Left pleural effusion caused by pancreaticopleural fistula with a pancreatic pseudocyst.

ABSTRACT: Pancreaticopleural fistula is an uncommon complication of chronic pancreatitis. The authors described a case of a man with medical history of alcohol-related chronic pancreatitis, presented with dyspnoea. The roentgenogram showed a massive left pleural effusion. Additional work-up revealed a pancreaticopleural fistula and amylase-rich pleural effusion. His respiratory state improved after the insertion of chest drainage tube. During his admission, conservative and endoscopic therapy was required for the treatment of his complication of mediastinal abscess and arterial aneurysm in the pancreatic pseudocyst.

Outcome of surgery for chronic pancreatitis related pancreatic ascites and pancreatic pleural effusion.

Project description:BackgroundAn internal pancreatic fistula involves an abnormality in the way that the pancreas communicates with organs and spaces within the body. This is usually due to a disrupted pancreatic duct or psuedocyst leakage (Ascitic or pleural fluid amylase level >1000 S units/dl and fluid protein level >3 g/dl). The study aims to report our experience with surgery for chronic pancreatitis-related pancreatic ascites and pancreatic pleural effusions.MethodsAll the patients, who underwent surgical intervention for pancreatic ascites and pancreatic pleural effusion between August 2007 and December 2020 in the Department of Surgical gastroenterology, Institute of Postgraduate Medical Education and Research, Kolkata, India were retrospectively reviewed.ResultsOf the total 14 patients, 10 (71.4%) were men with a median age of 40 (4-49) years. The median interval between onset of symptoms of CP and diagnosis of IPF was 27 (3-60) months. All patients had a history of chronic abdominal pain and 5 (35.7%) had a prior history of hospitalization for pain. Eleven patients (78.5%) presented with abdominal distension and 3 (21.4%) patients had respiratory distress. Six (42.8%) patients had undergone endotherapy before surgery. Contrast-enhanced computed tomography detected pancreatic pseudocyst in 10 (71.42%) patients. The most commonly performed operation was lateral pancreaticojejunostomy (n = 11, 78.5%). Seven postoperative complications developed in 4 (28.5%) patients. After a median follow-up of 60 (6-86) months, no patient developed recurrence of pancreatic ascites or pleural effusion.ConclusionIn the experienced hand, surgery can be performed with acceptable perioperative morbidity and mortality and long-term satisfactory outcomes.

High-throughput autoantibody analysis in malignant pleural effusion and tuberculosis pleural effusion.

Project description:BackgroundMalignant pleural effusion (MPE) and tuberculosis pleural effusion (TPE) are 2 kinds of common pleural diseases. Finding efficient and accurate biomarkers to distinguish the 2 is of benefit to basic and clinical research. In the present study, we carried out the first high-throughput autoantibody chip to screen the beneficial biomarker with samples of MPE and TPE and the corresponding serum.MethodsWe collected pleural effusion and serum of patients with MPE (n = 10) and TPE (n = 10) who had been in Beijing Chao-Yang hospital from June 2013 to August 2014. Using RayBio Human Protein Array-G2 to measure the concentration of 487 defined autoantibodies.ResultsFold changes of Bcl-2-like protein 11 (BIM) autoantibody in MPE-serum/TPE-serum and MPE/TPE groups were 10 (P = .019) and 6 (P = .001); for decorin autoantibody, MPE-serum/TPE-serum ratio was 0.6 (P = .029), and MPE/TPE ratio was 0.3 (P < .001).ConclusionBIM autoantibody is a promising MPE biomarker by high-throughput autoantibody analysis in MPE and TPE.

Pleural Effusion Caused by Mycolicibacterium mageritense in an Immunocompetent Host: A Case Report.

Project description:Mycolicibacterium mageritense (M. mageritense) is a rare species among rapidly growing mycobacteria, and M. mageritense pleurisy is very rare. Here, we report for the first time, an immunocompetent patient with pleurisy caused by M. mageritense. The patient had no history of immunodeficiency and no recurrence of lung cancer after surgery. However, 8 months after surgery, he developed a new lung shadow and pleurisy. Although whole-genome analysis of the colony cultured from the patient's pleural fluid revealed M. mageritense, we could not identify it in time, resulting in a poor outcome. M. mageritense pleurisy in this case might have occurred via a bulla rupture of the lung lesion because computed tomography of the patient's chest showed pneumothorax and a lung lesion in contact with thoracic cavity. This case emphasized that nontuberculous mycobacterial pleurisy should be considered in the differential diagnoses of pleural effusion even in immunocompetent patients. Advancement of comprehensive and rapid analyses of genomic data from clinical specimens will lead to better treatment strategies.

Project description:We screened pleural effusion proteomes of mesothelioma and lung adenocarcinoma patients to identify novel soluble mesothelioma biomarkers. We performed quantitative mass spectrometry-(MS-) based proteomics using isobaric tags for relative and absolute quantification (iTRAQ) and used narrow range immobilized pH gradient/high resolution isoelectric focusing (IPG/HiRIEF; pH 4 to 4.25) prior to analysis by nano liquid chromatography-coupled MS/MS. Pleural effusions from patients with malignant mesothelioma (n=6), lung adenocarcinoma (n=6), or benign mesotheliosis (n=7) were analyzed, and more than 1,300 proteins were identified.

Pancreatic-pleural fistula from tail pseudocyst in a patient with pancreatic head cancer: a case report.

Project description:A 76-year-old man was diagnosed with resectable pancreatic ductal adenocarcinoma (PDAC) of the pancreatic head. Concurrently, the patient had an approximately 2-cm cystic mass originating from the pancreatic tail. After preoperative chemotherapy for the resectable PDAC, the patient is presented with dyspnea and lower left thoracic pain. Chest X-ray revealed massive left pleural effusion, and laboratory analysis of the pleural fluid showed a very high amylase level. Computed tomography confirmed a fistula directly connecting the pancreatic tail pseudocyst to the left diaphragm. These findings suggested pancreatic-pleural fistula (PPF) from the pancreatic tail to the left pleura. Medical treatments of thoracic drainage, endoscopic pancreatic ductal drainage, and antibiotics were unsuccessful; therefore, a distal pancreatectomy, fistula closure, and thoracoscopic pleural decortication were performed before the pancreaticoduodenectomy for the PDAC. After surgery, the pleural effusion resolved and the symptoms were improved immediately. PPF is an uncommon complication in which pancreatic enzymes drain directly into the pleural cavity. Herein, we present a rare case of PPF after preoperative chemotherapy for PDAC with a review of the literature.Supplementary informationThe online version contains supplementary material available at 10.1007/s13691-022-00555-w.

Urinothorax causing massive left side pleural effusion in a young lady; case report and literature review.

Project description:Urinothorax is the presence of the urine in the pleural space. This condition is very rare and occurs due to unrelieved obstruction of urinary flow. A 20-year-old female presented 7 days after cesarean section with tachypnea, and generalized abdominal pain. There was absent air entry over the left hemithorax. CT scan showed massive left pleural effusion and a stone obstructing the renal pelvis with hydronephrosis and peri-renal collection. The pleural fluid had high fluid creatinine level suggesting urine collection. Ureteroscopy done and double J catheter inserted. The effusion became loculated, thoracotomy and pleural decortication done.
