Project description:Mediastinal germ cell tumors are some of the less frequently encountered anterior mediastinal masses. We report an interesting case of a 26-year-old male with a ruptured mediastinal cystic teratoma. Initial plain radiograph and CT scan of the chest showed radiographic evidence of a ruptured cystic teratoma, including a peripherally enhancing, partially calcified mass with internal fat density. Upon surgical excision, the mass was found to adhere to the thymus and anterior aortic arch. The patient was promptly diagnosed via imaging and managed in a timely manner via complete surgical resection.

Project description:Arteriovenous malformations (AVMs) are rare congenital vascular anomalies where rupture can be fatal. This report describes a case of a 69-year-old woman who presented with sudden severe lower back pain radiating to her left buttock and thigh and an inability to move her left leg. Computed tomography angiography (CTA) showed an extensive ruptured retroperitoneal AVM on the left paravertebral side with active bleeding and a large hematoma. Cardiopulmonary resuscitation with considerable blood transfusion was required. The AVM was effectively treated with coil embolization of the feeding lumbar arteries. Shortly after the intervention, the patient developed abdominal compartment syndrome. Urgent laparotomy with evacuation of the retroperitoneal hematoma was required. The patient was discharged 16 days after admission in a good physical condition and has been free of symptoms for more than a year after intervention. However, follow-up CTA showed residual contrast enhancement in the AVM and further surveillance is required.

Project description:IntroductionOvarian teratomas are most common germ cell neoplasms. Immature ovarian teratoma comprises less than 1% of all ovarian teratomas. It usually occurs in first two decades of life.Case presentationWe report a case of 4 years old female child presenting with pain and huge lump in lower abdomen. On abdominal ultrasonography, it revealed a solid-cystic pelvic lesion arising from left ovary. Magnetic resonance imaging (MRI) corroborated the ultrasonographic findings. She underwent laparotomy with right oophorectomy with excision of the mass. The histopathological examination of the excised mass confirmed it to be immature ovarian teratoma with yolk sac tumor. The patient had an uneventful recovery with no sign of tumor recurrence at a one and a half year follow-up.ConclusionIn spite of immature ovarian teratomas having aggressive behaviour and lethal outcome, a high degree of suspicion and timely management can translate into a very good eventual prognosis.

Project description:Background Giant mediastinal tumors in the pediatric population can pose unique challenges for resection such as cardiovascular collapse on induction of anesthesia and injury to surrounding structures that may be compressed, displaced, or invaded by the mass. Principles that must be borne in mind during removal of giant mediastinal masses include: appropriate cross-sectional imaging to define extent of mass; airway control during induction of anesthesia; a multidisciplinary collaborative approach including cardiothoracic surgery; preparation for urgent sternotomy; plan for peripheral cannulation to institute cardiopulmonary bypass if needed; preservation of neurovasculature structures during dissection; complete resection whenever possible. While complete resection is desirable and results in an excellent prognosis, it may not be achievable especially if the tumor encases coronary arteries, and it is acceptable to leave small amounts of tumor behind. Case Description Here we present a case describing surgical management of a giant mediastinal teratoma in a two-month-old female. The patient was found to have a large mediastinal mass during workup for cough and noisy breathing. She underwent preoperative echocardiogram demonstrating normal cardiac function followed by uncomplicated, open resection of the mass. Conclusions Giant mediastinal tumors give rise to unique challenges for resection in small infants. The principles of airway control, preparation for urgent sternotomy, preparation for peripheral cardiopulmonary bypass cannulation, and preservation of neurovasculature during dissection must be borne in mind.

Project description:A 26-year old female had an incidental finding of a cystic mediastinal mass (10.8 × 9.4 × 10.0 cm) in the inferior-anterior mediastinum. It had compressed the superior vena cava, right atrium and right ventricle, and additional imaging studies could not exclude right heart involvement. She underwent exploration via right thoracoscopy (video-assisted thoracoscopy), and the right groin vessels were isolated for bypass, if needed. Examination of the mass revealed its extrapericardial origin from the thymus. Complete resection was achieved with minimally invasive techniques through a 2.5-cm incision using a three-port approach. She was discharged on postoperative day one. The interdisciplinary planning and cooperation in this case avoided thoracotomy or sternotomy and allowed for a safe and complete minimally invasive resection.

Project description:BackgroundFat necrosis is a common sequelae of breast trauma. Its many presentations have been described in the literature. Oil cyst is one of the less common presentations of fat necrosis. Giant oil cyst is rare and may take many years to mature in an unstable environment of the women's breast. To our best knowledge, rupture of a giant oil cyst has never been reported. Yet, it should be included on the list of differential diagnoses along with hematoma, infection, or neoplasm. When a patient presents with an expanding breast lump, clinicians should be aware of the potential for oil cyst rupture and its features.Case descriptionA 51-year-old woman presented to a breast clinic with concern of an expanding painless left breast lump for the last 4 months. The lump first appeared 12 years ago shortly after a car accident and was stable for many years. The initial left breast ultrasound was inconclusive, but diagnostic mammogram revealed a giant ruptured calcified oil cyst. Due to the availability of prior diagnostic images, we were able to retrospectively follow the unique sequelae of fat necrosis from the development of giant oil cyst shortly after the breast trauma and up to the final outcome of ruptured calcified oil cyst.ConclusionsTrauma to a woman's breast can lead to the formation of a large oil cyst which can remain present for years, peripherally calcify, and is susceptible to rupture. Clinicians should be aware of this potential complication when presented with the case of expanding breast lump. Diagnostic mammogram is a study of choice and demonstrates benign pathognomonic features of an oil cyst.

Project description:BackgroundMediastinal teratomas occasionally rupture into the thoracic cavity, which induces mediastinitis or various other severe complications. Surgical treatment is crucial for ruptured teratomas; however, few literature reviews to date have addressed the characteristics of ruptured mediastinal teratomas.Case presentationWe report a 29-year-old woman with severe mediastinitis owing to a mediastinal mature teratoma that ruptured into the mediastinum and right pleural cavity. Surgical resection by median sternotomy was performed within 24 hours after emergency admission. Intraoperative findings demonstrated the ruptured wall of the tumor with exposure of its white contents, which appeared similar to skin and fat, and necrotic tissue in the anterior mediastinum. The tumor was adhered to the right upper lobe, the ascending aorta, and pericardium. Owing to the severe adhesion of the tumor caused by inflammation in the surrounding tissues, a small portion of the tumor could not be removed, and hence complete resection with a sufficient surgical margin was not achieved. Pathologically, the tumor consisted of a solid mass and a cystic mass with severe adhesion to the resected portion of the lung, which included skin and lipid tissue. The tumor was concluded to be a mature teratoma as neither an immature component nor malignant transformation was observed. The patient had an uneventful postoperative course.ConclusionsTo our knowledge, this is the report of successful surgical resection of a ruptured mediastinal teratoma causing severe mediastinitis, with the first literature review of ruptured mediastinal teratomas. We also discuss relevant findings from reports in the literature.

Project description:Bilateral ophthalmic-ethmoidal dural arteriovenous fistulas (DAVFs) are extremely rare and may present as complex lesions. These DAVFs are associated with high risk of intracranial hemorrhage but can be presented with ocular symptoms, cranial nerve palsy or epistaxis. Endovascular approaches have been used to manage an increasing proportion of complex intracranial DAVFs safely and with good clinical results. We present a patient with subdural hematoma and severe epistaxis due to ruptured bilateral ophthalmic-ethmoidal DAVF that was successfully treated by transarterial embolization with precipitating hydrophobic injectable liquid.
