Comparison of Neurotoxic Potential of Abraxane versus Oxaliplatin and Paclitaxel

ABSTRACT: Interventions: Patients receiving abraxane chemotherapy for pancreatic cancer, breast cancer, colorectal cancer, non-small cell lung cancer or ovarian cancer will be recruited into the study to document the natural history of Abraxane-induced neurotoxicity through cross-sectional and prospective clinical and neurophysiological assessment. Examples of neurotoxicity seen with abraxane therapy include sensory dysasthesia including numbness and tingling in the hands and feet. Patients will be assessed monthly from the commencement of treatment until cessation, with 3-monthly follow up 6-12 months following cessation of treatment, to a maximum period of 2 years following enrolment. Primary outcome(s): To document the natural history of Abraxane-induced neurotoxicity through cross-sectional and prospective clinical and neurophysiological assessment. The severity of neuropathy will be assessed using the Total Neuropathy Score (TNS). Does Riluzole reduce the incidence of chemotherapy-induced nerve injury in patients with colorectal cancer?

Project description:Interventions: Patients receiving oxaliplatin chemotherapy for colorectal cancer will be randomised into either the study drug arm or placebo control arm. Participants will be started on either riluzole 50 mg oral tablet twice daily or the twice daily lactose placebo tablet. Following randomisation, therapy will be continued for the duration of oxaliplatin treatment (4-6 months) and for 2 weeks following treatment cessation. Primary outcome(s): To assess whether treatment with riluzole results in a reduction in the development of chronic neuropathy and neuropathic symptoms The severity of neuropathy will be assessed using the Total Neuropathy Score (TNS). Project description:Chemotherapy-induced peripheral neuropathy (CIPN) is a serious side effect of cancer treatment, often leading to cessation of therapy due to lack of adequate treatment options. In the peripheral nervous system the chemotherapeutic agent paclitaxel impairs mitochondrial function and induces vast changes in gene expression. However, changes at the protein level remain so far unexplored. Here we used an unbiased approach of quantitative proteome profiling in a clinically-relevant rat model of paclitaxel-induced peripheral neuropathy. We analysed two critical timepoints: (1) day 7, equivalent to 1 day after cessation of paclitaxel treatment, prior to neuropathy development; (2) approximately 4 weeks after paclitaxel initiation, when neuropathy has developed.

Signature of coevolution between determinants of defense and life span in Arabidopsis thaliana

Project description:The selective impact of pathogen epidemics on host defenses can be strong but remains transient. By contrast, life-history shifts can durably and continuously modify the balance between costs and benefits, which arbitrates the evolution of host defenses. Their impact, however, has seldom been documented. Here, we show with a simple mathematical model that the selective advantage of the defense system is expected to decrease with decreasing life span. We further document that, in natural populations of the model plant system Arabidopsis thaliana, the expression level of defense genes correlate positively with flowering time, a proxy for the length of vegetative life span. Using a genetic strategy to partition life span-dependent and –independent defense genes, we demonstrate that this positive co-variation is not explained by the pleiotropic action of major regulatory genes controlling both defense and life span. In agreement with our model, this study reveals that natural selection has likely assembled alleles promoting lower expression of defense genes with alleles decreasing the duration of vegetative life span in natural populations of A. thaliana. This is the first study demonstrating that life history evolution has a pervasive impact on the evolution of host immunity.

Abraxane, the Nanoparticle Formulation of Paclitaxel can Induce Drug Resistance by Up-regulation of P-gp

Project description:Abraxane, a nanoparticle (NP) formulation of paclitaxel (PTX), has been demonstrated to be more effective than Taxol, the small molecule formulation, for the treatment of breast cancer and non-small cell lung cancer (NSCLC). It was reported that Abraxane existed in plasma as particles with the size of ~10 nm. NPs get in and out of the cells by endocytosis and exocytosis, whereas small molecules by diffusion and efflux. It is intriguing to know whether the improved pharmaceutical performance is related to the “too-big-to-be-pumped-out” phenomenon. Here we established an Abraxane-resistant NSCLC cell line A549/Abr and compared its transcriptomes with that of the Abraxane-sensitive parental cell line by RNA-Seq technology. To our surprise, the most significantly up-regulated genes were ABC transporters, the common efflux pump for small molecules. We further found that the ABCB1 inhibitor Verapamil reversed the drug resistance and confirmed the important role of ABCB1 in Abraxane resistance.
