ABSTRACT: This study is aimed to demonstrate equivalence between combinations of lower strength OXN PR tablets (OXN PR LST) and single higher strength OXN PR tablets (OXN PR HST) taken at the same overall daily dose.

Low and High Dose Progestins Elicit Distinct Breast Cancer Biology

Project description:An incomplete understanding of dose-dependent progesterone receptor (PR) biology has led to the suboptimal exploitation of PR as a therapeutic target. While hormone replacement therapies (HRT) containing low-doses of synthetic progestogens (progestins) have been associated with an increased risk of breast cancer, high-dose progestin therapies have beneficial therapeutic effects in some patients with breast cancer. Herein we report that PR subcellular localization, post-trans3lational modifications, stability, chromatin binding, responsive gene expression and downstream biology differ considerably as a function of progestin concentration. Dose-dependent transcriptional profiling revealed that different concentrations of progestin elicit classic sigmoidal and/or biphasic gene induction at distinct target gene sets. Notably, when compared to high doses of progestin, low-dose treatments lead to minimal nuclear translocation of PR while supporting robust induction of a subset of target genes, indicating that PR occupancy has a complex relationship to target gene induction. Interestingly, high but not low-dose progestin treatments lead to increased phosphorylation at Ser294 and is associated with higher receptor turnover. Furthermore, while low-dose treatments stimulate cell proliferation, high-dose treatments do not result in significant cell cycle progression to S phase. Similar biphasic and sigmoidal patterns of gene induction were observed in human breast tumor explants treated with various concentrations of progestin, underscoring the need to fine tune the dosage of PR-targeting therapies. Given the wide range of medical applications of PR target therapy, our discovery of distinct dose-dependent PR biology may have important clinical implications.

Genome-wide DNA methylation profiling of primary colorectal laterally spreading tumors identifies disease-specific epi-mutations on common pathways

Project description:Colorectal laterally spreading tumors (LSTs) are rare comparing to adenomas but are becoming more prevalent. Unlike many neoplasms in colons, LST grows to extremely large size, usually greater than 10mm in diameter while rarely invades deeply. Also, there is a low tendency for LSTs to become cancerous. The epigenetic landscape of LST has not been defined. In this study, we used a genome-wide methylation profiling technology, i.e. the Illumina Human Methylation 450K array, to query the main epigenetic difference between LST and para-cancerous samples. Our results suggest that LST displays significant decrease in DNA methylation, especially in some intergenic regions (IGR). By integration of public data for adenomas and colorectal cancers, we defined the commonality and specific epigenetic signatures for these three types of neoplasms in colon, in particular LST and adenomas which represent precancerous conditions. These three diseases shared little specific DNA methylation abnormalities. However, our pathway-level analysis revealed that certain pathways were common targets of epimutation. More interestingly, different diseases seem to employ distinct epigenetic insult to disturb these pathways. Between LST and adenoma, we found eight pathways were commonly targeted but the epi-mutation patterns were opposite between the two diseases. Comparison between precancerous conditions and invasive states revealed the key pathways governing the progression to malignancy, including CAMs and PI3K-Akt pathways.

CLARITY study: Expression data from RRMS patients treated with Oral Cladribine versus Placebo at week 96

Project description:The CLARITY trial (NCT00213135) was designed as a double-blind, placebo-controlled study to allow the best comparison of absolute efficacy and safety of oral cladribine in RRMS subjects. 1326 patients with relapsing MS were randomized (1:1:1) to receive cladribine tablets 3.5 mg/kg or 5.25 mg/kg bodyweight or placebo. Gene expression data in whole blood samples at 96 weeks were prepared according to standard Affymetrix protocols Gene expression data in whole blood samples at 96 weeks were available from patients randomized to placebo (n=57), cladribine tablets 3.5 mg/kg (n=62), and cladribine tablets 5.25 mg/kg (n=70).

Higher-order chromatin organization defines PR and PAX2 binding to regulate endometrial cancer cell gene expression (ATACseq)

Project description:Estrogen (E2) and Progesterone (Pg) via their specific receptors, ER and PR respectively, are major determinants in the development and progression of endometrial malignancies. We have studied how E2 and the synthetic progestin R5020 affect genomic function in Ishikawa endometrial cancer cells. Using ChIPseq in cells exposed to the corresponding hormones, we identified cell specific binding sites for ER (ERbs) and PR (PRbs), mostly binding to independent sites and both adjacent to PAXbs. Long-range interactions (HiC) showed enrichment of PRbs and PAXbs, which we call progestin control regions (PgCRs) inside TADs with differentially progestin-regulated genes. Effects of hormone treatments on gene expression were detected by RNAseq. PgCRs correlate with open chromatin independently of hormonal stimuli. In summary, endometrial response to progestins in differentiated endometrial tumor cells results in part from binding of PR to compartmentalized PgCRs in hormone-independent open chromatin, which include binding of partner transcription factors, in particular PAX2.
