Project description:Background and study aims
Bowel cancer screening uptake has traditionally been lower than the government would like it to be. As screening can spot early signs of cancer and even stop bowel cancer from occurring, the public health service would like to improve screening rates.
This study aims to test a set of new, evidence-based materials, informed by psychological theory, to increase the return of the stool-based bowel cancer screening kit the NHS uses, in the North East region of England.
Who can participate?
Adults age 60 – 74 years eligible for bowel cancer screening
What does the study involve?
Participants are randomly allocated to one of four groups. All participants are sent an initial invitation letter for bowel cancer screening and the standard NHS information booklet.
Those in the first group receive an intervention pack four days after the letter, with an information sheet and short paper based task encouraging them to connect barriers likely to be encountered with effective responses. They receive the bowel cancer screening test four days later.
Participants in the second group receive an intervention pack containing the information sheet and a motivational leaflet. They receive the screening kit in the post four days later.
Participants in the third group receive both of the above intervention packs in the post four days after the initial letter, before the screening kit four days later.
Participants in the last group receive no additional information, and get a screening kit in the post eight days after the initial letter.

Project description:This clinical trial compares audio brochures and video brochures to usual care for increasing colorectal cancer (CRC) screening among adults living in Appalachia. Adults living in rural Appalachia experience increased rates of CRC, possibly due to the unique barriers to screening this population faces, such as lower socioeconomic status, limited health literacy, and less access to healthcare. The fecal immunochemical test (FIT) is a common CRC screening technique which allows individuals to complete the test at home and mail the kit back. A limitation of the current process for FIT screening outreach is that the instruction materials are heavy in text and not appropriate for individuals with limited health literacy. The audio and video brochures used in this clinical trial are instruction materials that have been specially designed to be appropriate for individuals of all health literacy levels. Providing these tailored audio and video brochures with the FIT kits may increase CRC screening among adults living in Appalachia, compared to usual care.

Project description:Interventions: Group 1: Focus of the letter/writing addressed to the 50 year old group (two intervention groups of the birth year 1962) is testing for occult blood in the stool, which is paid by the health insurance from that age (and up to 55 years) annually. Beside the information about this offer and the invitation to its perception, the letter/writing should contain an understandable and clear information booklet about the test and its application and significance. In one of the two intervention groups, a stool test kit for carrying out the tests for blood in the stool should be additionally attached (test cards for each two samples from three consecutive bowel movements). It should be checked whether this service offer further increases participation rate in Germany, as shown in other countries. People in the intervention group 2 (cover letter with the stool test kit) will be asked to apply the test cards and send them to the DKFZ. There, the tests will be analysed and the result will be reported directly by mail to the persons and, if desired, additionally to the family doctor of whom the address is reported on a request form. In case of a positive test result, a meeting is recommended with the family doctor to discuss the possibility of a colonoscopic investigation. The test results are pseudonymised forwarded to the study center in the Saarland, where they can be clearly assigned to the investigation group 2 based on the communication numbers (which are noted on the materials sent to them). To check the benefits of repeated letters/writing of the same birth cohort, people are contacted again on the following 51th birthday (regardless of participation in the previous year). Primary aims of the pilot project are the detection and quantification of the increase in participation rate in colorectal cancer early detection through a logistic uncomplex personal invitation procedure. The issues include in particular:
- To what extent can the use of the test for occult blood in the stool be increased through a personal invitation letter with an information booklet?
- To what extend can the sending of a stool test kit together with the personal invitation letter further increase participation?
- To what extend is a colonoscopy performed in case of a positive test result?
- To what extend can the use of screening colonoscopy be increased through a personal invitation letter with an information booklet?
- What effects can be achieved in terms of participation at initial cover letter and follow-up letter in one year distance?
- For all questions: Are there differences between women and men?

