Project description:Pathogen-host competition for manganese and intricate immunostimulatory pathways severely attenuates the efficacy of antibacterial immunotherapy against biofilm infections associated with orthopaedic implants. Herein, we introduce an unprecedented spatiotemporal sono-metalloimmunotherapy (SMIT) strategy aimed at efficient biofilm ablation by custom design of ingenious biomimetic metal-organic framework (PCN-224)-coated MnO2-hydrangea nanoparticles (MnPM) as a metalloantibiotic. Upon reaching the acidic H2O2-enriched biofilm microenvironment, MnPM can convert abundant H2O2 into oxygen, which is conducive to significantly enhancing the efficacy of ultrasound (US)-triggered sonodynamic therapy (SDT), thereby exposing bacteria-associated antigens (BAAs). Moreover, MnPM disrupts bacterial homeostasis, further killing more bacteria. Then, the Mn ions released from the degraded MnO2 can recharge immune cells to enhance the cGAS-STING signaling pathway sensing of BAAs, further boosting the immune response and suppressing biofilm growth via biofilm-specific T cell responses. Following US withdrawal, the sustained oxygenation promotes the survival and migration of fibroblasts, stimulates the expression of angiogenic growth factors and angiogenesis, and neutralizes excessive inflammation. Our findings highlight that MnPM may act as an immune costimulatory metalloantibiotic to regulate the cGAS-STING signaling pathway, presenting a promising alternative to antibiotics for orthopaedic biofilm infection treatment and pro-tissue repair.

Biodegradable Janus sonozyme with continuous reactive oxygen species regulation for treating infected critical-sized bone defects

Project description:Critical-sized bone defects are usually accompanied by bacterial infection leading to inflammation and bone nonunion. However, existing biodegradable materials lack long-term therapeutical effect because of their gradual degradation. Here, a degradable material with all time ROS modulation is proposed, defined as a sonozyme due to its functions as a sonosensitizer and a nanoenzyme. Before degradation, the sonozyme can exert an effective sonodynamic antimicrobial effect through the dual active sites of MnN4 and Cu2O8. Furthermore, it can promote anti-inflammation by superoxide dismutase and catalase activities. Following degradation, quercetin-metal chelation exhibits a sustaining antioxidant effect through ligand-metal charge transfer, while the released ions and quercetin also have great self-antimicrobial, osteogenic, and angiogenic effects. A rat model of infected cranial defects demonstrates the sonozyme can rapidly eliminate bacteria and promote bone regeneration. This work presents a promising approach to engineer biodegradable materials with long-time effects for infectious bone defects.

Osteoinductive biodegradable intramedullary implant accelerates bone healing and mitigates complications in a rat bone transport model

Project description:Bone transport distraction osteogenesis (DO) is one of the most successful surgery-driven endogenous tissue regeneration approaches for the treatment of large bone defects. However, prolonged consolidation, docking site nonunion, and pin tract infection remain challenging complications. Here, we engineered an osteoinductive, biodegradable intramedullary (IM) implant for sustained release of bone morphogenetic protein-2 (BMP-2) as an adjunctive therapy of bone transport to address the clinical challenges. A hybrid tissue engineering construct (HyTEC) technique was developed to enable sustained release of BMP-2 in a broad range. 100% bony fusion was achieved in the IM implants incorporating with 2 μg and 6 μg BMP-2 as early as 34 days after bone transport surgeries in the management of 8-mm femoral defect. Load bearing was restored 55 days after surgeries when the fixator was removed. Eluting BMP-2 from the IM implants accelerates bone formation and angiogenesis at early phase, and increase mineralization at late phase, especially at the docking sites, leading to early bony bridging. Moreover, no pin tract infection but seamless integration could be found in the 2 μg and 6 μg BMP-2 treated groups. Surgical control and IM implant showed high proportion of non-union and pin tract infections. 2 μg BMP-2 delivered by collagen sponge or 0.5 μg BMP-2-laden IM implant did not induce bone regeneration effectively, resulting in some non-unions and infections. A presence of bacteria of fecal origin in the infection sites was identified. In conclusion, this osteoinductive IM implant holds great promise in revolutionizing bone transport DO technique in the management of bone defect by accelerating bone regeneration and mitigating complications.

Tissue niche influences immune and metabolic profiles to Staphylococcus aureus biofilm

Project description:Infection is a devastating post-surgical complication, often necessitating additional procedures and prolonged antibiotic therapy. This is especially relevant for craniotomy and prosthetic joint infections, both of which are characterized by biofilm formation on the bone or implant surface, respectively, with S. aureus representing a primary cause. The effectiveness of immune responses to these infections is predicated on both host- and pathogen-derived signals in the infection microenvironment. However, the extent to which these signals differ across distinct tissue niches and influence immune function remains relatively unknown. Using mouse models of S. aureus craniotomy and prosthetic joint infection complemented with patient samples from both infectious modalities, we show profound metabolomic, transcriptomic, and functional differences that are dependent on tissue niche. These signatures were both spatially and temporally distinct, differing not only between surgical site infections but evolving over time within a single model. These findings highlight the unique immune attributes of biofilms that are heavily influenced by the local tissue microenvironment, which will likely have important implications when designing therapeutic approaches to target specific infections.

A biodegradable, flexible photonic patch for in vivo phototherapy

Project description:Therapeutic and diagnostic illumination on internal organs and tissues with high controllability and adaptability in terms of spectrum, area, depth, and intensity remains a major challenge. Here, we present a flexible, biodegradable photonic device iCarP featured with a micrometer scale air gap constructed between a refractive polyester patch and the embedded removable tapered optical fiber (TOF). ICarP combines the advantages of light diffraction by TOF, dual refractions on TOF/air and air/patch interfaces of the air gap, and reflection inside the patch to obtain a “bulb-like” illumination, guiding the light towards the target tissue. Large area, high intensity, wide spectrum, continuous/pulsatile, deeply penetrating illumination was achieved without puncturing the target tissues. ICarP supported phototherapies with different photosensitizers and achieved higher efficacy compared to optical fibers with clinically relevant optical fiber structures, attributed to the light scattering effect. The robust photonic device is compatible with thoracoscopy-based minimally invasive implantation and exhibited consistent performance on mechanically dynamic animal hearts in a demonstrative application of in situ photosynthesis for myocardial infarction treatment. The iCarP phototherapy improved survival of cardiac cells and preserved cardiac functions. These initial results showed that iCarP could be a safe, precise and widely applicable device suitable for internal organs/tissues illumination and associated therapy and diagnosis.

Biofilm Heterogeneity-Adaptive Photoredox Catalysis Enables Red Light-Triggered Nitric Oxide Release for Combating Drug-Resistant Infections

Project description:The formation of biofilms is closely associated with persistent and chronic infections, and physiological heterogeneity such as pH and oxygen gradients renders biofilms highly resistant to conventional antibiotics. To date, effectively treating biofilm infections remains a significant challenge. Herein, we report the fabrication of micellar nanoparticles adapted to heterogeneous biofilm microenvironments, enabling nitric oxide (NO) release through two distinct photoredox catalysis mechanisms. The key design feature involves the use of tertiary amine (TA) moieties, which function as sacrificial agents to avoid the quenching of photocatalysts under normoxic and neutral pH conditions and proton acceptors at acidic pH to allow deep biofilm penetration. This biofilm-adaptive NO-releasing platform shows excellent antibiofilm activity against ciprofloxacin-resistant Pseudomonas aeruginosa (CRPA) biofilms both in vitro and in a mouse skin infection model, providing a strategy for combating biofilm heterogeneity and biofilm-related infections.
