Project description:Athletes often have training-induced remodeling of the heart, and this can sometimes be seen as abnormal but non-pathological changes in the electrocardiogram. However, these changes can be confused with severe cardiovascular diseases that, in some cases, can cause cardiovascular death. Electrocardiogram data from athletes is therefore important to learn more about the difference between normal athletic remodeling and pathological remodeling of the heart. This work provides a dataset of electrocardiograms from 28 Norwegian elite endurance athletes. The electrocardiograms are standard 12-lead resting ECGs, recorded for 10 seconds while the athlete's lay supine on a bench. The electrocardiograms were then interpreted by an interpretation algorithm and by a trained cardiologist. The electrocardiogram waveform data and the interpretations were stored in Python Waveform Database format and made publicly available through PhysioNet.

Project description:RationaleTo evaluate the appropriateness of spirometric and plethysmographic reference equations in healthy young children according to ethnic origin.MethodsSpirometry data were collated in 400 healthy children (214 Black and 186 White) aged 6-12 years. Of these children, 68 Black and 115 White children also undertook plethysmography. Results were expressed as percent predicted according to commonly used equations for spirometry and plethysmography.ResultsBlack children had lower lung function for a given height compared to White children. The magnitude and direction of these differences varied according to specific outcome. In the studied age range (6-12 years) the ethnic-specific Wang equations were adequate for spirometry (mean results approximating 100% predicted in both ethnic groups). By contrast, significant differences were found between observed and % predicted plethysmographic lung volumes according to published equations derived from White children: Among the Black children, function residual capacity (FRC) and total lung capacity (TLC) were on average, 14 and 6% lower than predicted, whereas mean residual volume (RV) and RV/TLC were 4 and 10% higher. Among White children, the Rosenthal equations gave the best fit, with the exception of FRC which was, on average, 9% lower than predicted.ConclusionSpirometry equations may suffice in Black children; however, interpretation of static lung volumes in Black children is limited due to inappropriate reference equations. More appropriate plethysmographic reference equations that are applicable to all ethnic groups across the entire age range are urgently needed.

Project description:BackgroundRace- and ethnicity-based differences in treatment access and outcomes have been reported in the orthopaedic sports medicine literature. However, the rate at which race and ethnicity are reported and incorporated into the statistical analysis of sports medicine studies remains unclear.PurposeTo determine the rate at which race and ethnicity are reported and analyzed in athlete-specific sports medicine literature.Study designSystematic review; Level of evidence, 4.MethodsUsing PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, articles published by the 3 journals of the American Orthopaedic Society for Sports Medicine between 2017 and 2021 were considered for inclusion. Original sports medicine research studies that focused on athletes were included. Outcome measures included reporting and analysis of patient demographics (age, sex, race, ethnicity). Studies that included demographic variables in a multivariate analysis or that performed a race-/ethnicity-based stratified analyses were considered to have analyzed that variable. Studies that reported and/or analyzed patient demographics were examined. Chi-square tests were performed to determine statistical significance.ResultsA total of 5140 publications were screened, and 842 met the inclusion criteria. Age and sex were well reported (84.1% and 87.0%, respectively), while race (3.8%) and ethnicity (2.0%) were poorly reported. There was no difference in rates of reporting age, sex, race, or ethnicity between the American Journal of Sports Medicine (AJSM), the Orthopaedic Journal of Sports Medicine (OJSM), or Sports Health: A Multidisciplinary Approach (Sports Health). The rate of analysis was also calculated as a percentage of the studies that reported that variable. Of the studies that reported age, 38.5% analyzed age. Using this method, 26.2% of studies analyzed sex, 40.6% analyzed race, and 17.6% analyzed ethnicity. Although there was no difference in the overall rate at which studies from the 3 journals analyzed ethnicity, Sports Health studies analyzed age (P = .044), sex (P = .001), and race (P = .027) more frequently than studies published in AJSM and OJSM. Of the studies that analyzed race, most of those studies (8/13, 61.5%) found significant race-based differences in reported outcomes.ConclusionThis systematic review demonstrated that race and ethnicity are poorly reported and analyzed in athlete-specific sports medicine literature, despite the fact that a majority of studies analyzing race found significant differences between racial groups. Improved reporting of race and ethnicity can determine whether race- and ethnicity-based differences exist in patient interventions to ameliorate disparities in patient outcomes.

