Project description:Intramuscular lipoma is usually growing infiltratively and can affect any muscle, although the proximal muscles of the extremities are mostly commonly involved. This lesion can occur in the pronator quadratus. Because of muscle is small and deep seated, the intramuscular lipoma usually involves the whole muscle and even has an extramuscular component before it presents clinically. In this Technical Note, we report the technique of endoscopically assisted myectomy of the pronator quadratus for infiltrating intramuscular lipoma of the muscle. This has the advantage of minimally invasive surgery of minimal soft tissue dissection and fewer soft tissue complications.

Project description:Intramuscular lipoma is a rare soft-tissue tumor with an overall incidence of less than 1% of all lipomas. It primarily occurs in the trunk and proximal parts of the upper and lower extremities. Intramuscular lipoma of the soleus muscle has not been reported before in the English literature and the best treatment option is not known. The purpose of this Technical Note is to describe the details of endoscopically assisted resection of intramuscular lipoma of the soleus. This is indicated for well demarcated intramuscular lipoma of the soleus and is contraindicated in infiltrating intramuscular lipoma. Besides the advantage of minimally invasive surgery, this endoscopic technique allows en bloc resection of the intramuscular lipoma.

Project description:Lipoma is the most frequent benign soft-tissue tumor. It originates from fat cells. According to position, it is distinguished in superficial, typically subcutaneous lipoma, or deep, such as intramuscular lipoma. This latter form is infrequently and may resemble well-differentiated liposarcoma. For this reason, early radiological detection and characterization are necessary to obtain a wide complete resection and histopathologic evaluation to differentiate benign from malign lesions. We report an extremely rare case of an intrathoracic intramuscular lipoma of the chest wall detect and characterize with chest X-ray and computed tomography examinations, resected with thoracoscopic intervention and confirmed with histopathologic analysis.

Project description:IntroductionMagnetic Resonance Imaging (MRI) is the best approach to investigate the hypothalamic-pituitary region in children with central precocious puberty (CPP). Routine scanning is controversial in girls aged 6-8 year, due to the overwhelming prevalence of idiopathic forms and unrelated incidentalomas. Cerebral lipomas are rare and accidental findings, not usually expected in CPP. We report a girl with CPP and an unusually shaped posterior pituitary gland on SE-T1w sequences.Case descriptionA 7.3-year-old female was referred for breast development started at age 7. Her past medical history and physical examination were unremarkable, apart from the Tanner stage 2 breast. X-ray of the left-hand revealed a bone age 2-years ahead of her chronological age, projecting her adult height prognosis below the mid parental height. LHRH test and pelvic ultrasound were suggestive for CPP. Routine brain MRI sequences, SE T1w and TSE T2w, showed the posterior pituitary bright spot increased in size and stretched upward. The finding was considered as an anatomical variant, in an otherwise normal brain imaging. Patient was started on treatment with GnRH analogue. At a thorough revaluation, imaging overlap with adipose tissue was suspected and a new MRI scan with 3D-fat-suppression T1w-VIBE sequences demonstrated a lipoma of the tuber cinereum, bordering a perfectly normal neurohypophysis. 3D-T2w-SPACE sequences, acquired at first MRI scan, would have provided a more correct interpretation if rightly considered.ConclusionThis is the first evidence, to our knowledge, of a cerebral lipoma mimicking pituitary gland abnormalities. Our experience highlights the importance of considering suprasellar lipomas in the MRI investigation of children with CPP, despite their rarity, should the T1w sequences show an unexpected pituitary shape. 3D-T2w SPACE sequences could be integrated into standard ones, especially when performing MRI routinely, to avoid potential misinterpretations.

Project description:Abductor tendon tears are one of the common causes of recalcitrant laterally based hip pain and dysfunction. In most cases, abductor tendon tears are associated with chronic nontraumatic tearing of the gluteus medius tendon. Restoring abductor function of the hip by primary repair of the gluteus medius tendon has been reported to have good and excellent outcomes. However, primary repair might not be as effective for chronic detachment of the gluteus medius tendon with a wide separation from the femoral footprint or severe tendon loss. The lack of tendinous foot for repair and the intrinsically degenerative condition of the tendon may create high tension at the repair site thereby predisposing to surgical failure. We believe that the use of soft-tissue allograft from the Achilles tendon or human dermal allograft may help strengthen the surgical site. We describe a superior gluteal reconstruction technique that is suitable for cases with abductor tendon tear with severe tendon loss.

Project description:This article presents the authors' technique for evaluation of possible abductor tears with magnetic resonance imaging (MRI). By using T2 fat-saturated coronal and axial slices of the hip and T1 coronal images of the pelvis, the authors show how to visualize all tears of the abductor tendons and signs of chronic abductor deficiency. After diagnosis has been made, signs that may help the surgeon determine if open or arthroscopic surgery would be best used are reviewed. Finally, the author reviews indications for a gluteus medius repair with concomitant gluteus maximus reconstruction.

Project description:Despite a paucity of physiological evidence, simplistic biomechanical analyses have led researchers to assume that humans who have wider hips use more energy to walk. Pitting biomechanical first principles against physiological data has led to little deepening of our understanding of bipedalism and its evolution. Both approaches, however, use proxies for the energy used by muscles. We decided to approach the question directly. Using a musculoskeletal model of the human body that estimates the metabolic energy expenditure of muscle activation for 48 people (23 women), 752 trials were evaluated. Metabolic energy consumption for the abductor muscles was summed over a stride to create total abductor energy expenditure. We calculated the maximum hip joint moment acting in the coronal plane and the functional distance between the hip joint centers. We hypothesize that wider hips would be correlated with both maximum coronal plane hip moment and increased total abductor energy expenditure when mass and velocity were controlled. Linear regressions with multiple independent variables, clustered by participant to control for the non-independence of the data points, were performed in Stata. We found that hip width does not predict total abductor energy expenditure, although mass and velocity combine to predict 61% of the variation (both p<0.001). Maximum hip joint coronal plane moment is predicted by pelvic width (p<0.001) and, in combination with mass and velocity (both p<0.001), explains 79% of the variation. Our results indicate that people use their morphology in ways that limit differences in energy expenditure. Consistent with recent discussion, intraspecific variation might not be useful to understand differences among species.

Project description:The patient, a 20-year-old nulligravida, presented with a progressive swelling on the right side of her vulva over the past three years. She also reported dull, aching pain, particularly during long walks. Upon examination, the patient appeared comfortable, and her vital signs were within normal limits. Genitourinary examination revealed a right vulvar mass, 12 cm by 7 cm, which was movable, soft in consistency, and non-tender. Subsequent inguinovulvar ultrasonography confirmed the presence of a compressible soft-tissue mass measuring 10 × 7 × 4 cm, consistent with a diagnosis of lipoma. Surgical excision resulted in a significant improvement in the patient's condition and she was discharged from hospital.
