Ondansetron With/Out Dexamethasone to Prevent Vomiting in Patients Receiving Radiation to the Upper Abdomen

ABSTRACT: RATIONALE: Antiemetic drugs may help to reduce or prevent vomiting in patients treated with radiation therapy. It is not yet known if ondansetron is more effective with or without dexamethasone in preventing vomiting caused by radiation therapy. PURPOSE: This randomized phase III trial is comparing how well ondansetron works with or without dexamethasone in preventing vomiting in patients with cancer who are receiving radiation therapy to the upper abdomen.

Aprepitant Without Steroid in Preventing Chemotherapy-induced Nausea and Vomiting in Patients With Colorectal Cancer

Project description:Addition of aprepitant, an NK1 receptor antagonist to a 5-HT3 receptor antagonist and dexamethasone regimen was shown to be effective for prevention of chemotherapy-induced nausea and vomiting (CINV) with moderately emetogenic chemotherapy (MEC). Little is known about the efficacy of aprepitant when used without dexamethasone. Dexamethasone is widely used to prevent both acute and delayed nausea and vomiting induced by chemotherapy. However, multi-period use of dexamethasone could be associated with side effect, such as hyperglycemia, dyspepsia and insomnia. This randomized phase III trial studies antiemetic therapy with aprepitant and tropisetron to see how well they work compared to dexamethasone plus tropisetron in preventing chemotherapy-induced nausea and vomiting in patients with colorectal cancer receiving FOLFOX(oxaliplatin, leuvovorin and 5-fluorouracil) chemotherapy.

A study comparing the use of two drugs, Granisetron and Ondansetron, for the prevention of nausea and vomiting after surgical procedures under general anaesthesia with inhalational anaesthetic drugs

Project description:Intervention1: i.v. granisetron: Granisetron is a serotonin 5-HT3 receptor antagonist that is used as an antiemetic to treat nausea and vomiting after chemotherapy or radiotherapy. Its primary function is to inhibit the activity of the vagus nerve, which activates the vomiting centre in the medulla oblongata. It is on the WHOs List of Essential Medicines. Granisetron is a medication that is well-tolerated and has few side effects. The most generally reported side effects of its use are headache, dizziness, and constipation. There have been no significant drug interactions reported with this drugs use. Patients will receive 2 mg granisetron single dose iv bolus before extubation. Control Intervention1: ONDANSETRON: Studies have proved the benefit of intravenous granisetron in its adverse event profile and its long action when compared to the standard of care (intravenous ondansetron). But these studies are for certain specific subsets of patients undergoing certain surgical procedures. We plan to compare its safety and efficacy in a wide range of patients & disease profile undergoing surgery under general anaesthesia.Patients will receive 4mg ondansetron i.v. (SOC) i.v. single bolus dose before extubation. Cellular Damage in the Target and Out-of-Field Peripheral Organs during VMAT SBRT Prostate Radiotherapy: An In Vitro Phantom-Based Study

Project description:New developments show that patients with prostate cancer can benefit from radiotherapy deliv-ered with a hypo-fractionated regimen. The aim of our study was to investigate the effect of hy-po-fractionated stereotactic body radiation therapy (SBRT) of prostate cancer on out-of-field organs. We used a humanoid phantom to irradiate prostate cells in conditions similar to patient therapy, using SBRT planning. Our results show that radiation doses in the location of the intes-tine and lung resulted in significantly higher radiation doses than the further locations. We observed a high radiotoxic effect in the cells irradiated in the prostate, and a small increase in DNA damage and cell killing in the intestine location. Gene expression analysis revealed significant enrichment of the biological processes related to the radiation response in the prostate. In the lung and thyroid, the enrichment of several gene groups was revealed, however the processes were not clearly related to the response to radiation. Our study provides extensive data on out-of-field safety of prostate SBRT.
