Corrected scoliosis, cholinesterase deficiency, and cesarean section: a case report.

ABSTRACT: We describe a patient with severe scoliosis for which corrective surgery was performed at the age of 12. During a previous caesarean section under general anaesthesia pseudocholinesterase deficiency was discovered. Ultrasound guided spinal anaesthesia was performed enabling a second caesarean section under loco-regional anaesthesia.

Project description:BackgroundApnoea and prolonged paralysis after succinylcholine administration is not uncommon occurrence in anaesthetic practice. It occurs due to inherited or acquired deficiency of butyrylcholinesterase.Case reportHere we report a case of succinylcholine apnoea for 2 h in a 5 years old girl who was anaesthetized for bronchoscopic extraction of a foreign body. She was subsequently kept on assisted ventilation. She recovered few minutes after I.V. atropine and naloxone. Laboratory investigation revealed low cholinesterase activity. Thus the girl was given 150 mL fresh frozen plasma. She has been discharged the next day after complete recovery.ConclusionAs the genetic analysis was not available to confirm the diagnosis of atypical variant of cholinesterase. The family was advised to submit serum samples for assessment of cholinesterase activity and avoid exposure to cholinesterase inhibitors. Moreover, clear instructions were given to the family so they can warn the anaesthetists in case any family member undergoes general anesthesia for any reason in the future. Furthermore, they must be strongly advised to avoid exposure to anticholinesterases as they might have heightened sensitivity to these agents. It should be emphasized that Naloxone and atropine could help speed up recovery in such cases.

Spontaneous complete migration of suture material after subcuticular continuous suture in cesarean section: a case report.

Project description:BackgroundSkin closure during cesarean section is often performed with subcuticular running sutures by using a nonabsorbable suture material. However, this material has the risk of incomplete removal after wound healing and can migrate to other sites in rare cases.Case presentationA 34-year-old Korean woman who had undergone a cesarean section 5 months prior presented with a fine, blue object visible through the skin on her left lower abdomen. No pain or any other signs of inflammation were observed. The foreign body was revealed to be 10-cm-long suture material that had migrated laterally approximately 15 cm in intradermal layer during the previous 5 months, without tangling of the entire length.ConclusionsSmall remnants of suture materials in the subcutaneous tissue are known to migrate toward the superficial layer. The mechanism of these migrations is often thought to be related to foreign body immune reaction or the force generated in wound contracture. Long-distance migration of relatively long suture materials, as in the present case, has not been reported yet. Such a steady tension in a uniform direction within a human tissue layer cannot be explained clearly by the previously described mechanisms. That migration might have occurred in superficial subcutaneous tissue layers through the horizontal flow or movement of those layers during the recovery process that have not been revealed yet.

Perioperative management of emergent cesarean section in a patient with peripartum cardiomyopathy and orthopnea: a case report.

Project description:Perioperative management of pregnant women with heart failure is difficult. Management of anesthesia in pregnant women is especially difficult because all of the currently available choices present challenges. We report a patient with peripartum cardiomyopathy (PPCM) who required an emergent cesarean section and discuss the possible tactics for managing anesthesia. A 40-year-old primipara with severe cardiac and respiratory failure required an emergent cesarean section at 39+1 gestational weeks. Her left ventricular ejection fraction was between 10% and 15%, and she had orthopnea. General anesthesia was planned after inserting sheaths for percutaneous cardiopulmonary support from the femoral artery and vein. However, when the patient was asked to lie down on the operation bed, she panicked and resisted because of labor pain and dyspnea. Therefore, anesthesia was induced instead of the initial plan. Finally, we successfully managed the anesthesia and delivered the newborn. There are no alternatives to general anesthesia in patients with PPCM presenting with orthopnea. Anesthesia induction in the supine position is impossible in such patients owing to dyspnea. Anesthesia should be started with light sedation in the sitting position, and ketamine or low-dose remifentanil may be an option to maintain maternal hemodynamics and prevent neonatal asphyxia.

Emergency cesarean section performed in a patient with confirmed severe acute respiratory syndrome Coronavirus-2 -a case report.

