Outcome of Transanal Specimen Extraction Surgery for Elderly Colorectal Cancer Patients

ABSTRACT: Retrospective analysis of a cohort of elderly CRC patients (aged ≥ 65 years) underwent transanal-NOSES between August 2008 and February 2022 was constructed. Data were obtained from China NOSES Database (CNDB). The primary outcomes included time to first flatus, postoperative hospital stays, 30-day postoperative complication and hospital mortality. The secondary outcomes were overall survival (OS) and disease-free survival (DFS).

Integrated methylomic and transcriptomic characterisation of postoperative systemic inflammatory dysregulation (METHYLATION)

Project description:The inflammatory response to surgery is essential for healing and recovery, however hyperinflammation and altered immune competence increases the risk of inflammation mediated complications. We hypothesised that, in some patients, a state of postoperative systemic inflammatory dysregulation (PSID) exists increasing the risk or infection and sepsis, and that this will be associated with, and potentially predicted by, altered patterns of genome-wide peripheral blood mononuclear cell differential DNA methylation and gene expression. In this study we utilised phenotypic extremes of postoperative plasma C-reactive protein levels to define PSID, which manifested clinically in significantly higher rates of sepsis, complications and longer hospital stays following major abdominal surgery. We identified altered DNA methylation and differential gene expression in specific immune and metabolic pathways during PSID. Our findings suggest that dysregulation results in, or from, dramatic changes in differential DNA methylation and highlights potential targets for early detection and treatment. The combination of altered DNA methylation and gene expression confirms that dysregulation is mediated at multiple levels within specific gene sets and hence, non-specific anti-inflammatory treatments such as corticosteroids alone are unlikely to represent an effective therapeutic strategy.

HuiyiLi2024 - Prediction of death risk in patients with acute diquat poisoning using Random Forest Model

Project description:The objective of this study was to develop and validate predictive models to assess the risk of death in patients with acute diquat (DQ) poisoning using innovative machine learning techniques. The study analyzed data from 201 consecutive patients admitted for deliberate oral intake of DQ between February 2018 and August 2023 at the First Hospital and Shengjing Hospital of China Medical University. Initial clinical data were collected, and four machine learning methods—logistic regression, random forest, support vector machine (SVM), and gradient boosting—were applied to build the prediction models. The dataset was split into a training set and a test set in an 8:2 ratio. The performance of these models was evaluated in terms of discrimination, calibration, and clinical decision curve analysis (DCA). Additionally, the SHapley Additive ExPlanations (SHAP) interpretation tool was used to provide an intuitive explanation of the risk of death in patients with DQ poisoning by calculating the contribution and impact of each feature on the final prediction. The areas under the receiver operating characteristic curves (AUCs) for the models were 0.91 for logistic regression, 0.98 for random forest, 0.96 for SVM, and 0.94 for gradient boosting. The random forest model demonstrated the best predictive performance with the highest AUC of 0.98, highest F1-score (0.90), highest Matthews correlation coefficient (0.79), highest accuracy (0.90), and the lowest Brier score (0.07). The study concludes that these machine learning models, combined with SHAP, provide reliable and interpretable tools for predicting the death risk in patients with acute DQ poisoning, with the random forest model being identified as the best performing model.

Proteome of cerebral thrombi retrieved in acute phase of stroke (ThrombiOMIC)

Project description:The proteome of cerebral thrombi from patients recanalized by mechanical thrombectomy in an acute phase of stroke was carried out to determine biomarkers of stroke radio-clinical outcomes for stroke physicians (clinical outcome and mortality at 3 months, intracranial hemorrhagic transformation, etc.). Proteome of cerebral thrombi was also used to elucidate stroke physiopathology (paradoxical effect of glucose in acute phase of stroke) and evaluate recanalization treatments (intravenous thrombolysis by rt-PA and/or thrombectomy efficiency). ThrombiOMIC database was incremented by proteomic analyses of cerebral thrombi successfully retrieved from all consecutive stroke patients (n=59) with large vessel occlusion treated by mechanical thrombectomy in acute phase between October 2018 and August 2019 in the intensive care stroke unit of the University Hospital of Nice (France).

High expression of oleoyl-ACP-hydrolase predicts mortality in patients hospitalized with avian A(H7N9) influenza

Project description:Although respiratory viral infections cause significant morbidity and mortality, it is still not fully clear why some individuals succumb to severe disease. In patients hospitalized with avian A(H7N9) influenza, we investigated early drivers underpinning hypercytokinemia and fatal disease. Our transcriptomics studies identified differential expression of 10 early host genes, defined by 16 probe sets, between patients who recovered and died. Seven probe sets were specific for the same host gene encoding for a key enzyme mediating free fatty acid production, oleoyl-ACP-hydrolase (OLAH). High olah transcript levels in fatal A(H7N9) patients were detected early after hospital admission and persisted until patients died. Conversely, patients who recovered had minimal olah transcript expression throughout their hospital stays.
