Clear aligner treatment for a four-year-old patient with anterior cross-bite and facial asymmetry: A case report.

ABSTRACT:

Background

Clear aligners have been widely used to treat malocclusions from crowding, extraction cases to orthodontic-orthognathic cases, and practitioners are exploring the border of it. For the first time, clear aligners were used to early intervene anterior cross-bite and facial asymmetry.

Case summary

This case report described a four-year-old child presented with anterior cross-bite and facial asymmetry associated with functional mandibular shift, who had undergone a failed treatment with conventional appliances. The total treatment time was 18 weeks, and a stable outcome was obtained.

Conclusion

The increasing need in early treatment highlights the need for clinicians to thoroughly investigate for the patient regarding clinical manifestation as well as patient compliance. We hope that our case will be contemplated by clinicians when seeking for treatment alternatives.

Project description:BackgroundThe aim of this study is to conduct a comparative evaluation of different designs of clear aligners and examine the disparities between clear aligners and fixed appliances.Methods3D digital models were created, consisting of a maxillary dentition without first premolars, maxilla, periodontal ligaments, attachments, micro-implant, 3D printed lingual retractor, brackets, archwire and clear aligner. The study involved the creation of five design models for clear aligner maxillary anterior internal retraction and one design model for fixed appliance maxillary anterior internal retraction, which were subsequently subjected to finite element analysis. These design models included: (1) Model C0 Control, (2) Model C1 Posterior Micro-implant, (3) Model C2 Anterior Micro-implant, (4) Model C3 Palatal Plate, (5) Model C4 Lingual Retractor, and (6) Model F0 Fixed Appliance.ResultsIn the clear aligner models, a consistent pattern of tooth movement was observed. Notably, among all tested models, the modified clear aligner Model C3 exhibited the smallest differences in sagittal displacement of the crown-root of the central incisor, vertical displacement of the central incisor, sagittal displacement of the second premolar and second molar, as well as vertical displacement of posterior teeth. However, distinct variations in tooth movement trends were observed between the clear aligner models and the fixed appliance model. Furthermore, compared to the fixed appliance model, significant increases in tooth displacement were achieved with the use of clear aligner models.ConclusionsIn the clear aligner models, the movement trend of the teeth remained consistent, but there were variations in the amount of tooth displacement. Overall, the Model C3 exhibited better torque control and provided greater protection for posterior anchorage teeth compared to the other four clear aligner models. On the other hand, the fixed appliance model provides superior anterior torque control and better protection of the posterior anchorage teeth compared to clear aligner models. The clear aligner approach and the fixed appliance approach still exhibit a disparity; nevertheless, this study offers a developmental direction and establishes a theoretical foundation for future non-invasive, aesthetically pleasing, comfortable, and efficient modalities of clear aligner treatment.

Project description:Introduction and importance Pyogenic Granuloma (PG) commonly presents as a solitary, erythematous, non-tender, skin lesion, usually not exceeding 2.5 cm. Although Surgical excision is the first-line treatment, conservative treatments are recently developing. Solitary PG is well documented unlike multiple PGs, as the latter is rarely described in terms of its presentations and treatments. Case presentation This interesting case report describes a 23-year-old male who developed multiple PGs on the nasal area (2cmx2cm) and mandibular area (10cmx2cm) after sutures removal secondary to a recent history of trauma. The lesions regressed after successfully treated with two months of timolol drops and topical corticosteroids. Subsequently, intralesional corticosteroids injections were given once-per-month for two months, with residual fibrosis of the regressed lesion. Clinical discussion While the most common site for multiple PGs is the interscapular region, this case reports giant, facial, multiple PGs followed suture removal. Although β-blockers were reported to successfully treat solitary PG, only a few cases were raised to discuss this treatment in terms of multiple PGs. This case completes the series and reports successfully-treated multiple PGs using a combination of topical timolol and steroid. Conclusion This case supports the need to consider multiple PGs as a differential diagnosis following sutures removal even when it reaches rarely reported diameters. Also, it suggests topical timolol and steroids as an effective conservative treatment in similar cases of multiple giant PGs. Notably, the treatment failed to sidestep scar formation. Highlights • This report acts as a proof of the efficacy of topical timolol and steroid as a conservative treatment for multiple Pyogenic Granulomas, even when the lesions reach a large diameter.• It is noteworthy that this treatment failed to sidestep scar formation.• This case supports the need to consider Pyogenic Granulomas a differential diagnosis after sutures removal.• This case highlights the fact that multiple Pyogenic Granulomas can reach diameters larger than 2 cm.

Project description:BackgroundRefinements are very common in clear aligner treatments. The aim of this study is to assess whether the predictability of deep overbite correction is similar over several refinements using clear aligners (Invisalign, Align Technology, San Jose, Calif) and examine the accuracy of vertical movement and inclination change of individual teeth.MethodsThis retrospective study included 20 deep bite patients (7M and 13F; 32.63 ± 11.88 years old; an initial overbite of 5.09 ± 0.98 mm), consecutively treated from September 2016 and March 2023, who completed at least two sets of aligners, including refinements. The initial, predicted, and achieved models were exported from ClinCheck or OrthoCAD (Cadent Inc, Carlstadt, NJ) and superimposed via best-fit surface-based registration using SlicerCMF (version 4.9.0; cmf.slicer.org). We also examined 15 out of 20 patients who completed treatments. The overbite correction and changes in vertical movement and inclination for individual teeth were measured. Descriptive statistics and a paired t-test or Wilcoxon signed-rank test were performed. P < 0.05 was considered statistically significant.ResultsThe mean accuracy of overbite correction was 37.63% after 1st set, followed by 11.19%, 6.32%, and 13.80% (2nd-4th sets), respectively. There were statistically significant differences between the predicted and achieved vertical movements and inclination changes for all teeth for the 1st and 2nd sets. For the completed cases, the mean overbite correction was 38.54% compared to the initially planned overbite correction, which is similar to one of the 1st set. Still, the vertical movements and inclination changes of all teeth present statistically significant differences between the initially planned and finally achieved movements except for maxillary lateral incisor torque.ConclusionsThe most overbite correction occurs during the 1st set of aligners, and refinement treatment does not significantly improve the deep bite correction.