Project description:BackgroundSince the first case of severe acute respiratory syndrome Coronavirus-2 (SARS-CoV-2) occurred in Wuhan in December 2019, the virus has spread globally. The World Health Organization declared the virus outbreak a pandemic on March 11, 2020. On January 19, 2020, a 35-year-old woman who returned from China was confirmed as the first SARS-CoV-2 infected case in Korea. Since then, it has spread all over Korea.CaseWe report the first case of a SARS-CoV-2 positive woman delivering a baby through cesarean section at 37+6 weeks of pregnancy in the Republic of Korea.ConclusionsThis case suggested that negative pressure operating room, skillful medical team, and enhanced personal protective equipment including N95 masks, surgical cap, double gown, double gloves, shoe covers, and powered air-purifying respirator are required at the hospital for safe delivery in such a case.

Acute increase in a thyroid cyst during cesarean section under general anesthesia in a full-term parturient: a case report.

Project description:BackgroundPregnancy is associated with thyroid nodule formation and increased size. However, an acute increase in thyroid cysts during cesarean section is unusual.Case descriptionWe describe the case of a 23-year-old primiparous woman at 37 weeks of gestation, without a history of thyroid disease, who underwent a cesarean section under general anesthesia. When the baby was delivered after induction of general anesthesia, the patient's airway pressure increased by approximately 5-7 mmHg, and her blood pressure increased to 170/78 mmHg. After delivery of the baby, the mother's anterior region of the neck began to swell, and ultrasonography revealed a large cystic mass. Even after aspiration of approximately 120 mL of dark brown intra-cystic fluid, the neck swelled again. Airway and blood pressure decreased after delivery and remained within the normal range until the end of surgery. Computed tomography performed after recovery from anesthesia revealed an approximately 320 mL-sized hemorrhagic cyst in the left thyroid gland with right tracheal deviation. Despite repeated aspirations and two alcohol ablations, the cyst's size increased rapidly, and the patient underwent radiofrequency ablation.ConclusionsThis case indicates that rapid increases in intrathoracic and blood pressure could precipitate a rapid increase in pre-existing thyroid cysts in a parturient during delivery.

Two-dimensional power Doppler-three-dimensional ultrasound imaging of a cesarean section dehiscence with utero-peritoneal fistula: a case report.

Project description:INTRODUCTION: An imaging diagnosis after an iterative cesarean delivery is reviewed demonstrating a fine ultrasound-pathologic correlation. CASE PRESENTATION: A 33-year-old woman (G3, P3) presented referring intense dysmenorrhea and intermenstrual spotting since her third cesarean delivery, 1 year before. A cesarean section dehiscence with utero-peritoneal fistula was diagnosed by transvaginal ultrasound. CONCLUSION: We can conclude that transvaginal two-dimensional power Doppler and three-dimensional ultrasound are highly accurate in detecting cesarean section dehiscence and uterine fistula.

Project description:Cesarean scar ectopic pregnancies are a rare form of extrauterine pregnancies, yet their incidence is increasing given the rise in cesarean deliveries. Similar to other ectopic pregnancies, cesarean scar ectopic pregnancies pose a great risk for maternal hemorrhage and ultimately maternal mortality. This study presents the case of a cesarean scar ectopic pregnancy in a patient with 3 prior cesarean deliveries. Here, we highlight the importance of early diagnosis and treatment of cesarean scar ectopic pregnancies.

Complete resolution of myoclonus-like involuntary movements under subarachnoid block after midazolam administration in a patient undergoing cesarean section: a case report.

Project description:Involuntary movement during and after neuraxial anesthesia, such as spinal and epidural anesthesia, is rarely observed. In this report, we describe a case of myoclonus-like involuntary movement of the upper extremities in a patient undergoing a planned repeat cesarean section under spinal anesthesia with bupivacaine that completely subsided after administration of 2 mg of midazolam. The myoclonus-like movement did not recur or cause any apparent neurological side effects.

Robinsoniella peoriensis infection following surgery for scoliosis: a case report.

Project description:INTRODUCTION: Robinsoniella peoriensis was recently identified as a Gram-positive, spore-forming, anaerobic bacillus originally isolated from swine manure storage pits. Seven isolates have been subsequently reported from human sources. CASE PRESENTATION: We report the case of an infection caused by R. peoriensis in a 45-year-old Caucasian woman after posterior instrumentation correction of idiopathic thoracolumbar scoliosis. The identification was made by culture of samples inoculated onto blood agar and chocolate agar and was confirmed by 16?S ribosomal ribonucleic acid gene sequencing. CONCLUSIONS: We discuss similar cases suggesting that R. peoriensis is responsible for health care-associated infections with the colonic flora as a potential source of infection.
