Cross-cultural adaptation and validation of the Kerlan-Jobe Orthopaedic Clinic shoulder and elbow score in Finnish-speaking overhead athletes.

ABSTRACT:

Background

The Kerlan-Jobe Orthopaedic Clinic Shoulder and Elbow score (KJOC) is developed to evaluate the shoulder and elbow function in overhead athletes. To date, the score has not been adapted into Finnish language. The aim of this study was to perform a cross-cultural adaptation of the Kerlan-Jobe Orthopaedic Clinic Shoulder and Elbow score (KJOC) into Finnish language and evaluate its validity, reliability, and responsiveness in overhead athletes.

Methods

Forward-backward translation method was followed in the cross-cultural adaptation process. Subsequently, 114 overhead athletes (52 males, 62 females, mean age 18.1 ± 2.8 years) completed the Finnish version of KJOC score, Disabilities of the Arm, Shoulder and Hand (DASH), American Shoulder and Elbow Surgeons Standardized Shoulder Assessment Form (ASES) and RAND-36 to assess validity of the KJOC score. To evaluate reliability and responsiveness, the participants filled in the KJOC score 16 days and eight months after the first data collection. Validity, reliability, and responsiveness of the Finnish KJOC score were statistically tested.

Results

Minor modifications were made during the cross-cultural translation and adaptation process, which were related to culture specific terminology in sports and agreed by an expert committee. Construct validity of the KJOC score was moderate to high, based on the correlations with DASH (r = - 0.757); DASH sports module (r = - 0.667); ASES (r = 0.559); and RAND-36 (r = 0.397) questionnaires. Finnish KJOC score showed excellent internal consistency (α = 0.92) and good test-retest reliability (2-way mixed-effects model ICC = 0.77) with acceptable measurement error level (SEM 5.5; MDC 15.1). Ceiling effect was detected for asymptomatic athletes in each item (23.2-61.1%), and for symptomatic athletes in item 5 (47.4%). Responsiveness of the Finnish KJOC score could not be confirmed due to conflicting follow-up results.

Conclusion

The Finnish KJOC score was found to be a valid and reliable questionnaire measuring the self-reported upper arm status in Finnish-speaking overhead athletes.

Project description:BackgroundThe Kerlan-Jobe Orthopaedic Clinic shoulder and elbow score (KJOC) is sensitive enough to detect subtle changes in the functional level of the shoulder or elbow in overhead athletes. However, a Japanese version of the KJOC is not yet available. Moreover, the original KJOC uses a visual analog scale (VAS) assessment format that requires a print version so that users can add a check mark along a horizontal line. The KJOC can be completed using online tools such as Google Forms if a 10-point scale format can be used with comparable results.PurposesTo (1) translate and cross-culturally adapt the KJOC into Japanese (KJOC-J) and assess its validity and (2) evaluate the correlation between a standard VAS format and a 10-point scale format (KJOC-J10).Study designCohort study (Diagnosis); Level of evidence, 3.MethodsIn total, 318 athletes participated in the validation study, 255 in the test-retest reliability study for the KJOC-J, and 273 in the validation study for the KJOC-J10. Internal consistency was evaluated using the Cronbach alpha. Validity was evaluated by calculating the correlation between the KJOC-J and the Disability of the Arm, Shoulder and Hand (DASH). Test-retest reliability was assessed using the intraclass correlation coefficient (ICC). The validity of the KJOC-J10 was evaluated by calculating the correlation between the KJOC-J10 and KJOC-J.ResultsThe Cronbach alpha was 0.914, indicating good internal consistency. The KJOC-J was moderately correlated with the DASH (r = -0.581; P < .001) and had good test-retest reliability (ICC, 0.874). A strong correlation was found between the KJOC-J and KJOC-J10 (r = 0.846; P < .001).ConclusionThe study results demonstrated good internal consistency, validity, and reliability for the KJOC-J, indicating that it is a valid assessment tool for shoulder and elbow functions in Japanese overhead athletes. Moreover, a strong correlation was found between the VAS and 10-point formats of the KJOC-J.

Project description:ContextUpper extremity (UE) musculoskeletal injuries are common in baseball athletes due to the increased demand placed on the UE. The link between risk factors for UE musculoskeletal injuries and baseball athletes' perceived UE function and pain, as measured by the Kerlan-Jobe Orthopaedic Clinic (KJOC) questionnaire, is unclear.ObjectiveTo (1) describe the musculoskeletal characteristics of the UE (posture, range of motion, flexibility, and isometric strength) in a population of baseball athletes and (2) determine the predictive capability of UE musculoskeletal characteristics for the KJOC score in these athletes.DesignCohort study.SettingAthletic training room.Patients or other participantsA total of 37 male National Collegiate Athletic Association Division I baseball athletes (age = 20.10 ± 1.27 years, height = 186.96 ± 7.64 cm, mass = 90.60 ± 10.69 kg).Intervention(s)Athletes self-reported all shoulder musculoskeletal injuries and completed the KJOC questionnaire. Project description:Background: Shoulder impingement syndrome (SIS) is a prevalent condition among overhead athletes, often managed through therapeutic exercise interventions. However, the quality of reporting in exercise protocols significantly impacts their reproducibility and clinical implementation. The Consensus for Exercise Reporting Template (CERT) provides a standardized framework to assess the quality of exercise reporting in clinical research. Objectives: This systematic review aimed to evaluate the quality of exercise protocols used to treat SIS in overhead athletes by applying the CERT checklist. Additionally, the risk of bias was assessed to determine the methodological rigor of included studies. Methods: A systematic review was conducted following PRISMA guidelines. Six electronic databases (MEDLINE, CINAHL, Sport Discuss, Web of Science, and Cochrane) were searched for eligible studies. Inclusion criteria encompassed randomized controlled trials (RCTs), cohort studies, and case series that investigated exercise therapy for SIS in overhead athletes. Studies had to be published in English and provide details on exercise interventions. Exclusion criteria included non-human studies, acute injuries, and postoperative management. The primary outcome was the quality of intervention reporting, assessed using the CERT checklist. The secondary outcome was the risk of bias, evaluated using the modified Downs and Black checklist. Results: Five studies met the inclusion criteria, comprising four RCTs and one case series. CERT scores ranged from 6 to 13 (median = 8, IQR = 1), indicating suboptimal reporting quality. Commonly reported CERT items included equipment usage and exercise tailoring. However, key aspects such as adherence, motivation, and intervention fidelity were consistently underreported. None of the included studies provided comprehensive details on exercise interventions as per CERT guidelines, limiting their reproducibility and clinical application. Conclusions: The quality of reporting on exercise-based interventions for SIS in overhead athletes remains insufficient. Critical gaps in adherence monitoring, patient motivation, and intervention fidelity were identified. Future research should prioritize standardized and detailed reporting of exercise interventions to enhance reproducibility and facilitate evidence-based clinical practice.