Project description:Background and study aims
In England, the NHS Bowel Cancer Screening Programme (BCSP) offers screening to over 60 year olds. This involves completing a stool sample test at home. Recent data suggests that only 1 in 2 people who are offered the test, go on to complete it. However, this figure varies from more than 60% in the most socially advantaged groups to less than 35% in the most disadvantaged groups. The aim of this study is to reduce this difference in uptake rates between social groups. This study aims to test the effect of a leaflet by assessing its impact on participants intentions to use the test in a group of adults approaching the age of screening.
Who can participate?
Men and women aged between 45 to 59 and half years, who can read and write English.
What does the study involve?
The study involves sending all potential participants a study pack. The pack contains a covering letter from the participants GP practice, an 8-page questionnaire, a freepost return envelope, and an NHS envelope containing an example of the NHS BCSP invitation letter and a facts booklet about bowel cancer screening. For half of the participants, a narrative leaflet will also be included in the NHS envelope. Participants will be asked to read the contents of the NHS envelope before completing and returning the questionnaire in the freepost envelope to the research team.

Project description:Most studies have analysed the effects of high dose radiation such as atomic bomb survivors in Japan, people exposed during the Chernobyl nuclear accident, patients undergoing radiation therapy, uranium miners, etc. However, it has been difficult to measure and assess the risk of cancer in people exposed to lower doses of ionising radiation, such as the people living at high altitudes, who are exposed to more natural background radiation from cosmic rays than people at sea level. We measured the genomic response to X-ray ionising radiation (10 cGy and 100 cGy) in a skin tissue model to compare the effects of low and high dose ionising radiation at different time points. The microarray data was then analysed using state-of-the art “upside-down pyramid” computational systems biology methods to identify genes contributing to the difference in the response to the different radiation doses. The model is reconstructed skin tissue, which is composed of keratinocytes that make up the epidermal layer, and fibroblasts that make up the dermal layer of the skin. Tissues were irradiated with 0, 10, and 100 cGy X-ray radiation. Skin plugs were harvested at 0, 3, 8, and 24 hours post irradiation.

Project description:Background and study aims
In the UK, 42,000 people each year are diagnosed with bowel cancer. It is the fourth most common cancer. Surgery to remove the cancer is the best treatment. However, it has a risk of complications, which is doubled for people who are overweight/obese. Patients experiencing complications recover more slowly, stay in the hospital longer and need more care. This isn’t good for patients or the NHS. Physical fitness and well-controlled blood sugar are linked with fewer complications from surgery. For people who are overweight, weight loss improves both of these factors so that it may reduce complications. One reliable way to lose a meaningful amount of weight in the short period before surgery (3-4 weeks) is through a low-calorie diet programme: eating only special nutritious soups and shakes (880 calories/day) that have all the necessary vitamins. With weekly support from a dietitian, most people succeed. Typically, people lose 5% of their weight within 20 days. The NHS uses a version of this programme to treat type 2 diabetes. In small-scale studies, patients with cancer who are overweight have been willing and able to take part in less intensive weight management programmes before surgery but lose little weight. However, the period before bowel cancer surgery is associated with feelings of uncertainty and anxiety, so it is unclear if patients can follow a more intensive programme.
Who can participate?
To start to find out if this treatment is in the best interests of patients’ physical and mental health, we will recruit 72 overweight patients awaiting bowel cancer surgery
What does the study involve?
Half of the participants will be randomly allocated to continue with their usual care and half will be offered the weight loss programme. We will see whether enough patients are willing to take part, lose weight, and return for follow-up visits. We will monitor complications for 30 days after surgery and any reduction in muscle mass due to the weight loss. We will interview patients about their experiences.
This information will tell us if a full trial is worthwhile to test whether this programme can reduce complications from surgery, improve outcomes for people with bowel cancer, and if the financial costs are likely to be worth the benefits. It will also help us refine the treatment plans according to patient feedback.
The researchers discussed the study with seven patients and public representatives (PPI). They thought it was a critical study and the team included their suggestions for making it easier for patients to join the trial, stick to the programme, and attend visits. The research team will work with the PPI throughout the trial.
