Project description:<p>In order to explore opportunities for personalized and predictive health care, we collected serial clinical measurements, health surveys and multiomics profiles (genomics, proteomics, autoantibodies, metabolomics and gut microbiome) from 96 individuals. The participants underwent data-driven health coaching over a 16-month period with continuous digital monitoring of activity and sleep. Multiomics factor analysis resulted in an unsupervised, data-driven and integrated view of human health, revealing distinct and independent molecular factors linked to obesity, diabetes, liver function, cardiovascular disease, inflammation, immunity, exercise, diet and hormonal effects. The data revealed novel and previously uncovered associations between risk factors, molecular pathways, and quantitative lifestyle parameters. For example, ethinyl estradiol use had a distinct impact on metabolites, proteins and physiology. Multidimensional molecular and digital health signatures uncovered biological variability between people and quantitative effects of lifestyle changes, hence illustrating the value of the combined use of molecular and digital monitoring of human health.</p>

Project description:In the US, the majority of cancer samples analyzed are from white people, leading to biases in racial and ethnic treatment outcomes. Colorectal cancer (CRC) incidence and mortality rates are high in Alabama African Americans (AAs) and Oklahoma American Indians (AIs). We hypothesized that differences between racial groups may partially explain these disparities. Thus, we compared transcriptomic profiles of CRCs of Alabama AAs, Oklahoma AIs, and white people from both states. Compared to CRCs of white people, CRCs of AAs showed (a) higher expression of cytokines and vesicle trafficking toward modulated antitumor-immune activity, and (b) lower expression of the ID1/BMP/SMAD axis, IL22RA1, APOBEC3, and Mucins; and AIs had (c) higher expression of PTGS2/COX2 (an NSAID target/pro-oncogenic inflammation) and splicing regulators, and (d) lower tumor suppressor activities (e.g., TOB2, PCGF2, BAP1). Therefore, targeting strategies designed for white CRC patients may be less effective for AAs/AIs. These findings illustrate needs to develop optimized interventions to overcome racial CRC disparities.

Project description:The aim of the present study was to evaluate miRNAs as response predictors in FFPE head and neck samples from a phase II clinical trial designed to evaluate the feasibility of delivering cisplatin concurrent with radiotherapy after an induction chemotherapy (IC) regimen based on the combination of cisplatin plus paclitaxel in locally advanced head and neck squamous cell carcinoma (HNSCC) patients. For this purpose, we assessed the global miRNA expression profile of 15 HNSCC patients undergoing treatment, in order to identify miRNAs able to segregate resistant tumors from the sensitive ones, thus serving as markers to predict response. The results showed four miRNAs (miR-21, miR-494, miR-720 and miR-923) that were overexpressed in HNSCC FFPE samples.

Project description:We attempted to screen and identify new plasma EV delivered microRNA can simultaneously diagnose AA, early CRC and advanced CRC in healthy people. We employed microRNA deep sequencing assay for the primary screen and performed TaqMan microRNA assay for further test and validation. Through stepwise screen and validation in two independent cohorts (cohort I: n=75, cohort II: n=104), we demonstrated that plasma EV-delivered miR-185-5p is a potential indicator of AA and CRC in Chinese patients. In the supplementary phase, we also preliminarily proved that miR-185-5p can promote the development of CRC in vivo and in vitro.

Project description:Weight loss and physical activity are the cornerstones of therapy for type 2 diabetes. However, providing an effective lifestyle intervention is difficult because of limited availability of reliable programs, patient inconvenience, and cost. A worksite setting provides a unique opportunity for lifestyle therapy because it reduces these barriers. We conducted an 8-month randomized controlled trial in persons with obesity and diabetes to determine the therapeutic effects and potential mechanisms of intensive-lifestyle-therapy (energy restriction and supervised exercise training) conducted at the worksite. Intensive-lifestyle-therapy resulted in marked (17%) weight loss, associated with beneficial changes in body composition, cardiorespiratory fitness, muscle strength, glycemic control, β-cell function and insulin sensitivity in the liver, adipose tissue, and skeletal muscle, despite a decrease in diabetes medication use. These beneficial effects were associated with changes in skeletal muscle (increased metabolite content and expression of genes involved in NAD biosynthesis, sirtuin signaling, and mitochondrial biogenesis and function), adipose tissue (decreased expression of genes involved in extracellular matrix remodeling), and a major plasma mediator of insulin resistance (decreased plasma PAI-1). These findings demonstrate that effective intensive-lifestyle-therapy can be implemented at the worksite, and has profound therapeutic, clinical, physiological, and cellular effects in people with obesity and type 2 diabetes.

Patients and Methods: One hundred patients with unresectable and advanced or recurrent CRC who underwent the surgical resection from 1998 to 2010 were enrolled in this study. RNA extracted from FFPE samples was subjected to gene expression microarray. After comprehensive gene expression analysis, CRC were classified by an unsupervised hierarchical clustering and a principle component analysis (PCA). Mutation analysis of KRAS, BRAF, PIK3CA and TP53 genes were performed by direct DNA sequencing. Correlation between the biological information, clinicopathological factors and clinical outcomes were analyzed.

