ABSTRACT: Interventions: Group A. ESD group 1. The solution of a mixture of Glyceol and 1% 1900-KDa hyaluronic acid preparation is injected into the submucosal layer. 2. Mucosal incision is started with the knife for ESD. 3. The lesion is resected with submucosal dissection. Group B. EMR group 1. The solution of normal saline is injected into the submucosal layer. 2. Project description:Interventions: Procedure/Surgery : 1) onestep knife conventional ESD (control) ? Insert the colonoscope with the transparent cap attached. ? After puncturing the submucosal layer immediately around the lesion with a puncture needle for submucosal solution injection, inject the saline-based submucosal solution. ? When the lesion is sufficiently raised, submucosal dissection is performed using a dual knife. ? Retrieve the resected specimen, fix it on a styrofoam fixing plate, and ask the pathology department for tissue analysis. (2) onestep knife + Traction ESD (intermediate group) ? Insert the colonoscope with the transparent cap attached. ? After puncturing the submucosal layer immediately around the lesion with a puncture needle for submucosal solution injection, inject the saline-based submucosal solution. ? When the lesion is sufficiently raised, submucosal dissection is performed using a dual knife. ? When performing ESD, make an incision around the lesion. After fixing the rubber band to the endoscope clip from the outside of the patient, insert it into the channel of the colonoscope and fix the clip at the end of the pre-incision lesion using the clip. Using the second clip, hold the rubber band secured to the pre-fastened first clip and secure the clip in a position that will provide adequate traction. ? Implement additional ESD. ? Brief Title : Optimized Endoscopic Submucosal Dissection With Snaring for Colorectal Neoplasm

Project description:This study is aimed to analyze the outcomes after conventional endoscopic submucosal dissection (ESD) and optimized ESD with snaring (oESD-S) for colorectal neoplasm that is more than 20 mm in diameter of laterllay spreading tumor or flat elevated lesion without stalk. Optimized ESD with snaring means submucosal dissection followed by snaring when narrowed circumference of the remained submucosal tissue beneath the lesion is less than 5 mm in diameter with snaring, then resected by using an electric current. The investigators expect optimized ESD with snaring can provide more time-saving procedure with comparable en-bloc resection rate and perforation rate, when compared with the conventional ESD method.

Human Subperitoneal Fibroblasts and Cancer Cell Interaction Creates Microenvironment Enhancing Tumor Progression and Metastasis

Project description:Fibroblasts isolated from human colon submucosal and subperitoneal layer were stimulated by colon cancer cell line (DLD-1) cultured medium. Peritoneal invasion in colon cancer is an important prognostic factor, and the fibrosis with α-SMA was a significant pathological feature of the cancer microenvironment formed by peritoneal invasion (CMPI). The result indicated that the gene expression of subperitoneal fibroblasts showed more various gene modification than submucosal fibroblasts. And ACTA2 expression was higher in fibroblasts from subperitoneal layer than that from submucosal layer. Together with this concordant stromal protein expression in CMPI, this in vitro model is able to reflect the special microenvironment in CMPI. 3 cases of human colon submucosal and subperitoneal fibroblasts were isolated, and these fibroblasts were stimulated with DLD-1 cultured medium. Total RNA were extracted from these samples and hybridized in Affymetrix microarray to compare their gene expression changes through the DLD-1 stimulation

Project description:Interventions: 1) MLTD-ESD group Colorectal endoscopic submucosal dissection (ESD) is performed using the Multi Loop Traction Device (MLTD). The number of MLTD used in a single session is not limited. The conversion of the allocated therapeutic method is permitted. If the MLTD is ineffective for more than 10 minutes, the MLTD can be removed. 2) Conventional ESD group ESD is completed using a standard therapeutic strategy that does not involve the use of traction devices.
