Global gene expression profiling of tissue engineered neofascia of hernia patients with good and bad healing characterisitics.

ABSTRACT: The prevalence of hernia after laparatomy has been estimated at 2-15%. Apart from male gender, smoking and obesity (BMI>25), increased age (>45 years) were identified as potential risk factors. To support the wound healing process, synthetic mesh is now routinely used in laparatomy. A patient's risk for hernia formation and relapse can be estimated by his/her collagen type I / collagen type III ratio using sirius red stains.

Polypropylene mesh implantation for hernia repair triggers myeloid cell-dependent persistent inflammatory foreign body reaction

Project description:Polypropylene meshes that are commonly used for surgical groin hernia repair may trigger granulomatous foreign body reactions. Here, we show that asymptomatic patients display mesh-associated inflammatory granulomas long after surgery, which are dominated by monocyte-derived macrophages. In mice, subdermal mesh implantation induces a rapid and strong myeloid cell accumulation, without substantial attenuation for up to 90 days. Myeloid cells segregate into distinct macrophage subsets with separate spatial distribution, activation profiles and functional properties. Protein mass spectrometry confirms the inflammatory nature of the foreign body reaction, as characterized by cytokines, complement activation and immunoglobulin deposition.

Hernia After Colorectal Cancer Surgery

Project description:Defects in the abdominal wall (incisional hernia) is a frequent negative outcome after surgery. Reinforcing the incision with mesh seem to lower the incidence but in surgery that includes bowel resection a simple alternative, cheaper and less prone to infection than a synthetic mesh, would be of interest. The primary aim of this multicentre randomized controlled trial is to compare the incisional hernia incidence one year after planned colorectal cancer surgery performed through a midline incision which is closed either by a standardized small stitch 4:1 technique (the incision is closed with the use of suture of 4 times the length of the incision) or with the same technique plus a reinforced tension-line suture (a suture is applied in the fibrous tissue parallel to the incision which is then embraced by the 4:1 suture when the incision is closed). A difference in incisional hernia of 15% (20% without and 5% with reinforced tension-line suture) is assumed. Secondary aims are to evaluate incidences of wound dehiscence, other wound complications and incisional hernia after 3 years. Furthermore we aim to evaluate patient satisfaction and quality of life.

Project description:Analysis of embronic day 30.5 (E30.5) fetal rabbit left lung after creation of diapragmatic hernia (DH) at E25 and tracheal occlusion (TO) at E27. The fetuses of timed pregnant New Zealand rabbits were subjected to creation of a left diaphragmatic hernia at E25 with or without tracheal occlusion at E27. At E30.5, the fetuses were sacrificed with collection of the lateral aspect of the left upper lobe for NextGen mRNA sequencing.

Incisional hernia recurrence through genomic profiling: a pilot study

Project description:Skin and intact fascia were collected from 15 normal control (NC) patients with no hernia history and 18 patients presenting for recurrent incisional hernia (RH) repair. Microarray analysis was performed using whole genome microarray chips on NC (n = 8) and RH (n = 9). These samples were further investigated using a pathway-specific PCR array containing fibrosis-related genes.

Predicting Ventral Hernia Recurrence Through Genomic Profiling: An Observational Study

Project description:Patients with a history of recurrent ventral hernias (n=10) were compared with a control group (n=8) using high-throughput microarrays. Skin and fascia samples (taken away from the site of the ventral hernia) were analyzed from both groups. Our experiments show distinct genetic profiles between the skin and fascia of recurrent ventral hernia patients as compared to controls. There were also a great number of statistically significant genes in the skin as compared to the fascia. The functions of the genes in the skin were diverse and included wound healing, transcription regulation, and immunology. The genes in the fascia, however, were more specific concentrating primarily on immunology, regulation of gene expression and cellular metabolism, and development. Total RNA extracted from skin and fascia of patients with a history of recurrent ventral hernias and a control group. There were no replicates for the same source material for the same patient.

Parastomal Hernia Prevention Trial

Project description:Interventions: Mesh Intervention Procedure: Subjects in this arm of the study will have mesh permanently implanted at the stoma site at the time of their surgery. The procedure duration will differ between patients depending on the circumstances of, and indications for, the surgery. The stoma formation will be performed as follows: 1. Dissect away a section of peritoneum and posterior rectus sheath on the side of the colostomy from the posterior aspect of the rectus abdominus muscle with the centre of the superior-inferior aspect of this area at the level of the marked colostomy trephine site. 2. Place a piece of 15cm x 15cm mesh in the space. 3. Secure the lateral corners of the mesh to the lateral abdominal wall musculature with a permanent suture. 4. Make the trephine at the marked site using a circular skin incision and deepen through abdominal wall fat to the fascia. Make a cruciate incision in the anterior rectus sheath, split the rectus abdominus muscle longitudinally. 5. Cut a circle in the centre of the mesh and the peritoneum of a size just adequate to allow easy passage of the colon through the mesh and then the abdominal wall. 6. Deliver the colon through the trephine site to the skin. 7. Close fascia. 8. Irrigate wound. 9. Close the midline wound skin. 10. Apply a dressing to the midline wound. 11. Mature the colostomy using dissolvable sutures. 12. Apply the colostomy bag. Primary outcome(s): The formation of clinically-apparent parastomal herniae.
