Elevated Waste Anaesthetic Gas Concentration in the Paediatric Post-Anaesthesia Care Unit.

Project description:ObjectiveExposure to waste anaesthetic gas (WAG) is a recognised occupational hazard for health care professionals (HCP). In recovery rooms, scavenging and ventilation systems differ from those in the operating room, raising the question as to how efficient they are. This study aims to measure the levels of ambient sevoflurane over the course of consecutive workdays in the paediatric recovery room of a tertiary academic centre.MethodsThe following is a descriptive-analytic study of ambient air sevoflurane levels measured using a MIRAN® 205B Series SapphIRe portable ambient air analyser. Samples were obtained between 7:30 am and 6:30 pm for two non-consecutive weeks on consecutive weekdays in our paediatric recovery room area.ResultsThe ambient air levels of sevoflurane exceeded the ceiling concentration of 0.5 ppm recommended by the National Institute for Occupational Safety and Health on all days of measurement. The concentration of sevoflurane in ambient air correlates directly with the number of patients present.ConclusionEven in a modern recovery room constructed according to current building standard and code, ambient air levels of WAG exceed the recommendations. Future research and practice standards are needed to reduce this occupational exposure. Disregarding whether chronic exposure to WAG is harmful, we have shown that HCP working in recovery rooms are chronically exposed to concentrations which exceed recommended levels. Strategies are needed to reduce ambient levels of WAG in post-anaesthesia care units.

Expression data from pain model: rat L5 sensory ganglia after local inflammation

Project description:Inflammation plays a role in neuropathic pain conditions as well as in pain induced solely by an inflammatory stimulus. Robust mechanical hyperalgesia and allodynia can be induced by locally inflaming the L5 dorsal root ganglion (DRG) in rat. This model allows investigation of the contribution of inflammation per se to chronic pain conditions. Most previous microarray studies of DRG gene expression have investigated neuropathic pain models involving axon transection. To examine the role of inflammation, we used microarray methods to examine gene expression 3 days after local inflammation of the L5 DRG in rat. We observed significant regulation in a large number of genes (23% of observed transcripts), and examined 221 (3%) with a fold-change of 1.5-fold or more in more detail. Immune-related genes were the largest category in this group and included members of the complement system as well as several pro-inflammatory cytokines. However, these upregulated cytokines had no prior links to peripheral pain in the literature other than through microarray studies, though most had previously described roles in CNS (especially neuroinflammatory conditions) as well as in immune responses. The L5 dorsal root ganglion (DRG) was locally inflamed with zymosan/Incomplete Freund's Adjuvant. DRG were isolated 3 days later. Each sample was RNA extracted from a single DRG. 6 samples from rats with local DRG inflammation were compared with 6 samples from sham-operated rats.

Pain Relief After Colorectal Surgery: Spinal Combined With Painbuster vs Painbuster Alone.

Project description:Limiting surgical stress and managing postoperative pain are well understood to influence recovery and outcome from major surgery for colorectal cancer and both are fundamental aspects of enhanced recovery protocols. Traditional approaches for dealing with these problems such as epidural or patient controlled intravenous opioid analgesia are associated with problems that may be detrimental to postoperative recovery and surgical outcome. As a result there is evidence in the literature of increasing interest in alternative techniques such as intrathecal anaesthesia or continuous wound infusion of local anaesthetic, however nobody has examined the effect of combining the techniques or their impact on the surgical stress response. We intend to compare patients undergoing major resections for colorectal cancer receiving intrathecal anaesthesia in combination with a wound infusion of local anaesthetic with those receiving a continuous wound infusion alone. We will examine the surgical stress response and postoperative pain control in addition to objective measures of postoperative recovery. We suggest that our approach will attenuate the surgical stress response and provide optimal pain control that will ultimately translate in improved recovery and outcome following surgery for colorectal cancer.

Amplified electrokinetic response by concentration polarization near nanofluidic channel.

Project description:Ion concentration polarization is the fundamental transport phenomenon that occurs near ion-selective membranes, but this important membrane phenomenon has been poorly understood due to theoretical and experimental challenges. Here, we report the first direct measurements of detailed flow and electric potential profiles within and near the depletion region. This work is an important step toward a full characterization of this coupled transport problem. Using microfabricated electrodes integrated with the microfluidic device, we measured and confirmed that the electric field inside an ion depletion region is amplified more than 30-fold compared to outside of the depletion zone due to the highly nonuniform ion concentration distribution along the microchannel. As a result, the electrokinetic motion of both fluid (electroosmosis) and particle (electrophoresis) was significantly amplified. The detailed flow profile within the depletion zone was also measured for the first time by optically tracking photobleached neutral dye molecules. We further showed that the amplified electrokinetic flows generated in this device may be used as a field-controlled, microfluidic fluid pump and switch.

Using incubations with either lanthanum (La), neodymium (Nd), or a lanthanide cocktail, differential gene expression analysis revealed that up tp 41% of the encoded genes were differentially expressed.

Project description:Purpose: The goal of this study is to use chromatin accessibility data to study the information patterns of chromatin accessibility that encode TF-chromatin interactions. Methods: We generated chromatin accessibility data from the GM12878 lymphoblastoid cell line using a modified protocol where the ATAC-seq fragments are sonicated during the library preparation. The goal of the sonication step is to disrupt the observed fragment size distribution, therefore masking local TF-chromatin interactions encoded in the larger fragment sizes (e.g. nucleosome phasing) without affecting the levels of chromatin accessibility.

Concentration polarization and nonlinear electrokinetic flow near a nanofluidic channel.

Project description:A perm-selective nanochannel could initiate concentration polarization near the nanochannel, significantly decreasing (increasing) the ion concentration in the anodic (cathodic) end of the nanochannel. Such strong concentration polarization can be induced even at moderate buffer concentrations because of local ion depletion (therefore thicker local Debye layer) near the nanochannel. In addition, fast fluid vortices were generated at the anodic side of the nanochannel due to the nonequilibrium electro-osmotic flow (EOF), which was at least approximately 10x faster than predicted from any equilibrium EOF. This result corroborates the relation among induced EOF, concentration polarization, and limiting-current behavior.
