Project description:Crohn’s disease is a relapsing inflammatory disorder with a variable clinical course. While most patients present with purely an inflammatory phenotype (B1) at diagnosis, a subgroup (~20%) rapidly progresses to complicated disease manifestations that include stricturing (B2) within 5 years. DNA methylation is a key epigenetic mechanism that can regulate gene expression and thereby influence the development and progression of complex diseases. Site-specific DNA methylation differences have been reported in peripheral blood of patients with Crohn’s disease, but investigation of the temporal relationship between methylation and disease is required to establish whether the methylome plays a causal role and can be leveraged for therapeutic benefits. To this end, we conducted an epigenome-wide study of methylation (~850K sites) in peripheral blood at diagnosis and during follow-up from the RISK pediatric Crohn’s disease inception cohort. While some methylation changes associated with Crohn’s disease might be causal, in peripheral blood the vast majority are found to be a transient consequence of inflammation and thus a symptom of disease.

Project description:This longitudinal dataset was generated to assess changes in whole blood gene expression following low dose Mycobacterium tuberculosis infection of Cynomolgus macaques. In addition to changes in mRNA transcript abundance, changes in circulating immune cell populations were also assessed using both complete blood counts (CBC) and using flow cytometry. Disease severity was determined using both clinical diagnosis (active vs. latent) and 18F-flurodeoxyglucose (FDG avidity) by 6 months post-infection. The greatest change in transcriptional activity occurred 20-56 days after infection, when the maximum activity of immune related transcripts was observed. After stratifying macaques based on clinical diagnosis, those macaques with active disease exhibited a similar signature to that observed in humans with active TB. However, only modest transcriptional differences between active TB and latently infected macaques were observed. As an alternative stratification approach, the severity of lung inflammation measured by lung FDG avidity was used to separate macaques into high and low FDG groups. Blood transcript activity was highly correlated with lung inflammation at early time points post-infection. The differential expression signatures between animals stratified with high and low lung FDG were stronger than those observed with groups defined by clinical outcome.. Furthermore, gene level analysis of of pre-infection signatures suggest that interferon and inflammation signatures may distinguish eventual clinical outcomes and lung FDG avidity prior infection. In conclusion, this study demonstrates that transcriptional changes in the macaque model recapitulate those observed in human Mtb infection while providing additional insight into the early and pre-infection events associated with disease severity and outcome.

Project description:Severely-afflicted COVID-19 patients can exhibit disease manifestations representative of sepsis, including acute respiratory distress syndrome and multiple organ failure. We hypothesized that diagnostic tools used in managing all-cause sepsis, such as clinical criteria, biomarkers, and gene expression signatures, should extend to COVID-19 patients. Here we analyzed the whole blood transcriptome of 124 early (1-5 days post-hospital admission) and late (6-20 days post-admission) sampled patients with confirmed COVID-19 infections from hospitals in Quebec, Canada. Mechanisms associated with COVID-19 severity were identified between severity groups (ranging from mild disease to the requirement for mechanical ventilation and mortality), and established sepsis signatures were assessed for dysregulation. Specifically, gene expression signatures representing pathophysiological events, namely cellular reprogramming, organ dysfunction, and mortality, were significantly enriched and predictive of severity and lethality in COVID-19 patients. Mechanistic endotypes reflective of distinct sepsis aetiologies and therapeutic opportunities were also identified in subsets of patients, enabling prediction of potentially-effective repurposed drugs. The expression of sepsis gene expression signatures in severely-afflicted COVID-19 patients indicates that these patients should be classified as having severe sepsis. Accordingly, in severe COVID-19 patients, these signatures should be strongly considered for the mechanistic characterization, diagnosis, and guidance of treatment using repurposed drugs.

