Do Overcrowding and Turnover Cause Violence in Prison?

ABSTRACT: Violence is common in prison and its individual risk factors are well documented. However, there is a mixed evidence on the relationship between prison violence and institutional factors, such as overcrowding and turnover, and recent research suggested that these factors may not be important or relevant. This study investigated the association between prison violence and institutional factors in a Swiss pre-trial prison between 2013 and 2018. Measures included violence (assaults requiring immediate medical attention) as well as the annual overcrowding and turnover rates. Using a meta-regression, the results showed that prison violence was higher when overcrowding and turnover increased. Overall, our study highlighted that institutional prison factors might have notable detrimental effects on prison life. Reduction of prison overcrowding and turnover appear critical to reduce prisoners' vulnerability. Turning prison into safe places designed to promote desistance would probably not be achievable without considering these crucial factors.

Project description:BackgroundTransmission of hepatitis C virus (HCV) among the prisoner population is most frequently associated with sharing of non-sterile injecting equipment. Other blood-to-blood contacts such as tattooing and physical violence are also common in the prison environment, and have been associated with HCV transmission. The context of such non-injecting risk behaviours, particularly violence, is poorly studied. The modified social-ecological model (MSEM) was used to examine HCV transmission risk and violence in the prison setting considering individual, network, community and policy factors.MethodsThe Australian Hepatitis C Incidence and Transmission Study in prisons (HITS-p) cohort enrolled HCV uninfected prisoners with injecting and non-injecting risk behaviours, who were followed up for HCV infection from 2004-2014. Qualitative interviews were conducted within 23 participants; of whom 13 had become HCV infected. Deductive analysis was undertaken to identify violence as risk within prisons among individual, network, community, and public policy levels.ResultsThe risk context for violence and HCV exposure varied across the MSEM. At the individual level, participants were concerned about blood contact during fights, given limited scope to use gloves to prevent blood contamination. At the network level, drug debt and informing on others to correctional authorities, were risk factors for violence and potential HCV transmission. At the community level, racial influence, social groupings, and socially maligned crimes like sexual assault of children were identified as possible triggers for violence. At the policy level, rules and regulations by prison authority influenced the concerns and occurrence of violence and potential HCV transmission.ConclusionContextual concerns regarding violence and HCV transmission were evident at each level of the MSEM. Further evidence-based interventions targeted across the MSEM may reduce prison violence, provide opportunities for HCV prevention when violence occurs and subsequent HCV exposure.

Project description:BackgroundLevels of mental disorder, self-harm and violent behaviour are higher in prisons than in the community. The purpose of this study was to determine whether a brief peer-led problem-support mentor intervention could reduce the incidence of self-harm and violence in an English prison.MethodsAn existing intervention was adapted using a theory of change model and eligible prisoners were trained to become problem-support mentors. Delivery of the intervention took two forms: (i) promotion of the intervention to fellow prisoners, offering support and raising awareness of the intervention but not delivering the skills and (ii) delivery of the problem-solving therapy skills to selected individual prisoners. Training and intervention adherence was measured using mentor log books. We used an Interrupted Time Series (ITS) design utilizing prison data over a 31 month period. Three ITS models and sensitivity analyses were used to address the impact across the whole prison and in the two groups by intervention delivery. Outcomes included self-harm and violent behaviour. Routine data were collected at monthly intervals 16 months pre-, 10 months during and six months post-intervention. Qualitative data measured the acceptability, feasibility, impact and sustainability of the intervention. A matched case-control study followed people after release to assess the feasibility of formal evaluation of the impact on re-offending up to 16 months.FindingsOur causal map identified that mental health and wellbeing in the prison were associated with environmental and social factors. We found a significant reduction in the incidence of self-harm for those receiving the full problem-solving therapy skills. Project description:Overcrowding has been regarded as indicating material deprivation and treated as a proxy measure for individual socioeconomic status. Conventionally, 'persons per room' (PPR) has been employed to identify overcrowded households in UK survey data, though the 'bedroom standard' (BS) approach or the 'modified bedroom standard' (MBS) approach has been thought to capture overcrowded households better. Little is known about which overcrowding measure will perform well in regard to construct and face validity. In this work, associations between three overcrowding measures and socioeconomic (income and household tenure status) and health (satisfied with health and GHQ12) indicators were assessed, using the UK Household Longitudinal Study Wave 6 data. PPR, BS and MBS were derived using relevant housing grid information and housing information from the dataset, which were aggregated at a household level (N?=?18,848). Raw scores were categorised into 'under occupied (rooms?<?people)', 'balanced (rooms?=?people)', 'overcrowded (rooms?<?people)' according to an established cut-off point for each overcrowding measure. Kappa coefficient was used to assess the level of agreement between overcrowding measures. Construct validity of the measures were tested against log-transformed household equivalised income and housing tenure status as well as with each component of overcrowding measures. Using individual data (N?=?38,455), face validity of the overcrowding measures was tested against satisfaction with health and mental health indicated by GHQ12. Each overcrowding measure has a fair agreement with the others (kappa?=?0.44, p<0.001). All overcrowding measures were significantly correlated with income and household tenure in a similar manner. However, components of overcrowding measures were associated differently to these socioeconomic indicators, while they were better correlated with satisfaction with health compared to GHQ12, showing a complex aspect of overcrowding measures. In sum, use of PPR as a socioeconomic indicator is reasonable. However, given the complexity of the mechanism of health inequalities, the relevant household information is required to understand the link.