Project description:BackgroundECGs are often taught without clinical context. However, in the clinical setting, ECGs are rarely interpreted without knowing the clinical presentation. We aimed to determine whether ECG diagnostic accuracy was influenced by knowledge of the clinical context and/or prior clinical exposure to the ECG diagnosis.MethodsFourth- (junior) and sixth-year (senior) medical students, as well as medical residents were invited to complete two multiple-choice question (MCQ) tests and a survey. Test 1 comprised 25 ECGs without case vignettes. Test 2, completed immediately thereafter, comprised the same 25 ECGs and MCQs, but with case vignettes for each ECG. Subsequently, participants indicated in the survey when last, during prior clinical clerkships, they have seen each of the 25 conditions tested. Eligible participants completed both tests and survey. We estimated that a minimum sample size of 165 participants would provide 80% power to detect a mean difference of 7% in test scores, considering a type 1 error of 5%.ResultsThis study comprised 176 participants (67 [38.1%] junior students, 55 [31.3%] senior students, 54 [30.7%] residents). Prior ECG exposure depended on their level of training, i.e., junior students were exposed to 52% of the conditions tested, senior students 63.4% and residents 96.9%. Overall, there was a marginal improvement in ECG diagnostic accuracy when the clinical context was known (Cohen's d = 0.35, p < 0.001). Gains in diagnostic accuracy were more pronounced amongst residents (Cohen's d = 0.59, p < 0.001), than senior (Cohen's d = 0.38, p < 0.001) or junior students (Cohen's d = 0.29, p < 0.001). All participants were more likely to make a correct ECG diagnosis if they reported having seen the condition during prior clinical training, whether they were provided with a case vignette (odds ratio [OR] 1.46, 95% confidence interval [CI] 1.24-1.71) or not (OR 1.58, 95% CI 1.35-1.84).ConclusionECG interpretation using clinical vignettes devoid of real patient experiences does not appear to have as great an impact on ECG diagnostic accuracy as prior clinical exposure. However, exposure to ECGs during clinical training is largely opportunistic and haphazard. ECG training should therefore not rely on experiential learning alone, but instead be supplemented by other formal methods of instruction.

Project description:BackgroundScapular body fractures generally occur as a result of high-energy, direct trauma to the shoulder sustained in automobile accidents. While such mechanisms have been well described, little is known about scapular body fractures sustained during sporting activities.Questions/purposesWe sought to systematically review the literature on scapular body fracture sustained during sporting activity, recording rates and mechanisms of injury, management strategies, and return-to-sport times.MethodsFollowing Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we performed a systematic review of studies conducted between 1985 and 2017. Inclusion criteria were studies examining scapular body fractures sustained during sporting activity, fracture management, and patient outcomes. Exclusion criteria were studies on non-sporting-related fractures and those not reporting fracture management or patient outcomes.ResultsNine studies encompassing ten cases of scapular body fracture sustained during sporting activity were identified, with acute trauma responsible for 70% of fractures. No patient sustained any associated injuries. Fractures were treated conservatively in 90% of cases, with no reported complications. Mean overall time to return to sport was 2.5 months, while no significant difference in return to sport was appreciated in athletes with acute versus fatigue fractures.ConclusionScapular body fractures in athletes occur primarily from muscle contraction against a resisted force in the upper extremity during contact sports. Unlike non-sporting fractures, these fractures usually involve low-energy mechanisms without associated injury and conservative treatment is usually successful.

Project description:BackgroundThe relationship between ethnicity and COVID-19 is uncertain. We performed a systematic review to assess whether ethnicity has been reported in patients with COVID-19 and its relation to clinical outcomes.MethodsWe searched EMBASE, MEDLINE, Cochrane Library and PROSPERO for English-language citations on ethnicity and COVID-19 (1st December 2019-15th May 2020). We also reviewed: COVID-19 articles in NEJM, Lancet, BMJ, JAMA, clinical trial protocols, grey literature, surveillance data and preprint articles on COVID-19 in MedRxiv to evaluate if the association between ethnicity and clinical outcomes were reported and what they showed. PROSPERO:180654.FindingsOf 207 articles in the database search, five reported ethnicity; two reported no association between ethnicity and mortality. Of 690 articles identified from medical journals, 12 reported ethnicity; three reported no association between ethnicity and mortality. Of 209 preprints, 34 reported ethnicity - 13 found Black, Asian and Minority Ethnic (BAME) individuals had an increased risk of infection with SARS-CoV-2 and 12 reported worse clinical outcomes, including ITU admission and mortality, in BAME patients compared to White patients. Of 12 grey literature reports, seven with original data reported poorer clinical outcomes in BAME groups compared to White groups.InterpretationData on ethnicity in patients with COVID-19 in the published medical literature remains limited. However, emerging data from the grey literature and preprint articles suggest BAME individuals are at an increased risk of acquiring SARS-CoV-2 infection compared to White individuals and also worse clinical outcomes from COVID-19. Further work on the role of ethnicity in the current pandemic is of urgent public health importance.FundingNIHR.

