A new technique for the treatment of midcarpal instability.

ABSTRACT: Extensor carpi radialis brevis (ECRB) transfer to the hamate bone is a novel procedure that offers a new surgical treatment option for select patients with midcarpal instability (MCI) who do not respond to conservative treatment. We present our early experience with this procedure. In total, 12 patients (13 wrists) were reviewed with a follow-up duration ranging from 1 to 6 years. Complications and changes in active range of motion (AROM) were noted. Patient related outcome was measured using the Patient Rated Wrist/Hand Evaluation (PRWHE) questionnaire, amongst others. Two patients failed to show a positive response to the surgery. ECRB transfer to the ulnar side of the wrist is a new procedure that offers a potential option for the surgical treatment of MCI in select patients; however, further biomechanical and clinical studies are required. The level of evidence for this study is IV (therapeutic).

Arthroscopic thermal capsular shrinkage for palmar midcarpal instability.

Project description:Background Arthroscopic capsular shrinkage has been previously used to stabilize major joints. This is the first series of its use in the wrist for palmar midcarpal instability (PMCI). Materials and Methods This is a medium-term retrospective review of 13 patients (15 wrists) at an average follow-up of 48 months postoperative. All patients were assessed with a functional questionnaire for instability and a Disabilities of the Arm, Shoulder, and Hand (DASH) score, as well as clinical examination. Description of Technique Arthroscopic capsular shrinkage was performed to the palmar and dorsal capsules of the radiocarpal and midcarpal joints using a bipolar thermal probe. All wrists were immobilized for 6 weeks post operation. Results 100% follow-up was achieved . All cases had an improvement in the frequency and severity of instability symptoms. The average DASH score was significantly reduced. There were no complications. The average loss of movement following the procedure was 15%. Conclusions The medium-term results show that wrist instability due to PMCI can be improved significantly by thermal capsular shrinkage with only a minimal amount of secondary stiffness.

Surgical Techniques for the Management of Midcarpal Instability.

Project description:Palmar midcarpal instability (PMCI) is an uncommon and poorly understood disorder. Its etiology is believed to be due to traumatic or congenital laxity of the ligaments (volar and dorsal) that stabilize the proximal row. This laxity results in hypermobility of the proximal carpal row and unphysiologic coupling of the midcarpal joint. Clinically, the condition is manifested by a painful clunk with ulnar and radial wrist deviation. The purpose of this article is to chronicle our personal experience with this condition and to review our current treatment recommendations and outcomes.

Latarjet Technique for Treatment of Anterior Shoulder Instability With Glenoid Bone Loss.

Project description:Anterior glenohumeral instability is a common clinical entity, particularly among young athletic patient populations. Nonoperative management and arthroscopic treatment of glenohumeral instability have been associated with high rates of recurrence, particularly in the setting of glenohumeral osseous defects. Coracoid transfer, particularly the Latarjet procedure, has become the treatment of choice for recurrent anterior glenohumeral instability in the setting of osseous deficiencies greater than 20% to 30% of the glenoid surface area and may also be considered for the primary treatment of recurrent instability in the high-risk contact athlete, even in the setting of limited osseous deficiency. The following Technical Note provides a diagnostic approach for suspected glenohumeral instability, as well as a detailed description of the congruent-arc Latarjet procedure, performed with a deltoid split, with its postoperative management.

Arthroscopic Technique of Capsular Plication for the Treatment of Hip Instability.

Project description:Atraumatic instability or microinstability of the hip is a recognized cause of groin pain and hip instability. Risk factors include female sex, ligamentous laxity, and borderline dysplasia. Arthroscopically, the joint may distract easily, and there may be associated ligamentum teres tears and laxity of the capsule on manual probing. The use of arthroscopic capsular plication in this cohort of patients has shown good to excellent results. Biomechanically, a capsular plication aims to create an imbrication and inferior shift of the capsule to augment the screw-home mechanism of the capsuloligamentous structures and thereby improve stability in extension and external rotation. The purpose of this article is to detail the step-by-step surgical technique of arthroscopic capsular plication, in addition to the indications, pearls, and pitfalls of the technique.

Treatment of Proximal Trochlear Dysplasia in the Setting of Patellar Instability: An Arthroscopic Technique.

Project description:Patellar instability is a complex disorder with multiple etiologies, and treatment must be individualized to the unique pathoanatomy of each patient. Medial patellofemoral ligament reconstruction is one of the most commonly performed procedures for the treatment of patellar instability. Patients with a symptomatic supratrochlear spur, defined by the presence of a "jumping" J sign on examination, also may benefit from an adjunctive proximal trochlear resection. Here, we describe a technique for an arthroscopic proximal trochlear resection, or "bumpectomy," involving resection of the supratrochlear spur. In appropriately indicated patients, we have found this procedure to be a useful adjunct to medial patellofemoral ligament reconstruction without the need for concurrent trochlear sulcus deepening.

Arthroscopic Midcarpal Suture Anchor Repair of Dorsal Intercarpal Ligament Avulsion.

Project description:The dorsal intercarpal ligament (DICL) is crucial for carpal stability and is frequently associated with injuries to other carpal ligaments, notably the scapholunate and lunotriquetral interosseous ligaments. Although isolated DICL injuries are uncommon, they can manifest as ligament avulsions, bony avulsions, or attenuations from chronic injury. Surgical repair of isolated DICL tears may be necessary when conservative management fails. We have previously described the first isolated DICL avulsion from the scaphoid dorsal ridge, suggesting an arthroscopic repair via the radiocarpal joint. This article details a repair technique through the midcarpal joint.

Arthroscopic Technique for Isolated Posterolateral Rotational Instability of the Knee.

Project description:Isolated posterolateral corner injury is an uncommon injury that could be a source of unexplained knee pain and dysfunction. Most of these patients present instability caused by concomitant ligament injuries. Numerous studies have shown open approach posterolateral repair or reconstruction surgeries to restore posterolateral rotational stability. Still, there is a certain population of patients who present posterolateral rotation instability without significant injury to the fibular collateral ligament, popliteal fibular ligament, and popliteus tendon. The posterolateral capsular ligament is critical to provide posterolateral rotation stability. In this Technical Note, with video, we show a completely arthroscopic approach to stabilize the posterolateral corner, by stabilizing the posterior lateral joint capsule with the lateral meniscus attached to the rim of the lateral tibial plateau.
