A Medial Meniscal Root Pullout Repair With the Use of a Tibial Tunnel Suturing Technique.

ABSTRACT: A meniscal root tear is one of the common knee injuries that can lead to degenerative changes in the knee joint. Meniscal root repairs can restore proper biomechanics of the knee joint. We have developed a suturing technique that uses a tibial tunnel for a pullout suture medial meniscal root repair. This is a straightforward technique that helps to promote simple suturing of the medial meniscal root, avoid iatrogenic injuries to the articular cartilage, and produce an additional working portal during a meniscal root repair.

Project description:Meniscal root tears are increasingly recognized as an important pathology. Failure to recognize this pathology could lead to early onset osteoarthritis of the ipsilateral knee joint compartment similar to a total meniscectomy. Therefore, surgical treatment is necessary to restore meniscal function and to normalize contact pressures, when there is joint overload and sufficient remaining articular cartilage. This article details our anatomic posterior root repair procedure using a transtibial double tunnel pullout technique.

Project description:The menisci are important structures within the knee and play a critical role in maintaining proper stability, load distribution, and joint lubrication. Injury to these structures can significantly alter the complex biomechanics of the knee and thus affect the health and longevity of the joint. Meniscal root tears are increasingly recognized as an important pathologic condition that results in a nonfunctional meniscus if not properly repaired. Whereas early treatment of meniscal tears traditionally focused on removal of the injured tissue, recent attention on the long-term consequences of partial or total meniscectomy has led to increased attempts at meniscal repair whenever possible. This article details our anatomic anterior root repair procedure using a transtibial double-tunnel pullout technique.

Medial Meniscus Posterior Root Tear Repair Using a 2-Simple-Suture Pullout Technique.

Project description:Medial meniscus posterior root tear is one of the underestimated knee injuries in terms of incidence. Despite its grave sequelae, using simple but effective technique can maintain the native knee joint longevity. In the current note, a 2-simple-suture pullout technique was used to effectively reduce the meniscus posterior root to its anatomic position. The success of the technique depended on proper tool selection as well as tibial tunnel direction that allowed easier root suturing and better suture tensioning, without inducing any iatrogenic articular cartilage injury or meniscal tissue loss. Using anterior knee arthroscopy portals, anterolateral as a viewing portal and anteromedial as a working portal, a 7-mm tibial tunnel starting at Gerdy tubercle and ending at the medial meniscus posterior root bed was created. The 2 simple sutures were retrieved through the tunnel and tensioned and secured over a 12-mm-diameter washer at the tibial tunnel outer orifice. Anatomic reduction of the medial meniscus posterior root tear was confirmed arthroscopically intraoperatively and radiologically by postoperative magnetic resonance imaging.

Association between postoperative meniscal extrusion and clinical outcomes of the pullout fixation technique for medial meniscus posterior root tear.

Project description:Objective:To evaluate the correlation between meniscal extrusion in the coronal plane with clinical and radiological outcomes at a minimum of 1 year after transtibial pullout fixation for medial meniscus posterior root tear (MMPRT). Methods:Data from 30 patients (five males, 25 females) with MMPRT who underwent arthroscopic root fixation by the transtibial pullout technique between 2011 and January 2015 were retrospectively analyzed. The mean patient age was 60.7 ± 8.3years. The median time between injury and surgery was 3.8 months (range, 0.5-15 months). Medial meniscus radial displacement was assessed on magnetic resonance imaging performed preoperatively and at 1 year after fixation. The patients were divided into two groups based on the presence on magnetic resonance imaging of decreased meniscal extrusion (group A) or increased meniscal extrusion (group B) at 1 year postoperatively compared with preoperatively. The two groups were compared regarding factors including age at surgery, sex, body mass index, time between injury and surgery, and the Lysholm score and Kellgren-Lawrence (K-L) grade preoperatively and at 1 year postoperatively. Results:There were seven knees in group A, and 23 in group B. Both groups had a significantly improved Lysholm score postoperatively compared with preoperatively (p < 0.001). Although the postoperative K-L grade was significantly worse than the preoperative K-L grade in group B (p < 0.001), the postoperative K-L grade did not worsen postoperatively in group A. Conclusion:The K-L grade did not worsen postoperatively in the group with decreased meniscal extrusion at 1 year after transtibial pullout fixation for MMPRT compared with preoperatively.

Stay Ipsilateral: An Analysis of Tibial Tunnel Distance Between Cruciate Ligament Reconstruction and Posterior Meniscal Root Repair.

