A Multicenter, Open-label, Prospective Randomized Controlled Trial to evaluate the Effect of Low-Molecular-Weight Heparin on Prevention of Venous Thromboembolism after Laparoscopic Surgery

ABSTRACT: Interventions: For patients allocated to the trial group, a subcutaneous injection of enoxaparin 2000 IU twice daily is administered, in addition to conventional physical prophylaxis. Enoxaparin was started 24-36 hours after surgery and continued until 7th post operative day. For patients allocated to the control group, conventional physical prophylaxis such as elastic stocking and / or intermittent pneumatic compression device is provided for a few days Primary outcome(s): The rate of incidence of all the venous thromboembolism including deep vein thrombosis and pulmonary thromboembolism Study Design: Parallel Randomized

Project description:The risk factors leading to cancer-associated thromboembolism include the patient’s gender, age, cancer type and stage, comorbidities, and therapies. Low-molecular-weight heparins (LMWH) are administrated for the prophylaxis and/or treatment of cancer and comorbid diseases associated with thrombosis. The effects of LMWH on cancer cells and overall patient survival remain controversial.

Project description:Overall, only limited evidence exists regarding the clinical benefit of antithrombotic prophylaxis after laparoscopic surgery for cancer. Four studies reported on the incidence of venous thromboembolism (VTE) after laparoscopic surgery for cancer. These studies differ concerning study design, site of cancer, regimens for antithrombotic prophylaxis and reported incidence of VTE. The aim of this multicenter, randomized study is to assess the clinical benefit of extended (4 weeks) compared to short (one week) heparin prophylaxis after laparoscopic surgery for colorectal cancer.

Project description:Primary objectives: To assess the efficacy and safety of postdischarge extended (3 weeks) antithrombotic prophylaxis with rivaroxaban compared to placebo after planned laparoscopic surgery for colorectal cancer in patients who had received antithrombotic prophylaxis with LMWH during the hospital stay for 7±2 days Primary endpoints: The primary study outcome is a composite of symptomatic objectively confirmed VTE, asymptomatic ultrasonography-confirmed DVT or VTE-related death at 28±2 days from planned laparoscopic surgery for colorectal cancer in patients randomized to rivaroxaban or placebo.Pulmonary embolism will be diagnosed in the presence of:- intraluminal filling defect at computed tomography angiography or pulmonary angiography- high probability perfusion defect at perfusion lung scan with mismatch area compared to chest X Ray.Deep vein thrombosis will be diagnosed in the presence of:- an uncompressible vein segment in the deep vein of the lower limbs as assessed by complete compression ultrasonography examination

Adaptation to Volumetric Compression Drives an Apoptosis-Resistant and Invasive Phenotype in Liver Cancer

Project description:Physical constraints like compression influence cancer cell invasion and transcriptional dynamics in various tumors. Liver cancer is characterized by the rapid proliferation of tumor cells within a densely packed tissue matrix, subjecting the cancer cells to crowding and compression. The highly dysregulated mechanical environment highlights the need to elucidate the broader impact of compression on liver cancer development and evolution. In this study, we investigated and described a unique adaptive response of liver cells to prolonged compression. Liver cells presented significant transcriptional changes due to compression, including the loss of liver-specific markers and enrichment of epithelial-to-mesenchymal transition genes. Compression elevated Rac1 activity, which promoted cellular protrusions and YAP nuclear translocation and maintained cell viability under mechanical stress. Furthermore, compression disrupted intracellular calcium signaling, leading to resistance to apoptosis. Counteracting the effects of compression by inhibiting Rac1 or manipulating intracellular calcium facilitated death of compression-adapted cells. This study highlights compression as a critical biophysical signal in the tissue microenvironment that can induce cell state transitions and disease-driving phenotypes in the liver.

Transcriptomic Analysis of Long Non-coding RNAs and mRNAs in a Mouse Model of Venous Thrombosis

Project description:Recent advancements have identified numerous long non-coding RNAs (lncRNAs) involved in various biological processes and diseases. However, the roles of lncRNAs in venous thromboembolism (VTE) remain largely unexplored. This study aims to elucidate the expression profiles of lncRNAs and mRNAs in a well-established animal model of VTE. Acute thrombosis was induced by surgical ligation of the inferior vena cava (IVC) within 24 hours. The thrombus burden in venous vessels was evaluated using hematoxylin and eosin staining and Masson's trichrome staining. The IVC and associated thrombus were harvested 24 hours post-ligation, and the extracted RNA was subjected to RNA sequencing (RNA-seq) analysis. This study offers a comprehensive overview of the expression profiles of lncRNAs and mRNAs in a murine model of venous thrombosis, providing novel insights into the roles of RNAs in VTE.

Absence of COVID-19-associated Changes in Coagulation and Thrombosis in the Ferret Model

Project description:Many patients who are diagnosed with coronavirus disease 2019 (COVID-19) suffer from venous thromboembolic complications despite the use of stringent anticoagulant prophylaxis. Studies on the exact mechanism(s) underlying thrombosis in COVID-19 are limited as animal models commonly used to study venous thrombosis pathophysiology (i.e. rats and mice) are naturally not susceptible to Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). Ferrets are susceptible to SARS-CoV-2 infection, successfully used to study virus transmission, and were previously used to study activation of coagulation and thrombosis during influenza virus infection. Here, we used plasma and lung material from SARS-CoV-2-inoculated ferrets to explore their use in studying COVID-19-associated changes in coagulation and thrombosis. Lungs of ferrets inoculated intranasally with SARS-CoV-2 demonstrated alveolar septa that were mildly expanded by macrophages, and diffuse interstitial histiocytic pneumonia. However, no macroscopical or microscopical evidence of vascular thrombosis in the lungs of SARS-CoV-2-inoculated ferrets was found. Longitudinal plasma profiling using a mass spectrometry-based approach revealed minor differences in plasma protein profiles in SARS-CoV-2-inoculated ferrets up to 2 weeks post-infection. Apart from fibrinogen, the majority of plasma coagulation factors were stable and demonstrated a low coefficient of variation. We conclude that while ferrets are an essential and well-suited animal model to study SARS-CoV-2 transmission, their use to study SARS-CoV-2-related changes relevant to thrombotic disease is limited.
