A personal electronic health record (PEPA)

ABSTRACT: Background and study aims The Federal Ministry of Education and Research (BMBF) has funded the development of a patient controlled “Personal Electronic Health Record” (PEPA) as part of their current INFOPAT project. PEPA allows for the exchange of medical data between all interested groups, including the patients thenselves. A PEPA contains medical reports and findings including, for example, x-rays, CT scans and MRIs. The patients can safeguard their information and access all content. The aim of this study is to test the PEPA-prototype for cancer patients being treated at the National Center for Tumor Diseases (NCT) in Heidelberg. Who can participate? Adult colorectal cancer patients being treated at the National Center for Tumor Diseases (NCT) in Heidelberg. What does the study involve? There are two part to this study. The first part involves focus groups, in which a group of people are asked their opinion and attitudes towards, for example, an idea, product or service. Here, focus groups are set up that involve the patients, doctors and other health professionals to discuss current problems with the coordination of care offered to cancer patients, as well as discuss what people want to get from using PEPA (i.e. user requirements). The PEPA-prototype is then developed taking into account the conclusions from the focus groups. In the second part of the study, patients are encouraged to use the PEPA-prototype to prepare and follow up meetings with their physicians.

Project description:Expression levels of proteins and phosphoproteins, covering major cancer signaling pathways with a special focus on breast cancer biology, were obtained for a series of 164 breast cancer tumor specimens with positive estrogen receptor status. Tumor specimens from patients diagnosed with primary invasive breast carcinoma were collected at the time of surgery between 2005 and 2011 and provided by the NCT Tissue Bank Heidelberg as well as by the Department of Gynecology and Obstetrics / National Center for Tumor Diseases Heidelberg. None of the patients had received neoadjuvant therapy.Tumor specimens were processed within 20 min after surgery. Samples were stored snap frozen at -80M-BM-0C until further use. Only tumor samples with > 70% tumour cells and positive estrogen receptor status (immunoreactive score M-bM-^IM-% 3) as assessed by routine immunohistochemistry were selected for this study (n = 164). Tumor lysates were printed on a series of nitrocellulose coated glass slides and probed with with differnt primary antibodies directed against proteins and phosphoproteins of interest. Primary antibodies were selected to recognize proteins involved in major cancer signaling pathways with a special focus on breast cancer biology.

RNA profiling of matched pair samples of primary breast cancer vs. lung and pleura metastasis

Project description:Patient samples were recruited from a series of female patients with metastatic primary breast cancer (PBC) and biopsy-confirmed breast cancer lung or pleural metastasis (BCLPM) at the Department of Thoracic Surgery, Thoraxklinik, Univ. of Heidelberg, Germany. All patients were included who had received lung or pleural metastasis biopsies from 2003 to 2014 and had a prior history of PBC at the Department of Gynaecology and Obstetrics, Univ. Hospital. Formalin-fixed and paraffin-embedded tissue samples were provided by the Tissue Bank at the National Center for Tumor Diseases (NCT, Heidelberg, Germany) in accordance with the regulations of the Tissue Bank and approval of the Ethics Committee at the Medical Faculty of the Univ. of Heidelberg, approval no. S-716/2018.

A monocyte gene expression signature in the early clinical course of Parkinson’s disease

Project description:Recent studies suggest that the activation of the innate immune system is an integral part of Parkinson’s disease (PD) pathology. Monocytes have a critical role as effectors and regulators of the innate immune system, and a recent expression quantitative trait locus (eQTL) study links monocytes to PD. Patients diagnosed with PD according to the National Institute of Neurological Disorders and Stroke diagnostic criteria for PD were included. We applied a standardized procedure with blood drawing from fasting participants. PD patients with Hoehn and Yahr > 2 and/or cognitive impairment were not recruited. Participants with a history of cancer, infectious diseases within the past three months, or taking anti-inflammatory drugs were excluded. Altogether, we analyzed the monocytic transcriptome profiles of 10 PD patients and 9 age- and sex-matched controls using mRNA sequencing.

Transcription profiling on non-small cell lung cancer samples reveals CYP3A5 gene expression is associated with early recurrence

Project description:Study to identify genes associated with NSCLC recurrence in patients not receiving adjuvant chemotherapy. Purpose: Recent clinical trials suggest improvement in survival with adjuvant chemotherapy in non-small cell lung cancer (NSCLC). This study's aim is to identify genes associated with NSCLC recurrence in patients not receiving adjuvant chemotherapy. Experimental design: Banked NSCLC tumors of patients who underwent resection of stage Ia-IIIb disease were identified. Patients were stratified into two groups: recurrent (R, n=11) or non-recurrent (NR, n=16) 2 years after surgery. Microarray profiling and Cox multivariate analysis were performed. Conclusion: Increased CYP3A5 gene expression correlates with NSCLC recurrence and promotes proliferation through mechanisms that may involve, in part, CYP3A5 epoxygenase activity. Project description:The Genotype-Tissue Expression (GTEx) project is a collaborative effort that aims to identify correlations between genotype and tissue-specific gene expression levels that will help identify regions of the genome that influence whether and how much a gene is expressed. GTEx is funded through the Common Fund, and managed by the NIH Office of the Director in partnership with the National Human Genome Research Institute, National Institute of Mental Health, the National Cancer Institute, the National Center for Biotechnology Information at the National Library of Medicine, the National Heart, Lung and Blood Institute, the National Institute on Drug Abuse, and the National Institute of Neurological Diseases and Stroke, all part of NIH. This series of 837 samples represents multiple tissues collected from 102 GTEX donors and 1 control cell line. In total, 30 tissue sites are represented including Adipose, Artery, Heart, Lung, Whole Blood, Muscle, Skin, and 11 brain subregions. RNA-seq expression data, robust clinical data, pathological annotations, and genotypes are also available for these samples from dbGaP (http://www.ncbi.nlm.nih.gov/projects/gap/cgi-bin/study.cgi?study_id=phs000424.v2.p1) and the GTEx portal (www.broadinstitute.org/gtex). While GTEx is no longer generating Affymetrix expression data, donor enrollment continues and is expected to reach 1,000 by the end of 2015.
