Ondansetron in Treating Patients With Advanced Cancer and Chronic Nausea and Vomiting Not Caused by Cancer Treatment

ABSTRACT: RATIONALE: Antiemetic drugs, such as ondansetron, may help to reduce or prevent nausea and vomiting in patients with advanced cancer. PURPOSE: This randomized phase III trial is studying how well ondansetron works compared to a placebo in treating patients with advanced cancer and chronic nausea and vomiting that is not caused by cancer therapy.

Acupuncture for Postoperative Nausea and Vomiting in Patients Undergoing Colorectal Surgery

Project description:Objectives: This study aims to assess the effectiveness, safety, and feasibility of intensive acupuncture treatments combined with standard antiemetic medication as compared with P6 acupoint stimulation combined with standard antiemetic medication or with standard antiemetic medication alone, to prevent postoperative nausea and vomiting (PONV) in patients undergoing laparoscopic colorectal surgery. Background: PONV is one of the prevalent discomforts in the early phase of recovery after surgery. Evidence suggests that the stimulation of the P6 acupuncture point can reduce the occurrence of PONV. What remains unclear is whether a higher dose of acupuncture produces more benefits compared with P6 stimulation alone or whether acupuncture combined with standard antiemetic medication yields better outcomes. This study aims to assess the effectiveness and safety of different acupuncture regimen for the prevention of PONV: high-dose acupuncture treatments combined with standard antiemetic medication, P6 acupuncture-point stimulation combined with standard antiemetic medication, or medication alone.

Aprepitant Without Steroid in Preventing Chemotherapy-induced Nausea and Vomiting in Patients With Colorectal Cancer

Project description:Addition of aprepitant, an NK1 receptor antagonist to a 5-HT3 receptor antagonist and dexamethasone regimen was shown to be effective for prevention of chemotherapy-induced nausea and vomiting (CINV) with moderately emetogenic chemotherapy (MEC). Little is known about the efficacy of aprepitant when used without dexamethasone. Dexamethasone is widely used to prevent both acute and delayed nausea and vomiting induced by chemotherapy. However, multi-period use of dexamethasone could be associated with side effect, such as hyperglycemia, dyspepsia and insomnia. This randomized phase III trial studies antiemetic therapy with aprepitant and tropisetron to see how well they work compared to dexamethasone plus tropisetron in preventing chemotherapy-induced nausea and vomiting in patients with colorectal cancer receiving FOLFOX(oxaliplatin, leuvovorin and 5-fluorouracil) chemotherapy.

Evaluation of the Incidence of Nausea and Vomiting in Patients With Colorectal Cancer Receiving Irinotecan-based Therapy

Project description:This is a study to determine how much nausea and or vomiting is caused by irinotecan-based chemotherapy in patients with colorectal cancer. Patients with colorectal cancer scheduled to receive their first cycle of an irinotecan-based chemotherapy regimen are eligible. Any chemotherapy agents administered in combination with irinotecan must have low-minimal potential to cause nausea and or vomiting. Examples of acceptable regimens would be irinotecan in combination with infusional fluorouracil and leucovorin (FOLFRI) with or without bevacizumab and irinotecan in combination with cetuximab. Patients who have received prior non-irinotecan-based chemotherapy are eligible providing they experienced no vomiting and no greater than mild nausea with their prior chemotherapy.

A study comparing the use of two drugs, Granisetron and Ondansetron, for the prevention of nausea and vomiting after surgical procedures under general anaesthesia with inhalational anaesthetic drugs

Project description:Intervention1: i.v. granisetron: Granisetron is a serotonin 5-HT3 receptor antagonist that is used as an antiemetic to treat nausea and vomiting after chemotherapy or radiotherapy. Its primary function is to inhibit the activity of the vagus nerve, which activates the vomiting centre in the medulla oblongata. It is on the WHOs List of Essential Medicines. Granisetron is a medication that is well-tolerated and has few side effects. The most generally reported side effects of its use are headache, dizziness, and constipation. There have been no significant drug interactions reported with this drugs use. Patients will receive 2 mg granisetron single dose iv bolus before extubation. Control Intervention1: ONDANSETRON: Studies have proved the benefit of intravenous granisetron in its adverse event profile and its long action when compared to the standard of care (intravenous ondansetron). But these studies are for certain specific subsets of patients undergoing certain surgical procedures. We plan to compare its safety and efficacy in a wide range of patients & disease profile undergoing surgery under general anaesthesia.Patients will receive 4mg ondansetron i.v. (SOC) i.v. single bolus dose before extubation. Primary outcome(s): To compare the efficacy & safety of granisetron i.v. vs standard of care ondansetron i.v. in patients undergoing surgery under general anaesthesia.Timepoint: First 24 hours, grouped as 0-1 hours,1-3 hours,3-6 hours,6-12 hours,12-24 hours post surgery Study Design: Randomized, Parallel Group Trial Method of generating randomization sequence:Other Method of allocation concealment:Case Record Numbers Blinding and masking:Participant Blinded

Area postrema cell types that mediate nausea-associated behaviors

Project description:Nausea, the unpleasant sensation of visceral malaise, remains a mysterious process. The area postrema is implicated in some nausea responses, and contains poorly understood brain-resident sensory neurons that are anatomically privileged to detect bloodborne signals. To investigate nausea mechanisms, we built an area postrema cell atlas through single-nucleus RNA sequencing, revealing a few neuron types. Using mouse genetic tools for cell-specific manipulation, we discovered excitatory neurons that induce nausea-related behaviors, with one neuron type mediating aversion imposed by multiple poisons. Nausea-associated responses were observed to agonists of identified area postrema receptors, and suppressed by targeted cell ablation and/or gene knockout. Anatomical mapping revealed a distributed network of long-range excitatory but not inhibitory projections with subtype-specific patterning. These studies reveal the basic organization of area postrema nausea circuitry, and provide a framework towards understanding and therapeutically controlling nausea.
