THREE-row Circular STAPLER in Low Anterior Resection for Rectal Cancer

ABSTRACT: This trial aims to assess safety and efficacy of three-row circular staplers compared to two-row surgical stapllers in short-term and long-term perspective in patients with rectal cancer undergoing low anterior resection with stapled colorectal anastomosis. All the patients will undergo a low anterior resection. In a half of patients a colorectal anastomosis will be created with a three-row surgical circular stapler. In another half of patients a colorectal anastomosis will be created with a two-row surgical circular stapler.

Project description:The technique the investigators propose to perform colorectal and colo-anal anastomosis in patients underwent low and ultra-low anterior resection for rectal cancer could potentially reduce the anastomotic leakage rate by better trans-anal introduction of the circular stapler, elimination of the previous suture lines and dog ears, combined with direct inspection of the anastomosis, easy performance of trans-anal air leak tests and eventually direct repair of any small anastomotic defects. Another important point in cancer surgery is the easily identification of the distal margin. In fact, this technique is simple to perform, reproducible and safe in terms of complications.

Impact of a Digital Surgical Workflow and Digital Device Briefing Tool on Morbidity and Mortality in a Patient Population undergoing Primary Stapled Colorectal Anastomosis for Benign or Malignant Colorectal Disease: A Non-Interventional, Prospective Cohort Study

Project description:Interventions: Group 1: Patients whose treatment of their benign or malignant colorectal disease by left colorectal surgery with primary stapled colorectal anastomosis is supported by a digital surgical software system (digital surgical workflow on SPI platform) and additionally a new digital product briefing checklist called Digital Device Briefing Tool (DDBT). This product briefing checklist is a newly developed digital cognitive aid for primary stapled colorectal anastomosis called Digital Device Briefing Tool (DDBT). Group 2: Patients whose treatment of their benign or malignant colorectal disease is performed by left colorectal surgery with primary staple anastomosis without digitized surgical OR workflow. Group 3: Patients whose treatment of their benign or malignant colorectal disease by left-sided colorectal surgery with primary staple anastomosis is performed by supported by a digital surgical software system (digital surgical workflow on the SPI platform) without DDBT. Primary outcome(s): The primary endpoint is a composite outcome comprising the overall rate of surgical complications, including death, during hospitalization within the first 30 days after colorectal surgery with primary stapled colorectal anastomosis. For this study, a surgical complication is defined as any deviation from the normal postoperative course, including any undesirable clinical event that may be attributable to the surgical procedure or specifically to the circular stapler used to create the anastomosis and which might be expected in colon resection procedures up till discharge and up to 30-days ofter colorectal surgery. This includes all surgical procedure-related intraoperative and postoperative adverse events which might be expected in colon resection procedures. Project description:During amniote peripheral nervous system development, segmentation ensures the correct patterning of the spinal nerves relative to vertebral column. Along the antero-posterior (rostro-caudal) axis, each somite-derived half-sclerotome has intrinsic molecular properties that govern this segmentation process. Posterior half-sclerotomes express repellent molecules to restrict axon growth and neural crest migration to anterior half-sclerotomes. Here, we report an RNA-sequencing-based molecular characterization of anterior and posterior half-sclerotomes, using the chick embryo as a model organism.

We have previously demonstrated that chronically elevated fluid shear stress completely normalized blood flow in femoral artery ligated rabbits. This was realized by a surgical intervention: the creation of an arterio- venous anastomosis (shunt) between the femoral artery and the accompanying vein. The aim of this study was a gene profiling approach in order to identify differentially expressed genes during collateral growth in the shunt model when compared to pre-existent collaterals (sham).

Virus impact on proteome of H. annosum isolates

Project description:Circular agar plugs with fresh mycelium of H. annosum isolates were inoculated onto the center of 9-cm Petri plates containing 2% MEA (malt extract agar) and incubated at 22°C in the dark. Presence of viruses in H. annosum isolates used for the experiment: –1W, isogenic isolates fully cured of viral infections by thermal treatment lasting a one week; –A2, –A4, –A5, –C1, monohyphal cultures created by hyphal tipping; 2052–, isogenic isolates, which have received different combinations of viruses by horizontal transmission from a donor strain denoted by D; –3M, dual cultures maintained for three months.