Project description:PurposeTo establish mean distance or identify intersection between tibial tunnels for posterior meniscal root repair in the setting of anterior cruciate ligament (ACL) or posterior cruciate ligament (PCL) reconstruction.MethodsTwelve cadaver knees and 12 solid foam synthetic tibiae were used. ACL and PCL tunnels were drilled for single-bundle reconstruction, and both medial and lateral posterior root repair tunnels were drilled. Specimens underwent computed tomography scanning and shortest distances between tunnels in all planes were measured by 2 readers. Distances were compared between groups using a t-test.ResultsIn ACL reconstruction, the medial meniscal root tunnel was not significantly closer to the cruciate tunnel when drilled from either medial or lateral side (P = .333). The lateral meniscal root tunnel was significantly closer when drilled from medial compared to lateral side (P < .001). In PCL reconstruction, both medial (P = .037) and lateral (P = .028) meniscal root tunnels were significantly closer to the PCL tunnel when drilled from the contralateral side of the tibia.ConclusionThis study demonstrates that posterior meniscal root repair tunnels are often placed within a few millimeters and can even intersect cruciate ligament reconstruction tunnels in the proximal tibia.Clinical relevanceThe information in this study may assist surgeons in planning for cruciate ligament reconstruction with concomitant posterior meniscal root repair.

Meniscal Root Repair With Mini-Open Medial Collateral Ligament Release.

Project description:Meniscal root repair is becoming more common with increasing knowledge regarding the function of the meniscal root and its role in long-term joint preservation, as well as improved surgical techniques and instrumentation. Despite this, adequate visualization of the posterior horn of the medial meniscus may be challenging, particularly in those with tight medial compartments. Instrument handling and attempt to visualize and repair the meniscal tear may lead to iatrogenic chondral injuries in these instances. We describe our preferred technique for improved visualization via mini-open release of the distal medial collateral ligament that allows for increased joint distension, increased access to the posterior horn of the medial meniscus, and the ability to repair the released medial collateral ligament.

Arthroscopic Single-Tunnel Pullout Suture Fixation for Tibial Eminence Avulsion Fracture.

Project description:Various arthroscopic techniques have been devised for fixation of tibial eminence avulsions, namely percutaneous K-wires, arthroscopy-guided screw fixation, staples, TightRope (Arthrex)-suture button fixation, and transosseous suture fixation. Such techniques provide well-pronounced advantages including less postoperative pain, a reduced hospital stay, and minimal scar with resultant earlier and more compliant rehabilitation. As for transosseous suture fixation, the standard technique comprises the creation of 2 tibial tunnels exiting on both sides of the footprint of the avulsion fracture using an anterior cruciate ligament tibial guide with the angle set at 45°. Our technique entails the creation of a single tibial tunnel directed from the proximal anteromedial tibia to the center of the tibial eminence. The technique uses Ethibond suture (No. 5) and/or FiberWire suture (Arthrex) to fix the tibial eminence by pulling the anterior cruciate ligament fibers and tightening the pullout suture at the tibial exit of the tunnel with a 4-hole button. This modified single-tunnel pullout suture technique is an appealing option that has proved to be effective and economical with a shorter operative time. Moreover, it provides a less invasive option for skeletally immature patients.

Transosseous Medial Meniscal Root Repair Using a Modified Mason-Allen Suture Configuration.

Project description:Medial meniscal tears are among the most common injuries to the knee joint. Loss of the meniscus has been linked to increased contact pressures on the adjacent articular cartilage and progression of degenerative changes in the knee. A subset of tears known as "root tears" involves the insertion of the posterior horn of the meniscus to the bone. Arthroscopic partial meniscectomy for root tears led to undesirable outcomes, which prompted surgeons to explore restorative procedures. Multiple repair techniques have been presented with an emphasis placed on initial secure fixation and stimulation of potential healing. We present an arthroscopic-assisted technique for medial meniscal root repair with these goals in mind.

Full-Thickness Radial Medial Meniscal Tear: Fixation With Inside-Out Technique With Tibial Knotless Suture Anchors.

Project description:Complete radial tears of the meniscus render the entirety of the meniscus functionally incompetent (known as an ameniscal state); therefore, attempts at repair are essential. Although various techniques have been described, repair failures continue to frequently occur, especially with the medial meniscus. Inside-out repair and anchoring of the preserved meniscus to both the capsule and tibia may offer the advantage of a more robust repair. The objective of this Technical Note is to describe a method of repair for complete radial tears of the medial meniscus using a combination of inside-out sutures and secondary reinforcement to the tibia using all-suture knotless anchors.

Augmentation of the Pullout Repair of a Medial Meniscus Posterior Root Tear by Arthroscopic Centralization.

Project description:The meniscus roots are critical for meniscus function in preserving correct knee kinematics and avoiding meniscus extrusion and, consequently, in the progression of osteoarthritis. Several techniques exist for medial meniscus posterior root tear repair; however, current surgical techniques have been proved to fail to reduce meniscus extrusion, which has been shown to be associated with development of osteoarthritis, although significant improvements in the postoperative clinical findings have been achieved. This Technical Note describes an arthroscopic technique for the medial meniscus posterior root tear in which a pullout repair is augmented by a centralization technique to restore and maintain the medial meniscus function by efficiently reducing meniscus extrusion.
