Project description:Coronary artery disease (CAD) and ischaemic cardiomyopathy are leading causes of mortality and morbidity worldwide. Currently there are only two well established biomarkers used for heart failure diagnosis and prognosis. Therefore, more research into the genetic and epigenetic mechanisms of heart failure (HF) will allow identification of biomarkers that can be used for different stages of HF. DNA methylation is an epigenetic mark that is increasingly recognised to play a critical role in cardiovascular disease. However, there are no methylation studies that specifically focus on ischaemic cardiomyopathy. Here we performed a small scale methylome wide association study to identify epigenetic markers in peripheral blood in a cohort of patients with and without HF who otherwise share a similar coronary ischaemic burden, age, sex, and ethnicity. We identified 68 significantly differentially methylated regions in our data. Of these regions, 48 occurred within gene bodies and 25 were located near enhancer elements, some within coding genes and some in non-coding genes. Our gene set enrichment analyses identified 103 significantly enriched gene sets in HF. We have demonstrated the utility of methyl-binding domain capture sequencing to evaluate DNA methylation markers in a perioperative cohort of cardiac surgical patients with ischaemic cardiomyopathy and HF. The peripheral methylation status of specific coding genes, such as HDAC9, are candidates for larger longitudinal studies. We have further demonstrated the value and feasibility of examining DNA methylation patterns from peripheral blood to highlight cellular pathways and processes that may contribute to the development of HF.

Project description:A porcine microarray study of right ventricular failure due to coronary artery ligation of the right ventricular free wall and subsequent treatment of right ventricular failure by volume unloading using a shunt between superior vena cava and the pulmonary artery (Glenn-shunt) 1. Surgical preparation with a 12 mm graft between superior vena cava and pulmonary artery, the graft is then clamped - Baseline sample using a biopsy needle. 2. After surgical preparation the coronary arteries of the right ventricular free wall are ligated, then heart failure develops over 120 minutes - Failure sample using a biopsy needle. 3. The shunt is then opened and the superior vena cava closed between the shunt and right atrium, diverting the blood from superior vena cava through the shunt for a period of 15 minutes partially unloading the right ventricle - Shunt sample using a biopsy needle. A series of six pigs, three samples from each animal: baseline, failure and shunt/treatment.

Characteristics of gene expression in epicardial adipose tissue and subcutaneous adipose tissue in patients at risk for heart failure undergoing coronary artery bypass grafting

Project description:The study aimed to uncover the biological significance of epicardial adipose tissue (EAT) in heart failure (HF) development by comparing its gene expression to that of subcutaneous adipose tissue (SAT). Tissue samples were collected from 21 patients undergoing coronary artery bypass graft surgery to analyze differences in gene expression. The research further explored the relationships between these tissues in individuals categorized into different HF risk levels—specifically HF stage A (n=12) and pre-HF stage B (n=9). Analysis identified distinct gene expression profiles between EAT and SAT, revealing 17 gene clusters in EAT associated with characteristics of HF. These clusters were evaluated for their correlation with clinical HF markers to highlight their potential influence on heart function and disease progression. Comparative platelet releasate proteome profiling of acute coronary syndrome versus stable coronary artery disease

Project description:Upon activation, platelets release a host of soluble and vesicular signals, collectively termed the ‘platelet releasate’ (PR). The contents of this PR play a significant role in haemostasis, inflammation, and pathologic sequelae. Despite this, proteomic studies investigating the PR in coronary artery disease have not been performed. We undertook a comparative label-free quantitative (LFQ) proteomic profiling of the 1U/ml thrombin-induced PR from 13 acute coronary syndrome (ACS-STEMI) versus 14 stable angina pectoris patients using a tandem mass spectrometry approach. We identified differentially released platet proteins including tetranectin (CLEC3B), protein disulfide-isomerase-A3 (PDIA3), coagulation factor V (F5) and fibronectin (FN1). Strikingly, all 9 differential proteins were associated with the GO cellular component term ‘extracellular vesicle’ and reduced levels of EVs were detected in plasma of ACS-STEMI patients. Network analysis revealed 3 PR proteins either reduced (F5; FN1) or absent (CLEC3B) in ACS-STEMI patients, which are strongly connected to both the clotting cascade and major druggable targets on platelets. This moderated signature highlights the possible basis of platelet dysfunction in ACS-STEMI and may prove useful for non-invasive risk assessment of the progression of coronary artery disease.
