Project description:This study aims to demonstrate that a preoperative combination of mechanical bowel preparation and oral antibiotics, before elective laparoscopic colon cancer surgery, is associated with a reduction of postoperative surgical site infection rate, as compared to mechanical bowel preparation alone, oral antibiotics alone, or no colonic preparation. Project description:Surgical site infection (SSI) is a major postoperative complication after abdominal surgery especially in colorectal field, which significantly increases length of stay (LOS), readmission incidence and expense. Therefore, identification of the effective method to reduce SSI incidence is critically important. Combination of oral antibiotics and mechanical bowel preparation was reported with lower SSIs and LOS in some retrospecitve data analysis, however a prospective randmized controlled trial was absent. Herein, the current randomized controlled trial comparing MBP+OA with MBP alone in postoperative complications in order to guide clinical practise was conducted.

Project description:Intervention1: Oral antibiotic bowel preparation: All the patients in this arm in addition to the bowel preparation solution ( 45 ml of Exelyte bowel preparation solution ( monobasic sodium phosphate dihydrate 24.417 g, disodium hydrogen orthophosphate dihydrate 5.439 g) to be consumed at 8 am and 4 pm ), will also receive 3 tablets each of, 1 gm of Erythromycin Estolate and 400 mg of Metronidazole in the package, to be taken at 1pm, 2pm and 11 pm, on the day prior to surgery Control Intervention1: Mechanical Bowel preparation : Exelyte: 2 bottles of 45 ml of Exelyte bowel preparation solution ( monobasic sodium phosphate dihydrate 24.417 g, disodium hydrogen orthophosphate dihydrate 5.439 g) to be consumed at 8 am and 4 pm on the day prior to surgery. Primary outcome(s): . Risk assessment with gene expression markers in sepsis development

Project description:We investigated the individual phenotypic predisposition to developing uncomplicated infection or sepsis in a large cohort of non-infected patients undergoing major elective surgery. Whole blood RNA sequencing analysis was performed on preoperative samples taken from 267 patients. These comprised patients who developed postoperative infection with (n=77) or without (n=49) sepsis, non-infectious systemic inflammatory response (n=31), or an uncomplicated postoperative course (n=110). Machine learning classification models built on preoperative transcriptomic signatures predicted postoperative outcomes including sepsis.
