Fatal subacute myocarditis associated with human bocavirus 2 in a 13-month-old child.

ABSTRACT: Human bocavirus has rarely been incriminated in fatal or life-threatening respiratory infections. We report a case of fatal disseminated infection with subacute lymphocytic myocarditis in a 13-month-old child. The human bocavirus 2 genome was detected by PCR analysis in nasal swab, plasma, urine, ascitic fluid, and mesenteric node, skeletal muscle, and lung tissue specimens.

Project description:Trichodysplasia spinulosa-associated polyomavirus (TSV) was identified in a seven-month-old girl with myocarditis. The number of TSV genomes detected was higher in the heart than in the other organs. The full-length TSV genome was cloned from the heart. This suggests a possible role of TSV infection in the pathogenesis of myocarditis in infants.

Fatal myocarditis associated with acute parvovirus B19 and human herpesvirus 6 coinfection.

Project description:We report on the case of a healthy young boy who developed a fulminant myocarditis due to acute coinfection with erythrovirus (parvovirus B19) and human herpesvirus 6 (HHV-6) in the absence of an antiviral immune response. We suggest that the HHV-6-induced immunosuppression enhanced dissemination of parvovirus B19, which led to fatal myocarditis.

Marseillevirus adenitis in an 11-month-old child.

Project description:A Marseillevirus (giant virus of amoeba) has been found in the blood and stool samples of individuals who otherwise appear to be healthy. During an attempt to define a serological cutoff for Marseillevirus by enzyme-linked immunosorbent assay (ELISA) in children, we serendipitously detected high antibody responses to Marseillevirus in an 11-month-old boy suffering from adenitis. Marseillevirus DNA was then found in his blood using PCR and with a unique sequence. We identified Marseillevirus in a lymph node using fluorescence in situ hybridization (FISH) and immunohistochemistry, and the lymph node was removed surgically. The child was declared to be cured 1 year later. We conclude that adenitis during early childhood may be caused by Marseillevirus.

Fatal Myocarditis Associated With HHV-6 Following Immunosuppression in Two Children.

Project description:Fatal myocarditis is a rare complication in immunosuppressed children. Recent reports have linked human herpesvirus 6 (HHV-6) infection, typically a benign infection in childhood, with myocarditis. HHV-6 can reactivate during periods of immunosuppression. Here, we report 2 cases in which children were immunosuppressed, one for treatment of Evans syndrome and the other post hematopoietic stem cell transplantation, who developed rapid and fatal HHV-6-associated myocarditis. These cases suggest that HHV-6 infection should be considered as an etiology of myocarditis in immunosuppressed patients regardless of correlating blood levels. Early treatment of HHV-6 in patients with myocarditis could improve morbidity and mortality.

Fatal systemic toxoplasmosis in a 3-month-old young tibetan goat (Capra hircus).

Project description:BACKGROUND:Toxoplasmosis is one of the most common parasitic infections in both humans and animals. It is a frequent cause of abortion and stillbirth in intermediate hosts, especially sheep and goats but rarely causes fatal clinical form in adult animals. CASE PRESENTATION:In contrast, the study reports an unusual fatal case of toxoplasmosis in a young goat naturally infected with type II strain of Toxoplasma gondii. A three-month-old female goat was presented with dyspnea and died few days later. Grossly, lungs were firm, edematous and mottled with disseminated whitish areas. Generalized lymphadenopathy was found. The histopathological examination showed necrotic interstitial bronchopneumonia and necrotizing lymphadenitis with intralesional free and clustered within macrophages tachyzoites of T. gondii. DNA extracted from lungs and lymph nodes was positive for T. gondii by a fast qPCR. PCR-RFLP analysis and sequencing of GRA6 gene showed that the isolated strains belonged to type II genotype. CONCLUSIONS:This is an unusual report of acute systemic toxoplasmosis caused by the type II strain of T. gondii with a fatal outcome in a young goat.

Fatal outcome of SARS-CoV-2 infection (B1.1.7) in a 4-year-old child.

