Chronic Use of a Sensitized Bionic Hand Does Not Remap the Sense of Touch.

ABSTRACT: Electrical stimulation of tactile nerve fibers that innervated an amputated hand results in vivid sensations experienced at a specific location on the phantom hand, a phenomenon that can be leveraged to convey tactile feedback through bionic hands. Ideally, electrically evoked sensations would be experienced on the appropriate part of the hand: touch with the bionic index fingertip, for example, would elicit a sensation experienced on the index fingertip. However, the perceived locations of sensations are determined by the idiosyncratic position of the stimulating electrode in the nerve and thus are difficult to predict or control. This problem could be circumvented if perceived sensations shifted over time to become consistent with the position of the sensor that triggers them. We show that, after long-term use of a neuromusculoskeletal prosthesis that featured a mismatch between the sensor location and the resulting tactile experience, the perceived location of the touch did not change.

Restoring the sense of touch with a prosthetic hand through a brain interface.

Project description:Our ability to manipulate objects dexterously relies fundamentally on sensory signals originating from the hand. To restore motor function with upper-limb neuroprostheses requires that somatosensory feedback be provided to the tetraplegic patient or amputee. Given the complexity of state-of-the-art prosthetic limbs and, thus, the huge state space they can traverse, it is desirable to minimize the need for the patient to learn associations between events impinging on the limb and arbitrary sensations. Accordingly, we have developed approaches to intuitively convey sensory information that is critical for object manipulation--information about contact location, pressure, and timing--through intracortical microstimulation of primary somatosensory cortex. In experiments with nonhuman primates, we show that we can elicit percepts that are projected to a localized patch of skin and that track the pressure exerted on the skin. In a real-time application, we demonstrate that animals can perform a tactile discrimination task equally well whether mechanical stimuli are delivered to their native fingers or to a prosthetic one. Finally, we propose that the timing of contact events can be signaled through phasic intracortical microstimulation at the onset and offset of object contact that mimics the ubiquitous on and off responses observed in primary somatosensory cortex to complement slowly varying pressure-related feedback. We anticipate that the proposed biomimetic feedback will considerably increase the dexterity and embodiment of upper-limb neuroprostheses and will constitute an important step in restoring touch to individuals who have lost it.

Elective amputation and bionic substitution restore functional hand use after critical soft tissue injuries.

Project description:Critical soft tissue injuries may lead to a non-functional and insensate limb. In these cases standard reconstructive techniques will not suffice to provide a useful outcome, and solutions outside the biological arena must be considered and offered to these patients. We propose a concept which, after all reconstructive options have been exhausted, involves an elective amputation along with a bionic substitution, implementing an actuated prosthetic hand via a structured tech-neuro-rehabilitation program. Here, three patients are presented in whom this concept has been successfully applied after mutilating hand injuries. Clinical tests conducted before, during and after the procedure, evaluating both functional and psychometric parameters, document the benefits of this approach. Additionally, in one of the patients, we show the possibility of implementing a highly functional and natural control of an advanced prosthesis providing both proportional and simultaneous movements of the wrist and hand for completing tasks of daily living with substantially less compensatory movements compared to the traditional systems. It is concluded that the proposed procedure is a viable solution for re-gaining highly functional hand use following critical soft tissue injuries when existing surgical measures fail. Our results are clinically applicable and can be extended to institutions with similar resources.

Contribution of interaction force to the sense of hand ownership and the sense of hand agency.

Project description:When performing willed actions, we have the unified and coherent experience of owning and controlling our body. Body ownership is believed to emerge from the integration of coherent multisensory signals, while agency is believed to emerge from the coherence between predicted and perceived outcomes of actions. As a consequence, body ownership and agency can both be modulated by multisensory conflicts. The contribution of active movement generation to ownership and agency has not been parametrically explored. Here, we investigated the contribution of interaction force between the agent and the environment to the sense of hand ownership (SO) and the sense of hand agency (SA). By combining robotics and virtual reality, we manipulated the sensorimotor and visual information during immersive scenarios to induce and quantify altered states of SO and SA. First, we demonstrated that SO and SA could be successfully manipulated by our experimental paradigms. Second, we showed that interaction force strongly contributes to SA, but to a lesser extent to SO. Finally, we showed that SO and SA interact beyond their common multisensory basis. Our results, based on two independent studies, provide a direct link between sensorimotor interactions and subjective body experience and demonstrate a new dissociation between SO and SA.