Project description:BackgroundElectrocardiograms (ECGs) in athletes commonly reveal findings related to physiologic adaptations to exercise, that may be difficult to discern from true underlying cardiovascular abnormalities. North American and European societies have published consensus statements for normal, borderline, and abnormal ECG findings for athletes, but these criteria are not based on established correlation with disease states. Additionally, data comparing ECG findings in athletes to non-athlete control subjects are lacking. Our objective was to compare the ECGs of collegiate athletes and non-athlete controls using Z-scores for digital ECG variables to better identify significant differences between the groups and to evaluate the ECG variables in athletes falling outside the normal range.MethodsValues for 102 digital ECG variables on 7206 subjects aged 17-22 years, including 672 athletes, from Hawaii Pacific Health, University of Hawaii, and Rady Children's Hospital San Diego were obtained through retrospective review. Age and sex-specific Z-scores for ECG variables were derived from normal subjects and used to assess the range of values for specific ECG variables in young athletes. Athletes with abnormal ECGs were referred to cardiology consultation and/or echocardiogram.ResultsAthletes had slower heart rate, longer PR interval, more rightward QRS axis, longer QRS duration but shorter QTc duration, larger amplitude and area of T waves, prevalent R' waves in V1, and higher values of variables traditionally associated with left ventricular hypertrophy (LVH): amplitudes of S waves (leads V1-V2), Q waves (V6, III) and R waves (II, V5, V6). Z-scores of these ECG variables in 558 (83%) of the athletes fell within - 2.5 and 2.5 range derived from the normal population dataset, and 60 (8.9%) athletes had a Z-score outside the - 3 to 3 range. While 191 (28.4%) athletes met traditional voltage criteria for diagnosis of LVH on ECG, only 53 athletes (7.9%) had Z-scores outside the range of -2.5 to 2.5 for both S amplitude in leads V1-V2 and R amplitude in leads V5-6. Only one athlete was diagnosed with hypertrophic cardiomyopathy with a Z-score of R wave in V6 of 2.34 and T wave in V6 of -5.94.ConclusionThe use of Z-scores derived from a normal population may provide more precise screening to define cardiac abnormalities in young athletes and reduce unnecessary secondary testing, restrictions and concern.

Project description:Objective: This review aims to identify, appraise, and synthesize studies reporting the relationship between mindfulness and athlete burnout and the effects of mindfulness-based interventions (MBIs) on athlete burnout. Methods: Studies were identified through searching six electronic databases using combinations of three groups of keywords and manual search. Two independent reviewers screened the searched studies, extracted data of the included studies, and assessed the study quality. The extracted data were synthesized qualitatively and quantitatively. Results: Ten studies consisting of two controlled trials, six surveys, and two interview studies met the inclusion criteria. The two controlled trials had weak methodological quality, and the remaining studies were of moderate to high research quality. Results of controlled trials and interview research generally showed that MBIs had positive effects in burnout prevention. Meta-analytic results indicated a negative association between mindfulness and burnout. Conclusions: There is some evidence showing that mindfulness was negatively associated with athlete burnout. However, given the small number of interventions and qualitative studies, there is limited evidence on whether MBIs are useful in preventing athlete burnout. More studies are needed to corroborate these findings.

Project description:BackgroundAthlete monitoring has become an important aspect of demanding indoor team sports like handball. Monitoring external and internal load and well-being should help coaches to optimize load management and recovery strategies to improve athletic performance and maintain athlete health. However, there is a large variety in the methods described, their coherence, and their effect on performance and health. This systematic review will summarize the methods currently used to monitor external and internal load and well-being during training and competition in team handball.MethodsThis systematic review will follow the current Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines. A comprehensive search of electronic databases, including MEDLINE, Embase, and Web of Science, will be conducted until 30 April 2024. Original articles in English, German, or Spanish that assess methods for monitoring the external and/or internal load and/or well-being of healthy team handball players during training and/or competition will be considered. In addition, a backward search of all relevant full-text articles identified through the search will be conducted. Two reviewers will independently perform study selection, data extraction, and risk-of-bias assessment for included studies. Included studies will be assessed for risk of bias using the Newcastle-Ottawa scale. It is anticipated that the number of eligible studies will be limited, and that the data extracted from them will be highly heterogeneous. Consequently, a narrative synthesis of the extracted data will be presented.DiscussionThis systematic review will provide the current evidence on methods for monitoring external and internal load and well-being in competitive team handball players. The results may assist practitioners in implementing an athlete monitoring. We will share our findings through scientific conferences, educational meetings for coaches and medical staff, expert meetings, and publication in a peer-reviewed journal.Systematic review registrationPROSPERO CRD42024540676.

Project description:Cardiovascular diseases are the leading cause of death, globally. Stroke and heart attacks account for more than 80% of cardiovascular disease-related deaths. To prevent patient mismanagement and potentially save lives, effective screening at an early stage is needed. Diagnosis is typically made using an electrocardiogram (ECG) analysis. However, ECG recordings are often corrupted by different types of noise, degrading the quality of the recording and making diagnosis more difficult. This paper reviews research on automatic ECG quality assessment techniques used in studies published from 2012-2022. The CinC11 Dataset is most often used for training and testing algorithms. Only one study tested its algorithm on people in real-time, but it did not specify the demographic data of the subjects. Most of the reviewed papers evaluated the quality of the ECG recordings per single lead. The accuracy of the algorithms reviewed in this paper range from 85.75% to 97.15%. More clarity on the research methods used is needed to improve the quality of automatic ECG quality assessment techniques and implement them in a clinical setting. This paper discusses the possible shortcomings in current research and provides recommendations on how to advance the field of automatic ECG quality assessment.
