Project description:The historical focus of male lower urinary tract dysfunction (LUTD) has been benign prostatic enlargement and other aging-related processes. Little attention has been directed towards the influence of early life events on urinary physiology in advancing age. Here, we identify the intrauterine environment as a modifier of adult voiding function and risk factor for male LUTD. To model environmental chemical exposures, we exposed pregnant mice to the environmental contaminant 2,3,7,8 tetrachlorodibenzo-p-dioxin (TCDD, 1 µg/kg), coinciding with initiation of lower urinary tract development in male fetuses. Fetal TCDD exposure incites abnormal urodynamics in adult male mice, including increased urinary voiding frequency. TCDD also enhances adult prostate sensitivity to electrically evoked muscle contraction, suggesting increased autonomic tone. IUL TCDD exposure stably increases noradrenergic axon density beginning in the fetal period and persisting into adulthood. These changes are accompanied by increased abundance of a neurotrophin, Artemin (Artn), in the fetal prostate. This is the first evidence that intrauterine chemical exposures can reprogram prostate neuroanatomical development and drive prostatic smooth muscle hyperactivity in adulthood, which may create a susceptible phenotype for aging-related male lower urinary tract dysfunction.

Project description:Spinal cord injury (SCI) is a devastating clinical condition resulting in significant disabilities for affected individuals. Apart from local injury within the spinal cord, SCI patients develop a myriad of complications characterized by multi-organ dysfunction. Disorders, such as lung injury, cardiovascular disease, liver damage, kidney and urinary tract dysfunction, alterations in gut microbiome, neuropathic pain, depression and altered immune responses are common in SCI patients. Such whole body, systemic responses hinder the functional recovery and can be life-threatening in SCI population. Investigations of organ specific gene expression analyses can provide a better understanding of the injury response and identify molecular targets. Consequently, this can be used to develop treatment for SCI, promoting functional recovery and overall quality of life.

Project description:Sub-Saharan Africa represents 69% of the total number of individuals living with HIV infection worldwide and 72% of AIDS deaths globally. Pulmonary infection is a common and frequently fatal complication, though little is known regarding the lower airway microbiome composition of this population. Our objectives were to characterize the lower airway microbiome of Ugandan HIV-infected patients with pneumonia, to determine relationships with demographic, clinical, immunological, and microbiological variables and to compare the composition and predicted metagenome of these communities to a comparable cohort of patients in the US (San Francisco). Bronchoalveolar lavage samples from a cohort of 60 Ugandan HIV-infected patients with acute pneumonia were collected. Amplified 16S ribosomal RNA was profiled and aforementioned relationships examined. Ugandan airway microbiome composition and predicted metagenomic function were compared to US HIV-infected pneumonia patients. Among the most common bacterial pulmonary pathogens, Pseudomonas aeruginosa was most prevalent in the Ugandan cohort. Patients with a richer and more diverse airway microbiome exhibited lower bacterial burden, enrichment of members of the Lachnospiraceae and sulfur-reducing bacteria and reduced expression of TNF-alpha and matrix metalloproteinase-9. Compared to San Franciscan patients, Ugandan airway microbiome were significantly richer, and compositionally distinct with predicted metagenomes that encoded a multitude of distinct pathogenic pathways e.g secretion systems. Ugandan pneumonia-associated airway microbiome is compositionally and functionally distinct from those detected in comparable patients in developed countries, a feature which may contribute to adverse outcomes in this population. Project description:Detrusor underactivity (DUA) is defined as reduced detrusor contraction strength and/or duration, resulting in prolonged bladder emptying and/or a failure to achieve complete bladder emptying within a normal time span. DUA is a frustrating diagnosis for clinicians as well as patients since no effective pharmacological treatment is available. The prevalence of urodynamically confirmed DUA in elderly patients with lower urinary tract symptoms (LUTS) is known to be approximately 28% (40.2% in male and 13.3% in female), and the prevalence of DUA increases with age 65. Vascular endothelial damage (VED) also occurs during the human aging process and is an independent risk factor for atherosclerosis and hypertension. Pelvic arterial insufficiency, a common clinical problem in the elderly population, may lead to impaired lower urinary tract perfusion and have an important role in voiding dysfunction, such as DUA or detrusor overactivity (DO). Thus, to establish a reliable detrusor underactivity (DUA) rat model and to investigate the pathophysiology of chronic bladder ischemia (CBI) on voiding behavior and bladder function.
