Malignant peripheral nerve sheath tumours in neurofibromatosis 1.

Project description:BackgroundCross sectional studies have shown that 1-2% of patients with neurofibromatosis 1 (NF1) develop malignant peripheral nerve sheath tumours (MPNST). However, no population based longitudinal studies have assessed lifetime risk.MethodsNF1 patients with MPNST were ascertained from two sources for our north west England population of 4.1 million in the 13 year period 1984-1996: the North West Regional NF1 Register and review of notes of patients with MPNST in the North West Regional Cancer Registry.ResultsTwenty-one NF1 patients developed MPNST, equivalent to an annual incidence of 1.6 per 1000 and a lifetime risk of 8-13%. There were 37 patients with sporadic MPNST. The median age at diagnosis of MPNST in NF1 patients was 26 years, compared to 62 years in patients with sporadic MPNST (p<0.001). In Kaplan-Meier analyses, the five year survival from diagnosis was 21% for NF1 patients with MPNST, compared to 42% for sporadic cases of MPNST (p=0.09). One NF1 patient developed two separate MPNST in the radiation field of a previous optic glioma.ConclusionThe lifetime risk of MPNST in NF1 is much higher than previously estimated and warrants careful surveillance and a low threshold for investigation.

Project description:Investigate the genomic landscape of MPNST tumours. Cases where a diagnosis of MPNST had been considered in the differential diagnosis with a variable degree of confidence was included. The diagnosis of MPNST was excluded in 12/51 (49 cases had WES data) of the sequenced tumours following review of the pathology, new IHC, molecular tests and clinical information. Project description:The diagnostic differentiation of canine fibrosarcomas and peripheral nerve sheath tumors (PNST) is mainly based on their histopathologic phenotype. Histologic differentiation of these tumors can, however, be difficult and there is a lack of immunohistochemical markers to prove their histogenic origin. To identify possible PNST markers and to further investigate the histogenic origin of the two tumor types, we compared histologically defined canine fibrosarcomas and peripheral nerve sheath tumors by microarray analysis. Forty-five annotated gene products were significantly differentially expressed in both tumor types. Seven of these gene products, known to be specifically expressed in neuroectodermal tissues, showed higher expression in peripheral nerve sheath tumors: FMN2, KIF1B, GLI1, ROBO1, NMUR2, DOK4 and HMG20B. Furthermore, eight genes associated with carcinogenesis were up-regulated in fibrosarcomas: FHL2, PLAGL1, FNBP1L, BAG2, HK1, CSK and Cox5A. Comparison of the fibrosarcoma and PNST transcriptome therefore identified PNST phenotype-associated genes involved in neuroectodermal differentiation which may be potential PNST markers. Furthermore, the identified fibrosarcoma phenotype-associated genes may be potential markers to differentiate fibrosarcomas from other tumor types and may be involved in their pathogenesis. The aim of the present study was to examine the differences in global gene expression of histologically prototypic fibrosarcomas and PNST. To this end, complete transcriptome microarray analyses were used to quantify differences in gene expression activity in both tumor types.

Project description:No consensus treatment regime exists beyond surgery for malignant peripheral nerve sheath tumours (MPNST), and the purpose of the present study was to find new approaches to stratify patients with good and poor prognosis and to better guide therapeutic intervention for this aggressive soft tissue cancer. Genome-wide RNA expression was analysed for 30 MPNSTs and eight neurofibromas from Scandinavian patients, and the overall gene expression in these two groups were compared in order to identify prognostic markers.
