Data for vancomycin elution, activity and impact on mechanical properties when incorporated into orthopedic bone cement.

ABSTRACT: In this article, we report data on the antibiotic elution and efficacy, and mechanical properties of Palacos bone cement with different amounts of added vancomycin (0.0, 0.125, 0.25, 0.5, 1.0, 2.0?g), see "Vancomycin elution, activity and impact on mechanical properties when added to orthopedic bone cement" (Bishop et al., 2018) [1]. Mechanical testing was performed for four-point bending, compression, and fracture toughness. The release characteristics of vancomycin was recorded for up to 60 days to estimate the elution profile. The eluted vancomycin efficacy at eliminating the four most common causative orthopedic implant pathogens is also reported.

Elution and Mechanical Strength of Vancomycin-Loaded Bone Cement: In Vitro Study of the Influence of Brand Combination.

Project description:Antibiotic-loaded bone cement (ALBC) is widely used in orthopaedic surgery for both prevention and treatment of infection. Little is known about the effect of different brand combinations of antibiotic and bone cement on the elution profile and mechanical strength of ALBC. Standardized specimens that consisted of one of the 4 brands of bone cement and one of the 3 brands of vancomycin were fashioned, producing 12 combinations of ALBC. Two dosages of vancomycin in 40g bone cement were used to represent the high (4g vancomycin) and low (1g vancomycin) dose groups. Concentrations of vancomycin elution from ALBC was measured for up to 336 hours. The ultimate compression strength was tested at axial compression using a material testing machine before and after elution. In both high-dose and low-dose groups, Lyo-Vancin in PALACOS bone cement resulted in the highest cumulative elution and Vanco in Simplex P bone cement resulted in the lowest elution (458% and 65% higher in high- and low-dose groups, respectively). The mechanical strength was not significantly compromised in all groups with low dose vancomycin (range: 70.31 ± 2.74 MPa to 87.28 ± 8.26MPa after elution). However, with the addition of high dose vancomycin, there was a mixed amount of reduction in the ultimate compression strength after cement aging, ranging from 5% (Vanco in Simplex P, 81.10 ± 0.48 MPa after elution) to 38% (Sterile vancomycin in CMW, 60.94 ± 5.74 MPa after elution). We concluded that the selection of brands of vancomycin and bone cement has a great impact on the release efficacy and mechanical strength of ALBC.

Assessing the Rheological, Mechanical, and Photocatalytic Properties of Niobium Oxide-Incorporated White Cement Pastes.

Project description:Niobium oxide (Nb2O5) is a semiconductor that exhibits photocatalytic properties, making it potentially valuable in addressing air pollution, self-cleaning, and self-disinfection in cement-based materials (CBMs). Therefore, this study aimed to evaluate the impact of different Nb2O5 concentrations on various parameters, including rheological characteristics, hydration kinetics (measured using isothermal calorimetry), compressive strength, and photocatalytic activity, specifically in the degradation of Rhodamine B (RhB) in white Portland cement pastes. The incorporation of Nb2O5 increased the yield stress and viscosity of the pastes by up to 88.9% and 33.5%, respectively, primarily due to the larger specific surface area (SSA) provided by Nb2O5. However, this addition did not significantly affect the hydration kinetics or the compressive strength of the cement pastes after 3 and 28 days. Tests focusing on the degradation of RhB in the cement pastes revealed that the inclusion of 2.0 wt.% of Nb2O5 was insufficient to degrade the dye when exposed to 393 nm UV light. However, an interesting observation was made concerning RhB in the presence of CBMs, as it demonstrated a degradation mechanism that was not dependent on light. This phenomenon was attributed to the production of superoxide anion radicals resulting from the interaction between the alkaline medium and hydrogen peroxide.

Acrylic Bone Cement Incorporated with Low Chitosan Loadings.

Project description:Despite the potential of acrylic bone cement (ABC) loaded with chitosan (CS) for orthopedic applications, there are only a few in vitro studies of this composite with CS loading ? 15 wt.% evaluated in bioactivity tests in simulated body fluid (SBF) for duration > 30 days. The purpose of the present work was to address this shortcoming of the literature. In addition to bioactivity, a wide range of cement properties were determined for composites with CS loading ranging from 0 to 20 wt.%. These properties included maximum exotherm temperature (Tmax), setting time (tset), water contact angle, residual monomer content, flexural strength, bending modulus, glass transition temperature, and water uptake. For cement with CS loading ? 15 wt.%, there was an increase in bioactivity, increase in biocompatibility, decrease in Tmax, increase in tset, all of which are desirable trends, but increase in residual monomer content and decrease in each of the mechanical properties, with each of these trends, were undesirable. Thus, a composite with CS loading of 15 wt.% should be further characterized to explore its suitability for use in low-weight-bearing applications, such as bone void filler and balloon kyphoplasty.

Contrasting effects of physical wear on elution of two antibiotics from orthopedic cement.

Project description:The use of antibiotics as a supplement to bone cement for the purposes of providing a local release of antibiotics is common practice in arthroplasty surgery and the kinetics of elution of the antibiotics in such systems have been investigated previously. However, in these previous studies no account was taken of the potential effects that wear may have on the elution kinetics of the antibiotic. Here, we have modified an existing wear testing rig to allow the simultaneous study of the elution kinetics of bone cement samples containing antibiotics being subjected to immersion only and immersion and conjoint wear. The results show contrasting effects with two commonly used antibiotics. Bone cement containing daptomycin showed no substantial change in antibiotic elution due to wear, while cement containing gentamicin (the most commonly used antibiotic in this application) in contrast demonstrated a substantial reduction in the rate of antibiotic elution when wear was applied. Scanning electron microscopy revealed a possible explanation for these diverse results, due to wear-induced "sealing" of the surface in conjunction with the crystal morphology of the antibiotic.