Project description:This case report highlights details of a case of critical acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) with B1.1.7 variant in a 4-year-old girl who died due to pneumonia and pulmonary hemorrhage. The girl was referred to our University ECMO Center from another University hospital for veno-arterial extracorporeal membrane oxygenation (VA-ECMO). In the clinical course, superinfection with Pseudomonas aeruginosa was detected. Virological evidence of herpes simplex sepsis was also obtained in blood samples on her day of death. Transcription polymerase chain reaction (PCR) confirmed SARS-CoV-2 infection in lung tissue. Postmortem computed tomography showed pulmonary hemorrhage with inhomogeneous density values in both lungs. Lung tissue showed no ventilated areas. Autopsy revealed a massively congested lung with evidence of acute respiratory distress syndrome (ARDS) and pneumonia with multiple abscesses. Histopathology showed a mixture of diffuse alveolar injury with hyaline membranes, massive hemorrhage, and bronchopneumonia with multiple granulocytic abscesses. Cardiac examination revealed pericarditis. Suspicion of myocarditis or myocardial infarction could not be confirmed microscopically. To our knowledge, this is the first autopsy-based case report of the death of a previously healthy child due to the new variant B 1.1.7 in Germany.

We report an asymptomatic 17-month-old who passed eggs and an adult Macracanthorhynchus ingens worm, indicating parasite maturation and reproduction. Macracanthorhynchus ingens may have a greater capacity to mature in humans versus Macracanthorhynchus hirudinaceus.

Symbolic labeling in 5-month-old human infants.

Project description:Humans' ability to create and manipulate symbolic structures far exceeds that of other animals. We hypothesized that this ability rests on an early capacity to use arbitrary signs to represent any mental representation, even as abstract as an algebraic rule. In three experiments, we collected high-density EEG recordings while 150 5-month-old infants were presented with speech triplets characterized by their abstract syllabic structure-the location of syllable repetition-which predicted a following arbitrary label (e.g., ABA words were followed by a fish picture, AAB words by a lion). After a brief learning phase, EEG responses to novel words revealed that infants built expectations about the upcoming label based on the triplet structure and were surprised when it happened to be incongruent. Preverbal infants were thus able to recode the incoming triplets into abstract mental variables to which arbitrary labels were flexibly assigned. Importantly, infants also generalized to novel trials in which the pairing order was reversed (with the label preceding the auditory structure). Beyond conditioned associations, infants instantly inferred a bidirectional mapping between the abstract structures and the following label, a foundational operation for any symbolic system.

Focused attention predicts visual working memory performance in 13-month-old infants: A pupillometric study.

Project description:Attention turns looking, into seeing. Yet, little developmental research has examined the interface of attention and visual working memory (VWM), where what is seen is maintained for use in ongoing visual tasks. Using the task-evoked pupil response - a sensitive, real-time, involuntary measure of focused attention that has been shown to correlate with VWM performance in adults and older children - we examined the relationship between focused attention and VWM in 13-month-olds. We used a Delayed Match Retrieval paradigm, to test infants' VWM for object-location bindings - what went where - while recording anticipatory gaze responses and pupil dilation. We found that infants with greater focused attention during memory encoding showed significantly better memory performance. As well, trials that ended in a correct response had significantly greater pupil response during memory encoding than incorrect trials. Taken together, this shows that pupillometry can be used as a measure of focused attention in infants, and a means to identify those individuals, or moments, where cognitive effort is maximized.

Project description:Introductionand importance: A mature mediastinal teratoma is a rare type of tumor that can contain fully developed tissues and it is usually considered benign, more common in female and may require surgical removal. The most common locations for teratomas are the tailbone (coccyx), ovaries, and testicles, but in this case, the teratoma was located in the mediastinum.Case presentationFurthermore, intestinal malrotation could potentially lead the patient to gastrointestinal (GI) obstruction and PO intolerance. Here, we present a 2-month-old patient with considerable manifestation of GI intolerance due to intestinal malrotaion accompanied by mature mediastinal teratoma.Clinical discussionAn evaluation of malrotation should be part of every upper GI tract examination in pediatric patients, particularly newborns and infants.ConclusionFinally, in this case, surgical intervention could enhance both malrotation and mediastinal teratoma.
