimate ending of the pandemic in Bangladesh by December 2022. Conclusion: We hope that the results of our analysis could contribute to the elucidation of critical aspects of this outbreak and help the concerned authority toward decision making.

Optimizing non-pharmaceutical intervention strategies against COVID-19 using artificial intelligence.

Project description:One key task in the early fight against the COVID-19 pandemic was to plan non-pharmaceutical interventions to reduce the spread of the infection while limiting the burden on the society and economy. With more data on the pandemic being generated, it became possible to model both the infection trends and intervention costs, transforming the creation of an intervention plan into a computational optimization problem. This paper proposes a framework developed to help policy-makers plan the best combination of non-pharmaceutical interventions and to change them over time. We developed a hybrid machine-learning epidemiological model to forecast the infection trends, aggregated the socio-economic costs from the literature and expert knowledge, and used a multi-objective optimization algorithm to find and evaluate various intervention plans. The framework is modular and easily adjustable to a real-world situation, it is trained and tested on data collected from almost all countries in the world, and its proposed intervention plans generally outperform those used in real life in terms of both the number of infections and intervention costs.

Clinician Absences and Contributing Factors During a COVID-19 Surge: Potential Areas for Intervention and Planning.

Project description:IntroductionOur goal was to quantify healthcare clinician (HCC) absenteeism in the emergency department (ED) during the coronavirus disease 2019 (COVID-19) surge and to identify potential interventions that may mitigate the number of absences.MethodsThis was a retrospective, descriptive record review that included 82 resident physicians, physician assistants, and staff physicians who were scheduled to work more than three clinical shifts during March 2020 in an urban, academic ED that received a high number of coronavirus disease 2019 (COVID-19) patients. Exposure was defined as a healthcare clinician who was not wearing appropriate personal protective equipment (PPE) having contact with a confirmed COVID-19 positive patient in the ED. The main outcome was the number of HCC absences secondary to exposure to or symptoms concerning for COVID-19.ResultsDuring March 2020, of 82 ED HCCs, 28 (34%) required an absence from clinical duties, totaling 152 absentee calendar days (N = 13 women [46%]; N = 15 men [54%]). Median HCC age was 32 years (interquartile range 28-39), and median number of days absent was four (interquartile range 3-7). While 16 (57%) of the total absences were secondary to a known exposure, 12 (43%) were symptomatic without a known exposure. A total of 25 (89%) absent HCCs received COVID-19 testing (N = 5 positive [20%]; N = 20 negative [80%]) with test results returning in 1-10 days. Eleven (39%) symptomatic HCCs had traveled domestically or internationally in the prior 30 days.ConclusionEmergency departments should anticipate substantial HCC absences during the initial surge of a pandemic. Possible interventions to mitigate absences include early and broad use of PPE, planning for many asymptomatic HCC absences secondary to exposures, prioritizing HCC virus testing, and mandating early travel restrictions.
