How Satisfied Are Patients When Their Choice of Funded Glucose Meter Is Restricted to a Single Brand?

ABSTRACT:

Background

Many governments and insurers are driving down the cost of medical devices, including glucose meters, by the central management of purchasing decisions. We report patients' responses to an "enforced" change in brand of glucose meter, one year after the introduction of a national sole supplier arrangement for funded glucose meters and strips.

Method

Specialist diabetes clinic attendees from two geographical locations completed a questionnaire one year after the final meter changeover date. In the first location, consecutive patients were asked to complete a glucose meter satisfaction questionnaire during their clinic visit. In the second location, this questionnaire was mailed to clinic attendees. Responses to open questions were analyzed thematically.

Results

Response rates were 85% and 31% from the first and second locations, respectively and 378 questionnaires were suitable for analysis, 309 from the first and 69 from the second location. Insulin users composed 90% of participants. Results from the two locations were broadly similar. Most participants adapted well to the changeover, however 36% reported ongoing dissatisfaction with their "new" meter. The commonest concern, expressed by 23% of participants, related to meter accuracy and precision.

Conclusions

One year after glucose meter changeover, a third of participants expressed dissatisfaction with their meter, with many participants describing a failure to adapt to the sole supplier arrangement. Providing a choice of meters and strips, ideally from two or more brands that have demonstrable differences in technical and ergonomic features, is likely to produce higher overall patient satisfaction than is a sole supplier arrangement.

Project description:BackgroundDiabetes is a global epidemic affecting approximately 30 million people in the United States. The World Health Organization recommends using technology and telecommunications to improve health care delivery and disease management. The Livongo for Diabetes Program offers a remote monitoring technology with Certified Diabetes Educator outreach.ObjectiveThe purpose of this study was to examine health outcomes measured by changes in HbA1c, in time in target blood glucose range, and in depression symptoms for patients enrolled in a remote digital diabetes management program in a Diabetes Center of Excellence setting.MethodsThe impact of the Livongo for Diabetes program on hemoglobin A1c (HbA1c), blood glucose ranges, and depression screening survey results (Patient Health Questionnaire-2 [PHQ-2]) were assessed over 12 months in a prospective cohort recruited from the University of South Florida Health Diabetes Home for Healthy Living. Any patient ?18 years old with a diagnosis of diabetes was approached for voluntary inclusion into the program. The analysis was a pre-post design for those members enrolled in the study. Data was collected at outpatient clinic visits and remotely through the Livongo glucose meter.ResultsA total of 86 adults were enrolled into the Livongo for Diabetes program, with 49% (42/86) female, an average age of 50 (SD 15) years, 56% (48/86) with type 2 diabetes mellitus, and 69% (59/86) with insulin use. The mean HbA1c drop amongst the group was 0.66% (P=.17), with all participants showing a decline in HbA1c at 12 months. A 17% decrease of blood glucose checks <70 mg/dL occurred concurrently. Participants with type 2 diabetes not using insulin had blood glucose values within target range (70-180 mg/dL) 89% of the time. Participants with type 2 diabetes using insulin were in target range 68% of the time, and type 1 diabetes 58% of the time. Average PHQ-2 scores decreased by 0.56 points during the study period.ConclusionsParticipants provided with a cellular-enabled blood glucose meter with real-time feedback and access to coaching from a certified diabetes educator in an outpatient clinical setting experienced improved mean glucose values and fewer episodes of hypoglycemia relative to the start of the program.

Project description:Despite recent interest in individual differences in psychological meanings of consumer brands, the concept of psychological employer brand as a factor independent of particular brands has not been examined. Drawing on an instrumental-symbolic framework, person-organization fit literature, and theory and research on salary and materialism, and combining consumer brand approaches with motivated identity construction theory, we examine the role of materialism and identity-motives-based inclination for the self-employer brand relationship in the situation of a dilemma between two job offers: one proposed by a strong employer brand with an unattractive salary and one from a subjectively weak brand with an attractive salary. A homogenous sample of 101 university students in academic fields related to financial careers participated in a quasi-experimental study. We found that participants preferred the offer from the weak employer brand with an attractive salary compared to the strong employer brand with an unattractive salary; however, supporting our hypothesis, those who preferred this offer anticipated lower job satisfaction. Following expectations, materialism negatively and inclination for self-employer brand relationship positively predicted preferences and evaluations of the unattractive salary offer proposed by the strong employer brand. However, materialism negatively predicted anticipated job satisfaction regarding this offer, as well as positively predicting evaluation of the weak brand with attractive salary job offer. Despite all the detailed hypotheses not being supported, the findings confirm the role of materialism in job offer preference and introduce the inclination to develop an identity-motives-based relationship with an employer brand as an important factor in reactions toward different employer branding recruitment strategies. We discuss the results in light of previous theories and research on person-organization fit, materialism, and brand effects, and consider potential short- and long-term outcomes of recruitment strategies.
