Background

Elective scrotal surgery is associated with a high rate of postoperative complications. There is no specific recommendation for postoperative care.

Aim

We investigated whether support underwear has an impact on postoperative complications and quality of life.

Materials and methods

From July 2020 to November 2021, patients with prior elective scrotal surgery were randomized into the intervention group "support underwear" or the control group. In addition to patient characteristics, intraoperative and postoperative findings were documented. The primary endpoint comprised postoperative complications. Secondary endpoints were prolonged length of hospital stay, emergency visits, unplanned readmissions, increased use of analgesics, and quality of life, which was recorded using the EQ5D (European Quality of Life 5 Dimensions) questionnaire preoperatively, on day 1 and 4 weeks postoperatively.

Results

Data from 50 patients were analyzed. The mean age was 46.7 years (standard deviation [SD] 18.6). Inguinal surgery with/without orchiectomy (52%), hydrocele resection (22%), or ligation of varicocele (14%) were performed most frequently. The mean operating time was 62.8 min (SD 35.2); length hospital stay was 2.6 days (SD 1.2). In all, 20% of the patients suffered a postoperative complication. Type of surgery was significantly associated with postoperative complications (p = 0.01) and unplanned readmission (p = 0.04). Regarding biometric and perioperative data, there were no significant differences between the interventional group (n = 27) and control group (n = 23).

Conclusion

A nonnegligible number of complications occurs after elective scrotal surgery. Complications affects quality of life up to 4 weeks after the surgery. Postoperative care with support underwear does not appear to affect the postoperative complication rate, but it positively influences the quality of life in patients with scrotal access.

Project description:PurposeTo compare 60-day complication rates, radiographic outcomes, and clinical outcomes following primary THA with conventional versus shortened stems, in a large cohort study.MethodsThe authors reviewed a consecutive series of 800 primary THAs, of which 781 met the inclusion/exclusion criteria: 395 received a conventional stem and 386 received a shortened stem. Intraoperative and postoperative complications were noted. Radiographic and clinical assessments were performed preoperatively and 60 days after surgery.ResultsCompared to conventional stems, shortened stems had significantly less intraoperative complications (2.8% vs 0.3%, p = 0.006), but no significant differences in complications that did not require reoperation (1.0% vs 1.3%, p = 0.620), complications that required reoperation without stem revision (2.0% vs 1.0%, p = 0.384), and complications that required stem revision (0.5% vs 0.5%, p = 1.000). Four hips (two from each group) required stem revision and were thus excluded from 60-day assessment. There were no significant differences between groups in subsidence ≥ 3 mm (1.0% vs 0.5%, p = 0.686), alignment (90.3%vs 86.7%, p = 0.192), net change in offset (within 3 mm, 32.3% vs 30.5%, p = 0.097), and limb length discrepancy (3.0 ± 2.6 mm vs 2.9 ± 2.4 mm, p = 0.695). Compared to conventional stems, shortened stems had significantly better preoperative mHHS (56.5 ± 18.5 vs 64.5 ± 13.5, p < 0.001), and significantly lower net improvement in mHHS (29.9 ± 17.1 vs 24.4 ± 15.0, p < 0.001), but no significant differences in postoperative mHHS (87.3 ± 11.9 vs 89.4 ± 9.6, p = 0.109).ConclusionsThere were no significant differences between conventional and shortened stems in terms of postoperative complication rates, radiographic outcomes, and postoperative mHHS. Project description:In the context of translational research, there is growing interest in studying surgical orthopedic pain management approaches that are common to humans and dogs. The validity of postoperative pain assessment methods is uncertain with regards to responsiveness and the potential interference of analgesia. The hypothesis was that video analysis (as a reference), electrodermal activity, and two subjective pain scales (VAS and 4A-VET) would detect different levels of pain intensity in dogs after a standardized trochleoplasty procedure. In this prospective, blinded, randomized study, postoperative pain was assessed in 25 healthy dogs during a 48-hour time frame (T). Pain was managed with placebo (Group 1, n?=?10), preemptive and multimodal analgesia (Group 2, n?=?5), or preemptive analgesia consisting in oral tramadol (Group 3, n?=?10). Changes over time among groups were analyzed using generalized estimating equations. Multivariate regression tested the significance of relationships between pain scales and video analysis. Video analysis identified that one orthopedic behavior, namely 'Walking with full weight bearing' of the operated leg, decreased more in Group 1 at T24 (indicative of pain), whereas three behaviors indicative of sedation decreased in Group 2 at T24 (all p<0.004). Electrodermal activity was higher in Group 1 than in Groups 2 and 3 until T1 (p<0.0003). The VAS was not responsive. 4A-VET showed divergent results as its orthopedic component (4A-VETleg) detected lower pain in Group 2 until T12 (p<0.0009), but its interactive component (4A-VETbeh) was increased in Group 2 from T12 to T48 (p<0.001). Concurrent validity established that 4A-VETleg scores the painful orthopedic condition accurately and that pain assessment through 4A-VETbeh and VAS was severely biased by the sedative side-effect of the analgesics. Finally, the video analysis offered a concise template for assessment in dogs with acute orthopedic pain. However, subjective pain quantification methods and electrodermal activity need further investigation.

Project description:BackgroundParotid gland tumors (PGTs) are the most common benign neoplasms of salivary gland tumor. Due to its tendency to affect the aesthetic appearance of the face, patients with PGTs are often prone to negative emotions, which seriously affects their recovery. However, there are fewer studies on risk factors associated with the development of anxiety and depression in patients. This study aimed to analyze the risk factors for negative emotions in the perioperative period of PGT patients and their effects on postoperative complication and quality of life and provide a reference basis for improving patient prognosis.MethodsWe retrospectively analyzed 186 patients with PGT admitted to our hospital from August 2017 to August 2021. We evaluated and compiled the general clinical characteristics of the patients. The t-test and chi-square test were used to analyze the relationship between patients' negative emotions and prognosis. Multiple linear regression was used to analyze the independent risk factors for negative emotions and poor prognosis of patients in the perioperative period. Patients were followed up 6 months after discharge by outpatient or telephone follow-up.ResultsAmong 186 patients with PGT, a total of 43 (23.1%) patients showed symptoms of anxiety and 46 (24.7%) patients showed depression. Multiple linear regression analysis showed that the patients' education (P=0.001), with or without hypertension (P=0.048), tumor characteristics (P=0.001), postoperative complications of transient or permanent facial palsy (P=0.008, P=0.027), and recurrence (P=0.018) were independent risk factors for anxiety among patients, while the patients' educational level (P=0.018), tumor metastasis (P=0.044), and postoperative Frey syndrome (P=0.011) were independent risk factors for depression among patients. In addition, we found that the patients' negative emotions (P=0.000), educational level (P=0.015), residency (P=0.027), with or without hyperlipidemia (P=0.042), tumor characteristics (P=0.000), surgery time (P=0.037), and postoperative complications (P=0.041) were independent risk factors for patient prognosis.ConclusionsPGT patients are prone to various postoperative complications and have a high likelihood of having negative emotions and seriously affects the prognosis. Clinical efforts should pay attention to patients' emotions and demeanor, the identification of relevant risk factors, the adoption of targeted measures to alleviate patients' anxiety and depression, and improve the prognosis.
