Project description:Families are impacted by a variety of risk and protective factors for maltreatment at multiple levels of the social ecology. Individual- and neighborhood-level poverty has consistently been shown to be associated with higher risk for child abuse and neglect. The current study sought to understand the ways in which individual- and neighborhood-level risk and protective factors affect physical child abuse and child neglect and whether these factors differed for families based on their individual poverty status. Specifically, we used a three-level hierarchical linear model (families nested within census tracts and nested within cities) to estimate the relationships between physical child abuse and child neglect and neighborhood structural factors, neighborhood processes, and individual characteristics. We compared these relationships between lower and higher income families in a sample of approximately 3,000 families from 50 cities in the State of California. We found that neighborhood-level disadvantage was especially detrimental for families in poverty and that neighborhood-level protective processes (social) were not associated with physical child abuse and child neglect for impoverished families, but that they had a protective effect for higher income families.

Project description:BackgroundUnderstanding factors that contribute to parents' use of physical and psychological parent-child aggression (PCA) is critical in efforts to mitigate child maltreatment.ObjectiveExtant research has not adequately distinguished risk factors that may differ by race.Participants and methodsThe present study investigated potential racial differences in worry, approval of PCA, justification for PCA use, negative child intent attributions, and discrimination experiences in relation to child abuse risk and physical and psychological PCA use in a sample of 292 Black (44.9 %) and White mothers.ResultsAs hypothesized, compared to White mothers, Black mothers demonstrated higher child abuse risk and reported more PCA use, stronger approval for using PCA, and more justification of their PCA to teach children obedience. Although Black mothers reported more discipline-relevant worry as well as more experience of discrimination, White mothers' lower trait worry related to their greater approval of PCA for discipline, which indirectly related to their abuse risk. Contrary to expectations, perceptions of greater discrimination were related to White mothers' increased child abuse risk, approval of PCA, and justification for PCA because of anger and to teach obedience-findings not observed for Black mothers.ConclusionsThe current results underscore the need for additional research on the role of discrimination and the cultural context of parenting and highlight the importance of explicitly testing racial differences to develop more culturally informed abuse prevention approaches.

Project description:Rates of substantiated child abuse and neglect vary significantly across counties. Despite strong cross-sectional support for links between social-contextual characteristics and abuse and neglect, few longitudinal studies have tested relations between these risk factors and substantiated rates of abuse/neglect. The goal of this study was to identify county-level socioeconomic and crime factors associated with substantiated abuse/neglect rates over 13 years (2004-2016). Annual county-level data for Tennessee, obtained from the KIDS COUNT Data Center, included rates of substantiated child abuse and neglect, children's race and ethnicity, births to unmarried women, teen birth rate, children in families receiving Supplemental Nutrition Assistance Program (SNAP) benefits, and children in families receiving Temporary Assistance for Needy Families. Annual county-level crime report data, obtained from the Tennessee Incident Based Reporting System, included sexual offenses, non-sexual assaults, stalking incidents, thefts, property damage, and drug-related offenses. Bayesian spatio-temporal models indicated that substantiated child abuse and neglect rates were independently and positively associated with teen birth rates, percentages of births to unmarried mothers, drug-related offenses, and percentages of children receiving SNAP benefits. In contrast, substantiated child abuse and neglect rates were negatively associated with percentages of African-American youth. The findings highlighted distinct demographic, socioeconomic, and crime factors associated with substantiated child abuse and neglect rates and have the potential to enhance identification of high-risk counties that could benefit from targeted abuse and neglect prevention efforts.

Project description:ObjectiveThis study investigated associations between the co-existence of multiple types of work-related psychosocial and physical risk factors, and (1) obesity; (2) smoking; and (3) leisure-time physical inactivity. It also aimed to identify sociodemographic characteristics related to clustering of work-related risk factors and lifestyle factors.MethodsCross-sectional data on work-related risk factors (e.g., decision authority and repetitive movements) and lifestyle was measured using a standardized questionnaire among 52,563 Dutch workers in health care, services, manufacturing and public sector. Multiple-adjusted logistic regression models assessed associations between the co-existence of multiple types of psychosocial and physical risk factors and lifestyle factors. Additionally, logistic regression models related age, gender and educational level to clustering of risk factors and lifestyle factors.ResultsThe co-existence of multiple types of work-related psychosocial risk factors was associated with higher odds of smoking and being physically inactive. For example, workers exposed to three psychosocial risk factors had a 1.55 times higher odds of being physically inactive (95%CI: 1.42-1.70) compared to unexposed workers. A higher number of physical risk factors was also significantly associated with higher odds of smoking and obesity. The co-existence of multiple types of physical risk factors was not associated with higher odds of physical inactivity. Clustering of work-related risk factors and at least one unhealthy lifestyle factor occurred in particular among workers with low educational level.ConclusionsResults imply that interventions are needed that focus on workers with a low educational level and address work-related physical and psychosocial risk factors as well as lifestyle.