Project description:Background and study aims
Chemotherapy is a common treatment for people with cancer. It can be associated with a number of side effects and symptoms, which if not adequately managed can have a negative impact on the lives of people living with cancer. Chemotherapy is often delivered on an outpatient basis and most people have to manage the side effects of their treatment at home, with limited input from health professionals. Therefore, it is important to look at ways of supporting people receiving chemotherapy to manage their symptoms and therefore improve their quality of life during periods when they are at home.
Researchers are conducting a large trial in 240 people to evaluate the impact of the use of the Vinehealth® app to help manage symptoms and side effects and hopefully improve the quality of life of people with breast, bowel or lung cancer. This is for patients who have just had surgery for their cancer and are about to start chemotherapy as part of their treatment for their cancer. Participation will last for 24 weeks.
The main aim of the trial is to see if using the Vinehealth® app in addition to usual care is better or worse than usual care on its own in the reporting and management of chemotherapy-related symptoms to help improve quality of life in the 24 weeks from the start of chemotherapy treatment.
Who can participate?
Patients aged 18 years or over who have had surgery for their cancer and are about to begin at least 12 weeks of chemotherapy treatment for their cancer
What does the study involve?
Once eligibility is confirmed patients will be randomly allocated into two groups using an online randomisation platform. One group will use the Vinehealth® app to log symptoms, complete questionnaires and find information for self-management, whilst the other group will complete questionnaires on paper and receive care that is usually provided at their hospital. Patients will participate in the trial for 24 weeks during their chemotherapy treatment period.
The groups that patients will be allocated into will be selected by a computer, which has no information about the individual, so people will be selected by chance. Patients in both groups will have their quality of life compared to see if one is better than the other. Other information, such as symptoms, other outcomes, supportive care needs, work limitations, confidence in their ability to complete tasks, and anxiety, will also be compared across both groups.

Project description:MicroRNAs (miRNA) are a class of small regulatory RNAs that mediate post-transcriptional silencing of specific target mRNAs. Data suggest the importance of miRNAs to cancer development and possibly to survival. Our overall hypothesis is that miRNA expression is unique to tumor molecular phenotype; that miRNA expression levels at time of diagnosis predicts survival; and that miRNA expression is associated with inflammation-related genetic and lifestyle factors key to colorectal cancer (CRC). This study takes a two pronged approach to addressing our hypotheses. While we propose to validate previously identified miRNAs that have been identified as associated with CRC (either by differential expression or from assessment of mutations), we will add to the field through discovery of new and important associations that may be unique to specific molecular phenotypes, to polyp to cancer progression, and to survival. We have analyzed the expression of 2006 human miRNAs using data derived from tumor and paired normal tissue at time of diagnosis from: 1975 people with incident colon cancer or rectal cancer and 290 polyps from colon and rectal cases (included in this study) who reported a prior polyp. MiRNA was obtained from dissected paraffin-embedded tissue and assessed using an Agilent microarray platform. We intend to extend our validation of previously identified mutated miRNAs and differentially expressed miRNAs to determine if these alterations are associated with specific tumor molecular phenotype (TP53, KRAS2, CIMP+, and for colon tumors MSI+), inflammation-related factors, clinical factors and survival. We will identify associations with miRNAs that are related to specific molecular phenotypes, with drivers in the carcinogenic process, and with clinical features and survival. These miRNAs will be validated using targeted Agilent Platform. Associations will be tested based on differential expression for both individual and groups of miRNAs using recent extensions of several statistical methods including ANOVA, logistic regression, and Cox proportional hazards models. Our sample size allows for both a training and validation component, and provides sufficient statistical power to meet the study goals. MiRNAs that are differentially expressed in polyps and in subsequent tumors will provide new insights into targets for screening and treatment as well as provide support that miRNAs function as the “driver” in the carcinogenic process. Testing of mutated miRNAs identified from sequencing in conjunction with tumor phenotype, clinical, and survival data will further validate the importance of these miRNAs, and provide insight as to which CRC molecular pathway the miRNAs function. Our rich dataset of lifestyle, genetic, clinical and prognosis, and tumor molecular phenotype on 1975 CRC and paired normal tissue allows us to examine factors that are associated with miRNA expression and mutation in a large sample of population-based cases in the most cost-efficient manner possible. The miRNAs identified in these analyses will elucidate pathways important in the etiology of CRC and will provide insight into potential targets for screening and treatment. MiRNAs are small, non-protein-coding RNA molecules that regulate gene expression either by post-transcriptionally suppressing mRNA translation or by mRNA degradation. We examine differentially expressed miRNAs in colorectal carcinomas, adenomas, and normal colonic mucosa. Data come from population-based studies of colorectal cancer conducted in Utah and the Kaiser Permanente Medical Care Program. A total of 1893 carcinoma/normal paired samples and 290 adenoma tissue samples were run on the Agilent Human miRNA Microarray V19.0 which contained 2006 miRNAs. We tested for significant differences in miRNA expression between paired carcinoma/adenoma/normal colonic tissue samples. Fewer than 600 miRNAs were expressed in >80% of people for colonic tissue; of these 86.5% were statistically differentially expressed between carcinoma and normal colonic mucosa using a False Discovery Rate of 0.05. Roughly half of these differentially expressed miRNAs showed a progression in levels of expression from normal to adenoma to carcinoma tissue. Other miRNAs appeared to be altered at the normal to adenoma stage, while others were only altered at the adenoma to carcinoma stage or only at the normal to carcinoma stage. Evaluation of the Agilent platform showed a high degree of repeatability (r=0.98) and reasonable agreement with the NanoString platform. Our data suggest that miRNAs are highly dysregulated in colorectal tissue among individuals with colorectal cancer; the pattern of disruption varies by miRNA as tissue progresses from normal to adenoma to carcinoma.
