Project description:As many as 10–30% of the nearly 750 million survivors of COVID-19 develop persistent symptoms that define the post-acute sequelae of COVID-19 (PASC) syndrome. To understand the molecular basis of this syndrome, we combined a machine learning based analysis of lung computed tomography (CT) with flow cytometry and single cell RNA-sequencing analysis of bronchoalveolar lavage fluid and nasal curettage samples in a cohort of thirty-five patients with respiratory symptoms and radiographic abnormalities more than 90 days after infection with COVID-19. CT images from patients with PASC revealed primarily fibrotic abnormalities involving 73 +/- 28% of the lung, most of which improved on subsequent imaging. The presence of profibrotic monocyte-derived alveolar macrophages in BAL fluid was significantly associated with increased levels of alveolar cytokines and fibrotic abnormalities on CT. Persistent infection with SARS-CoV-2 was identified in 6 patients and secondary bacterial or viral infections in two others. These findings suggest persistent alveolar inflammation characterized by the ongoing recruitment of monocyte derived alveolar macrophages is associated with fibrotic CT changes in some COVID-19 survivors with implications for diagnosis and therapy.

Plasma proteomic signature predicts who will get persistent symptoms following SARS-CoV-2 infection

Project description:The majority of those infected by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) during the first wave did not require hospitalisation. Most had a short-lived mild or asymptomatic infection, while others had symptoms that persisted for weeks or months. We performed a longitudinal targeted analysis of the plasma proteome of 156 healthcare workers (HCW) with and without lab confirmed SARS-CoV-2 infection. We hypothesized that the plasma proteome would reflect differences in the inflammatory response that linked to symptom severity and duration. We showed that perturbation of the proteome persisted for over six weeks, tracking symptom severity and antibody responses. Additionally, we identified a prognostic signature at the time of seroconversion that identified individuals likely to suffer from persistent symptoms related to SARS-CoV-2 infection.

Longitudinal Analysis of the Lung Proteome Reveals Persistent Repair Months after Mild to Moderate COVID-19

Project description:In order to assess the restoration of homeostasis in the lung proteome after COVID-19 infection, we performed bronchoalveolar lavage on 45 patients with mild to moderate disease at three phases (acute, recovery, convalescence) over a year. Changes in proteins were assessed using a multimodal approach. During the acute phase, inflamed and uninflamed phenotypes were characterized by the expression of tissue repair and host defense response molecules. With recovery, inflammatory and fibrogenic mediators declined and clinical symptoms abated. However, at nine months, quantified radiographic abnormalities had resolved in the majority of patients, and yet compared to healthy persons, all showed ongoing activation of cellular repair processes and depression of the renin-kallikrein kinin-coagulation and complement systems. This dissociation of prolonged reparative processes from symptom and radiographic resolution suggests that occult ongoing disruption of the lung proteome is underrecognized and may be relevant to recovery from other serious viral pneumonias.
