Project description:Interventions: Trial participants will be allocated to either the intervention or control group. There is currently only one DACC dressing (Leukomed Sorbact) available in the wound care dressing. The intervention group will receive the DACC containing dressings following surgery (Leukokmed Sorbact). The DACC dressing is a cover dressing applied over the incision site The control group will receive a dressing that does not contain DACC and has the same dressing properties without the interventional dressing characteristics. Participants allocated to the control group will receive a non DACC dressing. The wear time for both groups will be 14 days, unless otherwise clinical indicated for a dressing change and assessment by the lead PI. Dressing application and wear time is to be conducted according to the manufacturer specifications. Dressings will be applied as per standard of post operative care. Dressings will be visually checked daily by the study lead wound care clinician.
Primary outcome(s): The primary outcome will be determined by clinical assessment and diagnosis as well as patient reported outcome. Post discharge surveillance using telemedicine and a phone call at 3o days will be conducted on all trial participants. The primary outcome measurement is defined as the occurrence of surgical site infection as per Centres of Disease Control definition and coding ICD-11 T81.49. [Day 30 postoperative ];The primary outcome measurement is defined as the occurrence of surgical wound dehiscence as defined by World Union of Wound Healing Societies definition and coding ICD 11: T81.3. Post discharge surveillance will be conducted using telemedicine and a phone call at 30 days after surgery. [Day 30 postoperative. ]

Project description:Butyrylcholinesterase (BChE) is an α-glycoprotein synthesized in the liver. BchE’s serum level decreases in many clinical conditions such as acute and chronic liver damage, inflammation, injury and infections, and malnutrition. The Investigators prospectively evaluate patients undergoing elective procedures for colorectal diseases. Blood samples are collected preoperatively (at day 0), post-operatively in the recovery room (day 1), and on the subsequent four days (days 2, 3, 4, and 5) for assessment of BChE, C-reactive protein, and white blood cell concentrations. The same surgical team operates all patients and is blinded to the study. Patients are monitored for post-operative infection by using standard laboratory and clinical methods. If surgical site infection (SSI) is suspected the wound is swabbed and empirical antibiotics are started. The aim of the current trial is to study whether BChE is a reliable marker for the presence of SSI in patients undergoing colorectal surgery.

Project description:A campaign for malaria control, using Long Lasting Insecticide Nets (LLINs) was launched in South Sudan in 2009. The success of such a campaign often depends upon adequate available resources and reliable surveillance data which help officials understand existing infections. An optimal allocation of resources for malaria control at a sub-national scale is therefore paramount to the success of efforts to reduce malaria prevalence. In this paper, we extend an existing SIR mathematical model to capture the effect of LLINs on malaria transmission. Available data on malaria is utilized to determine realistic parameter values of this model using a Bayesian approach via Markov Chain Monte Carlo (MCMC) methods. Then, we explore the parasite prevalence on a continued rollout of LLINs in three different settings in order to create a sub-national projection of malaria. Further, we calculate the model's basic reproductive number and study its sensitivity to LLINs' coverage and its efficacy. From the numerical simulation results, we notice a basic reproduction number, [Formula: see text], confirming a substantial increase of incidence cases if no form of intervention takes place in the community. This work indicates that an effective use of LLINs may reduce [Formula: see text] and hence malaria transmission. We hope that this study will provide a basis for recommending a scaling-up of the entry point of LLINs' distribution that targets households in areas at risk of malaria.

Project description:Memory CD8 T lymphocytes are a long-lived population of immune cells that provide formidable protection against infections and malignancy, and are thus often central to vaccine and cancer immunotherapy efficacy. Here we refine the identity, developmental relationships, and functional roles of memory CD8 T cells through delineation of a descrete population that confers robust protection against reinfection and exhibits an unexpected molecular program bearing typically divergent characteristics of short-lived effector cells and long-lived memory T cells. Mass-cytometry and single-cell RNA sequencing analyses revealed an analogous population of cells in humans. These findings hold broad implications for both informing immunotherapy treatments and predicting their efficacy.

Project description:Background: Malaria is a public health problem in parts of Thailand, where Plasmodium falciparum and Plasmodium vivax are the main causes of infection. In the northwestern border province of Tak parasite prevalence is now estimated to be less than 1% by microscopy. Nonetheless, microscopy is insensitive at low-level parasitaemia. The objective of this study was to assess the current epidemiology of falciparum and vivax malaria in Tak using molecular methods to detect exposure to and infection with parasites; in particular, the prevalence of asymptomatic infections and infections with submicroscopic parasite levels. Methods: Three-hundred microlitres of whole blood from finger-prick were collected into capillary tubes from residents of a sentinel village and from patients at a malaria clinic. Pelleted cellular fractions were screened by quantitative PCR to determine parasite prevalence, while plasma was probed on a protein microarray displaying hundreds of P. falciparum and P. vivax proteins to obtain antibody response profiles in those individuals. Results: Of 219 samples from the village, qPCR detected 25 (11.4%) Plasmodium sp. infections, of which 92% were asymptomatic and 100% were submicroscopic. Of 61 samples from the clinic patients, 27 (44.3%) were positive by qPCR, of which 25.9% had submicroscopic parasite levels. Cryptic mixed infections, misdiagnosed as single-species infections by microscopy, were found in 7 (25.9%) malaria patients. All sample donors, parasitaemic and non-parasitaemic alike, had serological evidence of parasite exposure, with 100% seropositivity to at least 54 antigens. Antigens significantly associated with asymptomatic infections were P. falciparum MSP2, DnaJ protein, putative E1E2 ATPase, and three others.