Project description:Child sexual abuse (CSA) is widely recognized as a global public health problem with negative consequences for victims, their families, and society. The child's testimony is essential to the case outcome, given the frequent absence of physical or biological evidence of the abusive acts. Thus, the child forensic interview plays a decisive role in criminal investigation. The present scoping review aims to identify and describe the judicial procedures for collecting CSA victims' testimony using an evidence-based approach and a structured methodology. The review followed Preferred Reporting Items of Systematic Reviews and Meta-Analysis-Scoping Review guidelines. Studies were identified through manual reference checking and in four electronic databases: PsycARTICLES, PubMed, SCOPUS, and Web of Science. In all, 146 studies were identified according to the defined inclusion criteria, that is, empirical studies identifying judicial procedures to collect CSA victims' testimony, published in English or Portuguese. In total, 30 different forensic interview procedures to collect the child victim's testimony were found. The National Institute for Child Health and Human Development investigative interview protocol was the most frequently mentioned. Despite the variety of protocols, it was possible to conclude that they have a similar general structure. This review also identified gaps in interviewing practices with CSA victims. The scoping review corroborates the importance of forensic interviews with CSA victims, stating its implications for criminal investigation, the legal system, and the child's recovery process.

Project description:BackgroundThe Child Abuse Blame Scale - Physical Abuse (CABS-PA) was translated into Japanese and its subscale items modified by the authors according to the Japanese cultural context. The aim of the current study was to investigate the appropriateness, reliability, and clinical applicability of the CABS-PA Japanese version (CABS-PA-J). Modifications were made to enable the determination of child abuse recognition in a Japanese cultural setting and early clinical intervention in child abuse cases.MethodsThe CABS-PA text was translated into Japanese, then back translated. The appropriateness of scale item translations was verified based on e-mail discussions with the original CABS-PA author. Exploratory and confirmatory factor analyses were performed to examine the validity of CABS-PA-J responses and to confirm the validity of factor structure. Criterion-related validity was also confirmed. The Japanese scale was used to examine the characteristic differences between mothers of premature infants (< 1500 g) and those of other infants (>or= 1500 g).ResultsExploratory and confirmatory factor analyses found the factor structure to be similar between the original scale and the translated CABS-PA-J, suggesting adequate factor validity. There was a statistically significant correlation between social support from a spouse or third party and the abuse score on a subscale, partially demonstrating criterion-referenced validity. Similarities and differences were found in the stress reactions of the mothers of premature infants (< 1500 g) and those of other infants (>or= 1500 g).ConclusionCABS-PA-J was shown to be appropriate and reliable. It is an effective tool for determining the recognition of child abuse among Japanese mothers.

Project description:Missed cases of child physical abuse (CPA) persist despite known risk factors. Prior studies have not evaluated missed medical appointments as a risk factor for CPA. The objective of this study was to determine if an association exists between missed appointments and hospitalization for CPA. We conducted a 20-year, single health system, retrospective chart review of hospitalized patients ≤36 months of age meeting International Classification of Diseases (ICD) 9/10 criteria for CPA with ≥1 scheduled appointment in our system prior to their admission. Cases were categorized as definite CPA, high likelihood, or no concern for CPA/unable to be determined. Cases identified as definite or high likelihood of CPA were matched (5:1) with controls based on age, distance to primary care provider's (PCP's) office, sex, prior hospitalization, and race. Missed appointments were compared between cases (n = 146) and controls (n = 730). A significant difference was identified between cases and controls (26 % vs 9 %, p < 0.001) for the median proportion of missed appointments. After adjusting for matched and significant covariates, there was a 3 % increase in a patient's odds of admission for CPA for every 1 % increase in missed appointments. We found an association between missed appointments and future admission for CPA. This finding has potential to assist clinicians with CPA risk stratification and future child abuse research. Limitations include single healthcare system, ICD criteria determined by research team, and narrow definition of definite CPA.

