Drug Repurposing Using Molecular Network Analysis Identifies Janus Kinase as Targetable Driver in Necrobiosis Lipoidica

Project description:Drug repurposing is an attractive strategy for therapy development, particularly in rare diseases where traditional drug development approaches may be challenging due to high cost and small numbers of patients. Here we used a novel drug identification and repurposing pipeline to identify candidate targetable drivers of disease and corresponding therapies through application of causal reasoning using a combination of open-access resources and transcriptomics data. We optimized our approach on psoriasis as a disease model, demonstrating the ability to identify known and novel molecular drivers of psoriasis and link them to current and emerging therapies. Application of our approach to a cohort of tissue samples of necrobiosis lipoidica, an unrelated, rare, and to date molecularly poorly characterized cutaneous inflammatory disorder, identified a unique set of upstream regulators, particularly highlighting the role of IFNG and the JAK-STAT pathway as a likely driver of disease pathogenesis and linked it to JAK inhibitors as potential therapy. Analysis of an independent cohort of NL samples validated these findings with the overall agreement of drug matched upstream regulators above 96%. These data highlight utility of our approach in rare diseases and offer a novel opportunity for drug discovery in other rare diseases in dermatology and beyond.

Whole-genome sequencing and microarray analysis of ex vivo Plasmodium vivax reveal selective pressure on putative drug resistance genes

Project description:Plasmodium vivax causes 25-40% of malaria cases worldwide, yet research on this human malaria parasite has been neglected. Nevertheless, the recent publication of the P. vivax reference genome now allows genomics and systems biology approaches to be applied to this pathogen. We show here that whole genome analysis of the parasite can be achieved directly from ex vivo-isolated parasites, without the need for in vitro propagation. A single isolate of P. vivax obtained from a febrile patient with clinical malaria from Peru was subjected to whole genome sequencing (30X coverage). This analysis revealed over 18,261 single nucleotide polymorphisms (SNPs), 6,257 of which were further validated using a tiling microarray. Within core chromosomal genes we find that one SNP per every 985 bases of coding sequence distinguishes this recent Peruvian isolate, designated IQ07, from the reference Sal1 strain obtained in 1970. This full-genome sequence of a P. vivax isolate, the second overall and first of an uncultured patient isolate, shows that the same regions with low numbers of aligned sequencing reads are also highly variable by genomic microarray analysis. Finally, we show that the genes containing the largest ratio of nonsynonymous to synonymous SNPs encode two AP2 transcription factors and the P. vivax multidrug resistance-associated protein (PvMRP1), an ABC transporter shown to be associated with quinoline and antifolate tolerance in P. falciparum. This analysis provides a new data set for comparative analysis with important potential for identifying markers for global parasite diversity and drug resistance mapping studies. Changes in the erythroblast miRNA profile in the bone marrow of a Plasmodium vivax-infected patient

Project description:Human studies of Plasmodium vivax in the bone marrow are scarce. Here, we present a detailed characterization of bone marrow aspirates taken from a P. vivax patient with high parasitaemia on admission and 42 days after treatment. Analysis of miRNAs related to erythropoiesis revealed a distinct series of differentially expressed miRNAs during infection compared to at convalescence. These results suggest that parasites in the bone marrow affect erythropoiesis.

A single-cell liver atlas of relapsing human malaria [Bulk-seq]

Project description:Malaria-causing Plasmodium vivax parasites can linger in the human liver for weeks to years, and then reactivate to cause recurrent blood-stage infection. While an important target for malaria eradication, little is known about the molecular features of the replicative and non-replicative states of intracellular P. vivax parasites, or their human host-cell dependencies and the host responses to them. Here, we leverage a bioengineered human microliver platform to culture patient-derived P. vivax parasites in primary human hepatocytes and conduct transcriptional profiling. By coupling enrichment strategies with bulk and single-cell analyses, we captured both parasite and host transcripts in individual hepatocytes throughout the infection course. We define host- and state-dependent transcriptional signatures and identify previously unappreciated populations of replicative and non-replicative parasites, sharing features with sexual transmissive forms. We find that infection suppresses transcription of key hepatocyte function genes, and that P. vivax elicits an innate immune response that can be manipulated to control infection. Our work provides an extendible framework and resource for understanding host-parasite interactions and reveals new insights into the biology of P. vivax dormancy and transmission.