Biomimetic encoding model for restoring touch in bionic hands through a nerve interface.

Project description:ObjectiveHand function can be restored in upper-limb amputees by equipping them with anthropomorphic prostheses controlled with signals from residual muscles. The dexterity of these bionic hands is severely limited in large part by the absence of tactile feedback about interactions with objects. We propose that, to the extent that artificial touch mimics its natural counterpart, these sensory signals will be more easily integrated into the motor plan for object manipulation.ApproachWe describe an approach to convey tactile feedback through electrical stimulation of the residual somatosensory nerves that mimics the aggregate activity of tactile fibers that would be produced in the nerve of a native hand during object interactions. Specifically, we build a parsimonious model that maps the stimulus-described as time-varying indentation depth, indentation rate, and acceleration-into continuous estimates of the time-varying population firing rate and of the size of the recruited afferent population.Main resultsThe simple model can reconstruct aggregate afferent responses to a wide range of stimuli, including those experienced during activities of daily living.SignificanceWe discuss how the proposed model can be implemented with a peripheral nerve interface and anticipate it will lead to improved dexterity for prosthetic hands.

Clinical implementation of a bionic hand controlled with kineticomyographic signals.

Project description:Sensing the proper signal could be a vital piece of the solution to the much evading attributes of prosthetic hands, such as robustness to noise, ease of connectivity, and intuitive movement. Towards this end, magnetics tags have been recently suggested as an alternative sensing mechanism to the more common EMG signals. Such sensing technology, however, is inherently invasive and hence only in simulation stages of magnet localization to date. Here, for the first time, we report on the clinical implementation of implanted magnetic tags for an amputee's prosthetic hand from both the medical and engineering perspectives. Specifically, the proposed approach introduces a flexor-extensor tendon transfer surgical procedure to implant the tags, artificial neural networks to extract human intention directly from the implanted magnet's magnetic fields -in short KineticoMyoGraphy (KMG) signals- rather than localizing them, and a game strategy to examine the proposed algorithms and rehabilitate the patient with his new prosthetic hand. The bionic hand's ability is then tested following the patient's intended gesture type and grade. The statistical results confirm the possible utility of surgically implanted magnetic tags as an accurate sensing interface for recognizing the intended gesture and degree of movement between an amputee and his bionic hand.

Mechanical control of the sense of touch by β-spectrin.

Project description:The ability to sense and respond to mechanical stimuli emanates from sensory neurons and is shared by most, if not all, animals. Exactly how such neurons receive and distribute mechanical signals during touch sensation remains mysterious. Here, we show that sensation of mechanical forces depends on a continuous, pre-stressed spectrin cytoskeleton inside neurons. Mutations in the tetramerization domain of Caenorhabditis elegans β-spectrin (UNC-70), an actin-membrane crosslinker, cause defects in sensory neuron morphology under compressive stress in moving animals. Through atomic force spectroscopy experiments on isolated neurons, in vivo laser axotomy and fluorescence resonance energy transfer imaging to measure force across single cells and molecules, we show that spectrin is held under constitutive tension in living animals, which contributes to elevated pre-stress in touch receptor neurons. Genetic manipulations that decrease such spectrin-dependent tension also selectively impair touch sensation, suggesting that such pre-tension is essential for efficient responses to external mechanical stimuli.

Biomimetic versus arbitrary motor control strategies for bionic hand skill learning.

