OXTOX: Can Oxaliplatin neurotoxicity be reduced with ibudilast in people with metastatic colorectal cancer – a phase II randomised study

ABSTRACT: Interventions: Ibudilast 30mg oral twice a day for prevention of acute neurotoxicity for the duration of taking oxaliplatin chemotherapy commencing 2 days prior to starting oxaliplatin. Primary outcome(s): Acute neurotoxicity assessed by the Oxaliplatin Acute Symptom Questionnaire[Day 3 of week 4 for CAPOX and day 3 of week 5 for FOLFOX.] Study Design: Purpose: Prevention; Allocation: Randomised controlled trial; Masking: Blinded (masking used);Assignment: Parallel;Type of endpoint: Safety/efficacy

Ibudilast for the prevention of nerve symptoms caused by oxalipatin chemotherapy, in patients with metastatic gastrointestinal cancer

Project description:Interventions: Ibudilast is a non-selective phosphodiesterase (PDE) inhibitor with anti-inflammatory properties which has shown to improve acute neurotoxicity in animal models. The study intervention will be oral ibudilast 30mg twice daily for the duration of one cycle, named Cycle B. The duration of the cycle is 2 weeks for patients receiving infusional fluorouracil with oxaliplatin (FOLFOX) and 3 weeks for patients receiving oral capecitabine with oxaliplatin (CapeOx). There is no washout period. Drug tablet return at the end of the cycle and weekly phone contact will be used to monitor adherence to the intervention. Primary outcome(s): Proportion of patients developing neurotoxicity > Grade 2 following their chemotherapy cycle as measured by the oxaliplatin-specific neurotoxicity scale (OSNS) modified. Monosialotetrahexosylganglioside Sodium Injection for Prevention Neurotoxicity of mFOLFOX 6 in mCRC

Project description:The morbidity of colorectal cancer(CRC) is 10%～15% in China.mFolfox6 has become one of the standard regimes for metastatic colorectal cancer (mCRC). Neutropenia and oxaliplatin-induced neurotoxicity are the most common adverse effects which even result in discontinue of chemotherapy, especially for patients suffered from heavily acute neurotoxicity. Monosialotetrahexosylganglioside is a component of membrane of nerve cells. Previous phase II clinical trial showed, it can reduce oxaliplatin-induced neurotoxicity(OIN). But it did not certificated by phase III trial. Investigators designed the phase III trial to investigate the effect and safety of monosialotetrahexosylganglioside Sodium Injection for prevention OIN at colorectal cancer.

Long-term Clofibric acid study

Project description:F344 rats were divided into 4 groups: vehicle control, DEN, DEN+CLO and CLO. After one week of basal diet, rats belonging to the groups DEN and DEN+CLO underwent intraperitoneal injection of DEN (30mg/kg body weight) dissolved in NaCl 9‰. The two other groups were injected with NaCl 9‰ alone. At 30mg/kg, DEN is non-necrogenic thus only exhibiting initiating properties. Twelve days after injection, diet from rats belonging to the groups CLO and DEN+CLO was changed for diet containing 5000ppm CLO for up to 608 days. Series of 5 rats from each group were necropsied at days 18, 46, 102, 264, 377, 447 (reverse phase from day 377: no more CLO in the diet for rats belonging to the DEN+CLO group), and for the CLO and Control group 524, and 608 days after the injection of DEN or saline. RNA sequencing of primary acute lymphoblastic leukemia xenograft brains with and without treatment with chimeric antigen receptor T (CAR-T) cell therapy

Project description:NSG mice were IP injected with 30mg/kg busulfan. The following day, mice received 1,000,000-3,000,000 primary blasts derived from the peripheral blood of patients with ALL. Mice were monitored for engraftment for ~10-13 weeks via tail vein bleeding for engraftment. Some mice received high dose CAR-T cell therapy and an isotype control antibody treatment. RNA was isolated using miRNeasy Micro kit (Qiagen, Gaithersburg, MD, USA) and treated with RNase-Free DNase Set (Qiagen, Gaithersburg, MD, USA). RNA-seq analyses of brain sections harvested from mice treated with CAR-T cells showed significant upregulation of genes regulating the T cell receptor, cytokine receptors, T cell immune activation, T cell trafficking, and T cell and myeloid cell differentiation. Xenografts treated with CAR-T cell therapy developed neurotoxicity.

Derivation of model and analysis of anti-inflammation. Journal of Theoretical Biology 242, 1 (2006). The acute inflammatory response, triggered by a variety of biological or physical stresses on an organism, is a delicate system of checks and balances that, although aimed at promoting healing and restoring homeostasis, can result in undesired and occasionally lethal physiological responses. In this work, we derive a reduced conceptual model for the acute inflammatory response to infection, built up from consideration of direct interactions of fundamental effectors. We harness this model to explore the importance of dynamic anti-inflammation in promoting resolution of infection and homeostasis. Further, we offer a clinical correlation between model predictions and potential therapeutic interventions based on modulation of immunity by anti-inflammatory agents.