Project description:The term "hyperscanning" refers to the simultaneous recording of multiple individuals' brain activity. As a methodology, hyperscanning allows the investigation of brain-to-brain synchrony. Despite being a promising technique, there is a limited number of publicly available functional Near-infrared Spectroscopy (fNIRS) hyperscanning recordings. In this paper, we report a dataset of fNIRS recordings from the prefrontal cortical (PFC) activity of 33 mother-child dyads and 29 father-child dyads. Data was recorded while the parent-child dyads participated in an experiment with two sessions: a passive video attention task and a free play session. Dyadic metadata, parental psychological traits, behavioural annotations of the play sessions and information about the video stimuli complementing the dataset of fNIRS signals are described. The dataset presented here can be used to design, implement, and test novel fNIRS analysis techniques, new hyperscanning analysis tools, as well as investigate the PFC activity in participants of different ages when they engage in passive viewing tasks and active interactive tasks.

Project description:Improved prediction of physical child abuse could aid in developing preventive measures. Parental physical disease has been tested previously as a predictor of documented physical child abuse but in broad categories and with differing results. No prior studies have tested clinically recognizable categories of parental disease in a high-powered dataset. Using Danish registries, data on children and their parents from the years 1997-2018 were used to explore several parental physical disease categories' associations with documented physical child abuse. For each disease category, survival analysis using pseudovalues was applied. When a parent of a child was diagnosed or received medication that qualified for a category, this family and five comparison families not in this disease category were included, creating separate cohorts for each category of disease. Multiple analyses used samples drawn from 2,705,770 children. Estimates were produced for 32 categories of physical diseases. Using Bonferroni-corrected confidence intervals (CIc), ischemic heart disease showed a relative risk (RR) of 1.44 (CIc 1.13-1.84); peripheral artery occlusive disease, RR 1.39 (CIc 1.01-1.90); stroke, RR 1.19 (1.01-1.41); chronic pulmonary disease, RR 1.33 (CIc 1.18-1.51); ulcer/chronic gastritis, RR 1.27 (CIc 1.08-1.49); painful condition, 1.17 (CIc 1.00-1.37); epilepsy, RR 1.24 (CIc 1.00-1.52); and unspecific somatic symptoms, RR 1.37 (CIc 1.21-1.55). Unspecific somatic symptoms were present in 71.87% of families at some point during the study period.ConclusionMost parental physical disease categories did not show statistically significant associations, but some showed predictive ability. Further research is needed to explore preventive potential.What is known• Few and broad categories of parental physical disease have been examined as risk factors for severe physical child abuse; no prior study has used several categories as predictors.What is new• Unspecific symptoms, ischemic heart disease, peripheral artery occlusive disease, stroke, chronic pulmonary disease, stomach ulcer/chronic gastritis, painful condition, and epilepsy all showed to be potential predictors, with unspecific symptoms being the most prevalent.

Project description:BackgroundChild abuse is highly prevalent and associated with increased risk for a range of health problems, including cancer, cardiovascular disease, diabetes, psychiatric disorders, and other health problems. Little is currently known about the mechanism by which early adversity confers risk for health problems later in life.PurposeTo determine if there are epigenetic differences associated with child maltreatment that may help explain association between adverse childhood experiences and later health problems.MethodsAs part of a study examining genetic and environmental factors associated with depression, saliva DNA specimens were collected on 96 maltreated children removed from their parents due to abuse or neglect and 96 demographically matched control children between 2003 and 2010. In 2011, the Illumina 450K BeadChip was used on stored DNA specimens and analyzed to examine whole-genome methylation differences between maltreated and control children.ResultsAfter controlling for multiple comparisons, maltreated and control children had significantly different methylation values at 2868 CpG sites (p<5.0 × 10(-7), all sites; average methylation difference per site=17%; range=1%-62%). The gene set contained numerous markers of diseases and biological processes related to the health problems associated with early childhood adversity.ConclusionsAlthough replication is required, this study suggests that epigenetic mechanisms may be associated with risk for health problems later in life in maltreated children. This study lays the groundwork for future studies examining health and methylation measures to further characterize the role of epigenetic mechanisms in conferring risk for medical problems in individuals with histories of early adversity.