Project description:A long-standing engineering ambition has been to design anthropomorphic bionic limbs: devices that look like and are controlled in the same way as the biological body (biomimetic). The untested assumption is that biomimetic motor control enhances device embodiment, learning, generalization and automaticity. To test this, we compared biomimetic and non-biomimetic control strategies for non-disabled participants when learning to control a wearable myoelectric bionic hand operated by an eight-channel electromyography pattern-recognition system. We compared motor learning across days and behavioural tasks for two training groups: biomimetic (mimicking the desired bionic hand gesture with biological hand) and arbitrary control (mapping an unrelated biological hand gesture with the desired bionic gesture). For both trained groups, training improved bionic limb control, reduced cognitive reliance and increased embodiment over the bionic hand. Biomimetic users had more intuitive and faster control early in training. Arbitrary users matched biomimetic performance later in training. Furthermore, arbitrary users showed increased generalization to a new control strategy. Collectively, our findings suggest that biomimetic and arbitrary control strategies provide different benefits. The optimal strategy is probably not strictly biomimetic, but rather a flexible strategy within the biomimetic-to-arbitrary spectrum, depending on the user, available training opportunities and user requirements.

Losing one's hand: visual-proprioceptive conflict affects touch perception.

Project description:BACKGROUND: While the sense of bodily ownership has now been widely investigated through the rubber hand illusion (RHI), very little is known about the sense of disownership. It has been hypothesized that the RHI also affects the ownership feelings towards the participant's own hand, as if the rubber hand replaced the participant's actual hand. Somatosensory changes observed in the participants' hand while experiencing the RHI have been taken as evidence for disownership of their real hand. Here we propose a theoretical framework to disambiguate whether such somatosensory changes are to be ascribed to the disownership of the real hand or rather to the anomalous visuo-proprioceptive conflict experienced by the participant during the RHI. METHODOLOGY/PRINCIPAL FINDINGS: In experiment 1, reaction times (RTs) to tactile stimuli delivered to the participants' hand slowed down following the establishment of the RHI. In experiment 2, the misalignment of visual and proprioceptive inputs was obtained via prismatic displacement, a situation in which ownership of the seen hand was doubtless. This condition slowed down the participants' tactile RTs. Thus, similar effects on touch perception emerged following RHI and prismatic displacement. Both manipulations also induced a proprioceptive drift, toward the fake hand in the first experiment and toward the visual position of the participants' hand in the second experiment. CONCLUSIONS/SIGNIFICANCE: These findings reveal that somatosensory alterations in the experimental hand resulting from the RHI result from cross-modal mismatch between the seen and felt position of the hand. As such, they are not necessarily a signature of disownership.

Dual-Hand Motion Capture by Using Biological Inspiration for Bionic Bimanual Robot Teleoperation.

Project description:Bionic bimanual robot teleoperation can transfer the grasping and manipulation skills of human dual hands to the bionic bimanual robots to realize natural and flexible manipulation. The motion capture of dual hands plays an important role in the teleoperation. The motion information of dual hands can be captured through the hand detection, localization, and pose estimation and mapped to the bionic bimanual robots to realize the teleoperation. However, although the motion capture technology has achieved great achievements in recent years, visual dual-hand motion capture is still a great challenge. So, this work proposed a dual-hand detection method and a 3-dimensional (3D) hand pose estimation method based on body and hand biological inspiration to achieve convenient and accurate monocular dual-hand motion capture and bionic bimanual robot teleoperation. First, a dual-hand detection method based on body structure constraints is proposed, which uses a parallel structure to combine hand and body relationship features. Second, a 3D hand pose estimation method with bone-constraint loss from single RGB images is proposed. Then, a bionic bimanual robot teleoperation method is designed by using the proposed hand detection and pose estimation methods. Experiment results on public hand datasets show that the performances of the proposed hand detection and 3D hand pose estimation outperform state-of-the-art methods. Experiment results on a bionic bimanual robot teleoperation platform shows the effectiveness of the proposed teleoperation method.

Effects of touch-screen technology usage on the hand skills dataset.

Project description:This data article describes the hand skills of pre-school children between five and six years old from five schools under the Ministry of Education Malaysia. These data may be used in a journal article later to show the effects of touch-screen technology usage on hand skills of pre-school children. Demographic characteristics, hand skills ability and frequency of touch-screen technology usage data that was collected from August to September 2019. These data may be used in a future systematic review, meta-analysis and meta-regression analysis to conclude the effects of touch-screen technology usage on children's hand skills. Parents, teachers and health practitioners may refer to these data to note the effects of touch-screen technology usage on hand skills of pre-school children.