Influence of Nano-HA Coated Bone Collagen to Acrylic (Polymethylmethacrylate) Bone Cement on Mechanical Properties and Bioactivity.

Project description:This research investigated the mechanical properties and bioactivity of polymethylmethacrylate (PMMA) bone cement after addition of the nano-hydroxyapatite(HA) coated bone collagen (mineralized collagen, MC).The MC in different proportions were added to the PMMA bone cement to detect the compressive strength, compression modulus, coagulation properties and biosafety. The MC-PMMA was embedded into rabbits and co-cultured with MG 63 cells to exam bone tissue compatibility and gene expression of osteogenesis.15.0%(wt) impregnated MC-PMMA significantly lowered compressive modulus while little affected compressive strength and solidification. MC-PMMA bone cement was biologically safe and indicated excellent bone tissue compatibility. The bone-cement interface crosslinking was significantly higher in MC-PMMA than control after 6 months implantation in the femur of rabbits. The genes of osteogenesis exhibited significantly higher expression level in MC-PMMA.MC-PMMA presented perfect mechanical properties, good biosafety and excellent biocompatibility with bone tissues, which has profoundly clinical values.

Evaluation of the Effect of Ciprofloxacin and Vancomycin on Mechanical Properties of PMMA Cement; a Preliminary Study on Molecular Weight.

Project description:Antibiotic-loaded bone cement (ALBC) is commonly used in joint replacement therapy for prevention and treatment of bone infection and mechanical properties of the cement is still an important issue. The effects of ciprofloxacin and vancomycin was investigated on mechanical characterization of PMMA bone cement. Different properties of cement containing (0, 2.5, 5 and 10% W/W) antibiotics, including compressive and bending properties, microstructural, porosity and density were evaluated. Both antibiotics significantly reduced the density values and mechanical properties (compressive and flexural strength and modulus) in all groups in comparison to control over first two weeks (p?<?0.05). This reduction was due to increased porosity upon antibiotic addition (3.05 and 3.67% for ciprofloxacin and vancomycin, respectively) in comparison to control (2.08%) (p?<?0.001) and exposure to aqueous medium. Vancomycin as antibiotic with higher molecular weight (MW?=?1485) had significant effect on compressive strength reduction of the cement at high amount compared to ciprofloxacin (MW?=?367) (P?<?0.01), there was no difference between two antibiotics at lower concentrations (P?>?0.05). The effect of antibiotic loading is both molecular weight and drug content dependent. The time is also an important parameter and the second week is the probably optimum time to study mechanical behavior of ALBC.

Magnesium-alloy rods reinforced bioglass bone cement composite scaffolds with cortical bone-matching mechanical properties and excellent osteoconductivity for load-bearing bone in vivo regeneration.

Project description:Various therapeutic platforms have been developed for repairing bone defects. However, scaffolds possess both cortical bone-matching mechanical properties and excellent osteoconductivity for load-bearing bone defects repair is still challenging in the clinic. In this study, inspired by the structure of the ferroconcrete, a high-strength bifunctional scaffold has been developed by combining surface-modified magnesium alloy as the internal load-bearing skeleton and bioglass-magnesium phosphate bone cement as the osteoconductive matrix. The scaffold combines the high mechanical strength and controllable biodegradability of surface-modified magnesium alloy with the excellent biocompatibility and osteoconductivity of bioglass-magnesium phosphate bone cement, thus providing support for load-bearing bone defects and subsequently bone regeneration. The scaffolds generate hydroxyapatite (HA) during the degrading in simulated body fluid (SBF), with the strength of the scaffold decreasing from 180 to 100 MPa in 6 weeks, which is still sufficient for load-bearing bone. Moreover, the scaffolds showed excellent osteoconductivity in vitro and in vivo. In a New Zealand White Rabbit radius defect model, the scaffolds degrade gradually and are replaced by highly matured new bone tissues, as assessed by image-based analyses (X-ray and Micro-CT) and histological analyses. The bone formation-related proteins such as BMP2, COL1a1 and OCN, all showed increased expression.

How do porosity-inducing techniques affect antibiotic elution from bone cement? An in vitro comparison between hydrogen peroxide and a mechanical mixer.

Project description:BACKGROUND: Increasing the porosity of an antibiotic-loaded cement spacer increases the antibiotic elution, but the correlation between porosity and antibiotic elution is not well documented. The purposes of this study was to attempt new porosity-increasing methods and to investigate the correlation between antibiotic elution and both total and surface porosity. MATERIALS AND METHODS: Five types of antibiotic-loaded bone cement (ALBC) using 2 g cefazolin and 40 g cement were prepared. Other than manual mixing, hydrogen peroxide was used as a foaming agent and a mixing drill piece was used as a mechanical device to try to induce porosity when mixing the cement. Elution of antibiotic into phosphate-buffered saline was measured from 1 h to 1 week. Surface porosity was calculated from density values which were measured with a density kit and an electronic balance, while total porosity was quantified using micro-computed tomography. RESULTS: When a mixing drill piece was used to induce porosity, we observed a significant increasin antibiotic elution compared to a manually mixed ALBC. On the other hand, hydrogen peroxide reduced the elution significantly. Mild correlation between the total amount of cluted in 1 week antibiotic elution and total porosity was observed. CONCLUSIONS: In terms of improving elution, the mixing drill piece seemed to be efficient. A relationship between surface porosity and elution efficacy was not observed.
