Neuro-oncology: a selected review of ASCO 2012 abstracts.

ABSTRACT: The American Society of Clinical Oncology (ASCO), the largest clinical oncology meeting in the USA, meets annually and reproducibly provides an exciting forum to present new cancer clinical trials and research data. The ASCO 2012 CNS tumors section comprised 3 days of presentations and over 130 abstracts, providing an overview of neuro-oncology, including both metastatic diseases of the CNS and primary brain tumors. This brief review attempts to highlight select abstracts presented at this year's meeting in an organized manner that will provide a portrait of a large and multifaceted meeting.

The CNS tumors section of the American Society of Clinical Oncology 2016 covered various aspects of neuro-oncology including metastatic CNS diseases and primary brain tumors, presented via posters, oral talks and over 100 abstracts. Project description:Introduction and objectivesThe management of kidney stone disease has changed dramatically over the past 30 years. In particular, ureteroscopy (URS) has become a more efficacious procedure with less morbidity. As a result, based on physician surveys and reports from single centre series the rate of URS appears to have increased over time. However, large population-based evaluations to assess the changes over time in the surgical treatment of kidney stone disease have not been conducted. Our objective was to evaluate population-based trends in the use of extracorporeal shockwave lithotripsy (SWL), URS and percutaneous nephrolithotomy (PCNL) over the past 20 years, in Ontario.MethodsUsing the Ontario Health Insurance Plan physician claims database we conducted a population-based cross-section time series analysis by identifying all kidney stone treatments performed between July 1, 1991 and Dec. 31, 2010, in the province of Ontario. The primary endpoint was the proportion of all stone treatments represented by each modality, which was calculated for every 3-month block over the study period. Exponential smoothing models were utilized to assess for trends over time in the percent utilization of each of SWL, URS and PCNL.ResultsWe identified 194,781 kidney stone treatments between July 1, 1991 and Dec. 31, 2010. A total of 96,807 SWL treatments, 83,923 URS treatments and 14,051 PCNL treatments were performed. We observed a significant trend over time for decreased utilization of SWL (68.5% to 33.7%, p<0.0001) and an increase in URS utilization (24.6% to 59.5%, p=0.0002), while no change over time was found for PCNL (6.88% to 6.85%, p=0.97) (Fig. 1). By the end of 2004, URS had become the most widely used procedure.ConclusionsOur population-based study confirms the increased use of URS over time suggested by physician survey and single centre retrospective series. Accordingly, the utilization of SWL has decreased in a reciprocal fashion.

Adolescent and young adult neuro-oncology: a comprehensive review.

Project description:Adolescent and young adult (AYA; ages 15-39) patients represent a population that experiences significant challenges in cancer care and research, exemplified by poorer clinical outcomes as well as unmet psychosocial and reproductive health needs. Despite central nervous system (CNS) tumors being one of the most common malignancies diagnosed in the age group, there is a clear paucity of AYA CNS tumor-specific publications, especially those related to the unique psychosocial and reproductive health needs of this population of patients. In this review, we examine various aspects of AYA oncological care including tumor biology, clinical outcome, clinical trials enrollment rate, site of care, unique psychosocial needs, and oncofertility. We assess the current state of these issues, highlight areas of deficiencies, and outline the steps needed to address these concerns. We emphasize the importance of comprehensive molecular testing as part of the diagnostic work-up, expansion of clinical trial availability, access to psychosocial care and oncofertility expertise, and the development of AYA-specific clinical research to define best practices and advancing care for this population.

ACE-Neuro: A tailored exercise oncology program for neuro-oncology patients - Study protocol.

Project description:BackgroundPatients with primary brain tumours (i.e., neuro-oncology patients) lack access to exercise oncology and wellness resources. The purpose of the Alberta Cancer Exercise - Neuro-Oncology (ACE-Neuro) study is to assess the feasibility of a tailored neuro-oncology exercise program for patients across Alberta, Canada. The primary outcome is to assess the feasibility of ACE-Neuro. The secondary outcome is to examine preliminary effectiveness of ACE-Neuro on patient-reported outcomes and functional fitness.MethodsNeuro-oncology patients with a malignant or benign primary brain tumour that are pre, on, or completed treatment, are >18 years, and able to consent in English are eligible to participate in the study. Following referral from the clinical team to cancer rehabilitation and the study team, participants are triaged to determine their appropriateness for ACE-Neuro and other cancer rehabilitation services (including physiatry, physiotherapy, occupational therapy, and exercise physiology). In ACE-Neuro, participants complete a tailored 12-week exercise program with pre-post assessments of patient-reported outcomes and functional fitness, and objective physical activity tracked across the 12-week program. ACE-Neuro includes individual and group-based exercise sessions, as well as health coaching.ConclusionWe are supporting ACE-Neuro implementation into clinical cancer care, with assessment of needs enabling a tailored exercise prescription.

Glioblastoma in adults: a Society for Neuro-Oncology (SNO) and European Society of Neuro-Oncology (EANO) consensus review on current management and future directions.

Project description:Glioblastomas are the most common form of malignant primary brain tumor and an important cause of morbidity and mortality. In recent years there have been important advances in understanding the molecular pathogenesis and biology of these tumors, but this has not translated into significantly improved outcomes for patients. In this consensus review from the Society for Neuro-Oncology (SNO) and the European Association of Neuro-Oncology (EANO), the current management of isocitrate dehydrogenase wildtype (IDHwt) glioblastomas will be discussed. In addition, novel therapies such as targeted molecular therapies, agents targeting DNA damage response and metabolism, immunotherapies, and viral therapies will be reviewed, as well as the current challenges and future directions for research.

Vaccination strategies for neuro-oncology.

Project description:Vaccination against cancer-associated antigens has long held the promise of inducting potent antitumor immunity, targeted cytotoxicity while sparing normal tissues, and long-lasting immunologic memory that can provide surveillance against tumor recurrence. Evaluation of vaccination strategies in preclinical brain tumor models has borne out the capacity for the immune system to effectively and safely eradicate established tumors within the central nervous system. Early phase clinical trials have established the feasibility, safety, and immunogenicity of several vaccine platforms, predominantly in patients with glioblastoma. Definitive demonstration of clinical benefit awaits further study, but initial results have been encouraging. With increased understanding of the stimulatory and regulatory pathways that govern immunologic responses and the enhanced capacity to identify novel antigenic targets using genomic interrogation of tumor cells, vaccination platforms for patients with malignant brain tumors are advancing with increasing personalized complexity and integration into combinatorial treatment paradigms.
