Role of Tibial Nerve Stimulation for Enhanced Postoperative Recovery After Colorectal Surgery

ABSTRACT: Aim of the study is to evaluate the role of postoperative tibial nerve stimulation in the enhancement of the recovery of bowel function and in shortening the hospital stay.

Modulation of brain dead induced inflammation by vagus nerve stimulation

Project description:Because the vagus nerve is implicated in control of inflammation, we investigated if brain death causes impairment of the parasympathetic nervous system, hence contributing to inflammation. Brain death (BD) was induced in rats. Anaesthetised ventilated rats (NBD) served as control. Heart rate variability (HRV) was assessed by ECG. The vagus nerve was electrically stimulated (BD+STIM) during BD. Intestine, kidney, heart and liver were harvested after 6h. Affymetrix chip- analysis was performed on intestinal RNA. Quantitative PCR was performed on all organs. Serum was collected to assess TNFÎ± concentrations. Renal transplantations were performed to address the influence of vagus nerve stimulation on graft outcome. HRV was significantly lower in BD animals. Vagus nerve stimulation inhibited the increase in serum TNFÎ± concentrations and resulted in down-regulation of a multiplicity of pro-inflammatory genes in intestinal tissue. In renal tissue vagal stimulation significantly decreased the expression of E-selectin, IL1Î² and ITGA6. Renal function was significantly better in recipients that received a graft from a BD+STIM donor. Our study demonstrates impairment of the parasympathetic nervous system during BD and inhibition of serum TNFÎ± through vagal stimulation. Vagus nerve stimulation variably affected gene expression in donor organs and improved renal function in recipients. Project description:Purpose: To decipher the mechanisms responsible for the protective function of vagus nerve stimulation. Methods: Blood was collected from Control and Treated patients followed by isolation of PBMC'S. Total RNA was extracted and RNA sequencing was done following standard protocols Results: Detailed analysis of the transcriptomic data show that anti-inflammatory responses were upregulated in the treated samples. Conclusions: This study showed the protective role of vagus nerve stimulation in epilepsy patients.

Efficiency of transanal irrigation and stimulation of a nerve of the ankle to improve symptoms and quality of life after radical surgery of rectal cancer

Project description:Background and study aims Rectal cancer (cancer of the rectum) affects the lower part of the colon that connects to the anus. In many patients undergoing surgery to remove rectal cancer (and who have also received radiotherapy) the anus is not removed and patients may continue to defecate through this natural route. However, the removal of all or part of the rectum can cause faecal incontinence (involuntary leakage of faeces), urgency (must urgently seek a toilet), and fragmentation of the stool which forces them to perform small and repeated defecations. There is no specific treatment that improves these symptoms. This study compares the effects of two non-invasive and non-painful procedures for this situation. Transanal irrigation using a mechanical device (Peristeen®) attempts to empty the stool at once, allowing a normal life until the bowel is filled again with faeces. Electrical stimulation through a nerve in the leg (tibial nerve) can be used to improve the sensitivity and strength of the muscles of the pelvis and improve their ability to contain and expel the stool. The aim of this study is to compare the effects of Peristeen and tibial nerve stimulation in patients who have problems with continence and expulsion of feces that affect their quality of life and daily activities. Who can participate? Patients aged 20 to 80 who have been operated on for rectal cancer at least one year before the study What does the study involve? Participants are randomly allocated to be treated with either transanal irrigation using Peristeen® or posterior tibial nerve stimulation. The severity of their symptoms is recorded at the start of the study and after 12, 18 and 24 weeks.

Immunomodulation by the combination of statin and Matrix-Bound Nanovesicle enhances optic nerve regeneration

Project description:To identify a novel clinically-relevant therapy, fluvastatin and matrix bound nanovesicles (MBV) were focused in this study. We found that fluvastatin and MBV synergistically enhance optic nerve regeneration and RGC protection after optic nerve injury in mouse. To identify the molecular mechanisms underlying enhancement of RGC protection and axon regeneration after optic nerve injury, bulk RNA sequencing analysis of the whole retina treated with MBV, fluvastatin and the combination of MBV and fluvastatin or PBS as a control two days after optic nerve crush, was performed. Bulk RNA-seq analysis revealed that genes and cellular pathways related to inflammation, were robustly altered.

Impact of the Enhanced Recovery After Surgery System for Colorectal Surgery

Project description:This is an observational study to look at the impact of the Enhanced Recovery After Surgery system for colorectal surgery (Group 1) in shortening hospital length of stay, reducing postoperative narcotic consumption, lowering Visual Analog Scale scores, decreasing the incidence of postoperative nausea and vomiting, and reducing 30-day readmission when compared to patients who had colorectal surgery performed at UAMS prior to the implementation of the Enhanced Recovery After Surgery system (Group 2). This is a retrospective study using de-identified records and therefore will not require subject enrollment and is NOT Human Subjects Research.
