ABSTRACT: Study hypothesis: Comparisons of attendance, detection rates and acceptability of total colonoscopy, flexible sigmoidoscopy and fecal occult blood test (FOBT) as primary screening tests for colorectal cancer. Primary outcome(s): Attendance and detection rate to total colonoscopy, flexible sigmoidoscopy and FOBT as primary screening tests.

Participation and Detection Rate of Screening CT Colonography and Screening Sigmoidoscopy

Project description:The objectives of this multicenter, randomized trial is to assess the participation rate achievable through two different screening strategies (Computed Tomographic Colonography-CTC and sigmoidoscopy-FS), to compare detection rate of colorectal cancer (CRC) and advanced adenoma of tests and to evaluate their costs. The role of Computer-aided detection (CAD) for CTC screening will be also assessed. The trial involves 10 Italian centers located in the Piedmont Region and in Verona. Residents aged 58-60 years in those districts are target for recruitment. Exclusion criteria include: previous diagnosis of cancer or adenoma; family history or hereditary syndromes; personal history of inflammatory bowel disease; patients screened by colonoscopy or FOBT within 2 years; severe disease.

Peppermint oil for flexible sigmoidoscopy pre-medication

Project description:Interventions: All attenders at a flexible sigmoidoscopy based colorectal cancer screening group are invited to participate in this substudy. Participants are randomly allocated to receive a peppermint oil capsule taken orally at least 30 minutes prior to undergoing sigmoidoscopy. Additionally, the same participants are randomly allocated to recieve one (standard care) or two phosphate enemas per rectum as bowel preparation also at least 30 minutes prior toprior to sigmoidoscopy. Both interventions are single administrations only on the day of sigmoidoscopy. Primary outcome(s): Adenoma detection rate at sigmoidoscopy. Multicentre randomised trial of ‘once only’ flexible sigmoidoscopy screening for prevention of bowel cancer morbidity and mortality

Project description:Background and study aims Bowel cancer is the fourth most commonly diagnosed cancer in the UK. Bowel cancers develop slowly from common bowel growths or polyps, so removing them can help prevent cancer. Screening with flexible sigmoidoscopy (also called ‘bowel scope’ or Flexi-scope) involves insertion of a thin flexible tube with a light and camera on the end into the bowel to examine the inner surface. Small bowel polyps can be removed during screening. The aim of the UK Flexible Sigmoidoscopy Screening Trial (UKFSST) was to determine whether having just one flexible sigmoidoscopy screen at around 60 years of age could prevent bowel cancer from developing and reduce the number of deaths from bowel cancer. The trial also aimed to determine how long any benefit lasts, and what is the best age to do the screening examination. Who can participate? Men and women who were aged 55–64 years and registered at a participating GP practice between November 1994 and March 1999. What does the study involve? The UKFSST was a randomised controlled trial. People who participated in the trial were randomly assigned to receive either flexible sigmoidoscopy screening or no screening (which was the usual care offered at the time of the trial).

Screening for CRC Using a Mixed Strategy of Sigmoidoscopy and Colonoscopy in Average-Risk Population According to Age

Project description:BACKGROUND: Primary screening with sigmoidoscopy would miss a substantial proportion of advanced proximal neoplasia (APN), but screening with universal colonoscopy is costly. The aim of this study is to assess the efficacy of mixed strategy which uses sigmoidoscopy for younger patients and colonoscopy for older patients. MATERIALS and METHODS: We analyzed an established database containing consecutive average-risk adults aged 50 or older who underwent screening colonoscopy as part of health check-up. We assessed the efficacy of mixed screening strategy using colonoscopy for persons aged at and above a certain cut-off age and sigmoidoscopy for persons aged below that age. Those who underwent sigmoidoscopy initially would be referred for subsequent colonoscopy if distal sentinel lesion was detected.

Evaluation of the Impact of Water Method During Insertion Phase of Flexible Sigmoidoscopy in Training

Project description:Colonoscopy is a sedated procedure traditionally performed using air insufflation during the insertion phase of the procedure. Recently, the use of water method (eg, water infusion or water exchange techniques) during the insertion phase of colonoscopy has been reported to increase the proportion of patients in whom complete unsedated colonoscopy could be achieved, reduce patient recovery time burdens, decrease abdominal discomfort during and after colonoscopy, enhance cecal intubation, and increase willingness to repeat an unsedated colonoscopy. However, there has been no study on the use of water method during the training of primary care doctors or nurse endoscopists in flexible sigmoidoscopy for colorectal cancer screening. In unsedated endoscopic procedure such as FS, endoscope insertion techniques that can potentially reduce patient discomfort and increase the rate of achieving an adequate depth of scope insertion are desirable. Our current study aims to evaluate the impact of water method during insertion phase of FS in the training of primary care doctors or nurse endoscopists for colorectal cancer screening.

First approach on the use of samples from fecal occult blood screening kits for metabolomic analysis. Application in colorectal cancer population.

Project description:Cancer is the most common disease around the world and colorectal cancer is the second most common cancer. The early diagnosis of colorectal cancer is difficult and relies on invasive diag-nostic tools such as colonoscopy and tissue biopsy. Other non-invasive techniques such as fecal occult blood screening test (FOBT) are less sensitive and accurate. The advantage of FOBT together with high throughput technology such as metabolomics could provide the advantages of non-invasive tool and the effectiveness of detecting novel colorectal cancer markers. In this way, this work focuses on the novelty of using FOBT as samples to perform metabolomics analysis and its application on colorectal cancer population.