Project description:BACKGROUND & AIMS:Crohn's disease is a relapsing and remitting inflammatory disorder with a variable clinical course. Although most patients present with an inflammatory phenotype (B1), approximately 20% of patients rapidly progress to complicated disease, which includes stricturing (B2), within 5 years. We analyzed DNA methylation patterns in blood samples of pediatric patients with Crohn's disease at diagnosis and later time points to identify changes that associate with and might contribute to disease development and progression. METHODS:We obtained blood samples from 164 pediatric patients (1-17 years old) with Crohn's disease (B1 or B2) who participated in a North American study and were followed for 5 years. Participants without intestinal inflammation or symptoms served as controls (n = 74). DNA methylation patterns were analyzed in samples collected at time of diagnosis and 1-3 years later at approximately 850,000 sites. We used genetic association and the concept of Mendelian randomization to identify changes in DNA methylation patterns that might contribute to the development of or result from Crohn's disease. RESULTS:We identified 1189 5'-cytosine-phosphate-guanosine-3' (CpG) sites that were differentially methylated between patients with Crohn's disease (at diagnosis) and controls. Methylation changes at these sites correlated with plasma levels of C-reactive protein. A comparison of methylation profiles of DNA collected at diagnosis of Crohn's disease vs during the follow-up period showed that, during treatment, alterations identified in methylation profiles at the time of diagnosis of Crohn's disease more closely resembled patterns observed in controls, irrespective of disease progression to B2. We identified methylation changes at 3 CpG sites that might contribute to the development of Crohn's disease. Most CpG methylation changes associated with Crohn's disease disappeared with treatment of inflammation and might be a result of Crohn's disease. CONCLUSIONS:Methylation patterns observed in blood samples from patients with Crohn's disease accompany acute inflammation; with treatment, these change to resemble methylation patterns observed in patients without intestinal inflammation. These findings indicate that Crohn's disease-associated patterns of DNA methylation observed in blood samples are a result of the inflammatory features of the disease and are less likely to contribute to disease development or progression.

Project description:Stromal cell (SC) contributions to intestinal inflammation are inadequately understood. Here we show that CD90+vimentin+SMA- SCs isolated from Crohn’s disease intestine (Crohn’s SCs) displayed higher levels of spontaneously released and microbiota-induced inflammatory mediators, including IL-6 and TGF-b, and surface adhesion molecules, than SCs isolated from healthy intestine (Normal SCs). Comparative transcriptome analysis confirmed the predominance of upregulated inflammatory response and cytokine signaling genes and pathway molecules in Crohn’s SCs. Importantly, intestinal SCs provided retinoic acid (RA), which regulated DC maturation, and in an in vitro cross-talk system E. coli LF82-stimulated Crohn’s SCs promoted differentiation of more inflammatory TNFhi/IL-12lo/RAlo DCs and IFN-ghi/IL-17hi effector T cells than Normal SCs. Explaining this finding, Crohn’s SCs synthesized less RA and expressed more retinaldehyde reductase DHRS3, which inhibits retinol conversion to retinal, than Normal SCs. Our findings uncover microbe-SC crosstalk in which mucosal SCs initiate and perpetuate DC-driven T-cell-mediated inflammation in Crohn’s disease.

Project description:The enriched low mass proteome is unexplored as a source of differentiators for diagnosing and monitoring Inflammatory Bowel Disease (IBD) activity, less invasively than colonoscopy and histopathology. Differences in the enriched low mass plasma proteome (<25kDa) were assessed by label-free quantitative mass-spectrometry. A panel of marker candidates were progressed to validation phase and ‘Tier-2’ FDA-level validated quantitative assay. Proteins important in maintaining gut barrier function and homeostasis at the epithelial interface have been quantitated by Multiple Reaction Monitoring (MRM) in plasma and serum including both inflammatory rheumatoid arthritis controls (RA) and non-inflammatory healthy controls (C), ulcerative colitis (UC) and crohn’s disease (CD) patients. Detection by immunoblot confirmed presence at the protein level in serum. Correlation analysis and receiver operator characteristics were used to report the sensitivity and specificity. Five peptides discriminating IBD activity and severity had very little-to-no correlation to Erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), white cell or platelet counts. Three of these peptides were found to be binding partners to SPP24 protein alongside other known matrix proteins. These Proteins have the potentially improve effective diagnosis and evaluate IBD activity, reducing the need for more invasive techniques.
