Evaluation of the extraction site location in laparoscopic colorectal surgery midline incision vs off midline incision Randomized controlled study

ABSTRACT: Interventions: midline incision Extend the trocar hole in the umbilicus. Closure wound is suturing the white line and the peritoneum all together, suturing closure of the skin, compressing the cotton ball into the umbilicus. off midline incision Extend the transrectus trocarshole. Closure wound closes the skin by suturing the peritoneum and the posterior sheath, suturing the rectus abdominis anterior sheath and suturing the skin. Primary outcome(s): We will evaluate the small incision in the first year after surgery using postoperative wound evaluation POSAS (patient and observer scar assessment scale). For the closure of midline incision, patients allocated in experimental group will receive continuous running suture using absorbable barbed suture material. Patients allocated in control group will receive continuous running suture using absorbable nonbarbed suture material. Proteomics analysis of wound healing in porcine subcutaneous fatty tissue postmonopolar electrosurgery and harmonic blade incisions.

Project description:Two female pigs weighing 40-55 kg were used. Each animal had four 4-cm midline caudal abdominal incisions created with the test devices. The test devices were monopolar electrosurgery (Electrocautery Pencil, ConMed, Utica NY) and Harmonic blade (HK105, Ethicon Endo-Surgery, Cincinnati OH). Each animal had two Harmonic Blade incisions and two Electrosurgery incisions for a total of 8 incisions. The incisions were created by marking the site on the ventral aspect of the abdomen and identified by level from cranial to caudal, as incision A, B, C and D respectively. The incision was started with a scalpel blade through the skin (epidermis and dermis). Each incision was deepened through the subcutaneous tissues down to the linea alba using the Harmonic at power level 5 for cutting tissue and power level 3 for coagulating bleeders, or electrosurgery set at 40 Watts for cutting and Blend 2/Spray for coagulation. The linea alba was not incised. Closure of the incision was accomplished by apposing the skin with 3-0 nylon in a simple interrupted pattern. Dermabond was then applied to seal the epidermis. No bandages were applied to the incisions. After three days, incision sites were removed en bloc from the abdominal wall. The tissue block was sliced perpendicular to the surgically incision to create slabs 4-6 mm thick. The slab was trimmed to remove the skin from the top and muscle from the bottom of the sample. The side walls were trimmed to approximately 4 mm from the surgical incision leaving a rectangular block of subcutaneous fatty tissue. Four to six samples were taken from each incision. Animals were then euthanized. Samples for genomics and proteomics analysis were placed in sterile DNAse-, RNAse-, protein-free Eppendorf tubes, flash frozen in liquid nitrogen, held on dry ice throughout tissue collection, and then stored at -80 C. Tissue samples were also collected in 10% neutral buffered formalin for histopathology analysis.

Parastomal Hernia Prevention Trial

Project description:Interventions: Mesh Intervention Procedure: Subjects in this arm of the study will have mesh permanently implanted at the stoma site at the time of their surgery. The procedure duration will differ between patients depending on the circumstances of, and indications for, the surgery. The stoma formation will be performed as follows: 1. Dissect away a section of peritoneum and posterior rectus sheath on the side of the colostomy from the posterior aspect of the rectus abdominus muscle with the centre of the superior-inferior aspect of this area at the level of the marked colostomy trephine site. 2. Place a piece of 15cm x 15cm mesh in the space. 3. Secure the lateral corners of the mesh to the lateral abdominal wall musculature with a permanent suture. 4. Make the trephine at the marked site using a circular skin incision and deepen through abdominal wall fat to the fascia. Make a cruciate incision in the anterior rectus sheath, split the rectus abdominus muscle longitudinally. 5. Cut a circle in the centre of the mesh and the peritoneum of a size just adequate to allow easy passage of the colon through the mesh and then the abdominal wall. 6. Deliver the colon through the trephine site to the skin. 7. Close fascia. 8. Irrigate wound. 9. Close the midline wound skin. 10. Apply a dressing to the midline wound. 11. Mature the colostomy using dissolvable sutures. 12. Apply the colostomy bag. Primary outcome(s): The formation of clinically-apparent parastomal herniae. Pathogenic mechanisms of Pseudomonas aeruginosa wound infection in response to murine fascia

Project description:Wound infections are traditionally thought to occur when microbial burden exceeds the innate clearance capacity of host immune system. Here we introduce the idea that the wound environment itself plays a significant contributory role to wound infection. We developed a clinically relevant murine model of soft tissue infection to explore the role of activation of microbial virulence in response to tissue factors as a mechanism by which pathogenic bacteria cause wound infections. Mice underwent abdominal skin incision and light muscle injury with a crushing forceps versus skin incision alone followed by topical inoculation of Pseudomonas aeruginosa. Pseudomonas aeruginosa whole genome transcriptional profiling demonstrated that fascia induced the activation of multiple genes responsible for the synthesis of the iron scavenging protein pyochelin.

Array analysis of wound induced hair neogenesis (late stage)

Project description:Mice were wounded and measured for regeneration starting 4 days after wound closure with simultaneous measurement of hair follicle neogenesis and biopsing. At each time point, RNA was collected from one mouse with high number of regenerated follicles and one without regenerated follicles. Whole skin biopsies of wound scars were submitted for Affymetrix Exon arrays. 3 replicates of mice with high number of regenerated follicles, 3 replicates of mice with no regenerated follicles; each pair taken at a different date after wound closure.

We compared Mixed mice (B6/FVB/SJL), a strain of high regeneration, versus C57bl mice, a strain of low regeneration. Whole skin biopsies of wound scars were submitted for Affymetrix Exon arrays. 4 mice each of 2 distinct strains of differing regeneration levels were collected.

CXCR2 knockout promotes skin and hair regeneration without scarring

Project description:Mammals heal faster with imperfect fibrotic scars, while amphibians regenerate slower with scarless wound healing. These observations support the prevailing paradigm that speed of wound closure is inversely related to repair quality. Here we find evidence that this is a false trade-off. In multiple injury models, mice lacking CXCR2 (CXCR2-KO) globally improved both speed and quality of skin wound healing, including hair regeneration. We found CXCR2 primarily expressed in neutrophils, and injury induced neutrophils to secrete neutrophil extracellular traps (NETs). Mice engineered to be specifically deficient in myeloid CXCR2 or NET production partially recreated the phenotype with improved early speed of wound closure. Thus, CXCR2+ neutrophils regulate the speed of wound closure.
